General: Based on observation, interview and review of documentation, from 8:06am to 8:55am, there was only one (1) staff member working at the child-care center and her certification for CPR/First Aid had expired on 09/13/25. At 8:55am, an additional staff member arrived with certification in CPR/First Aid.

General: Based on review of documentation, staff member (DOH: 08/06/25) had not completed orientation training, which was required within ninety (90) days of employment. A review of ECE-TRIS verified this information.
General: Based on observation, in the One-year-old Classroom, there was a child, aged one-year-old, asleep in a bouncing chair. The surveyor left the room and returned after ten (10) minutes, and the child was still asleep in the bouncing chair.
General: Based on interview and review of documentation, all of the children's files were missing authorization for emergency care and the preferred hospital for care.
General: Based on observation, a basketball goal on the playground had a broken backboard with sharp edges and had fallen over, making it accessible to children.

General: Based on observation, the Preschool Classroom contained an electrical outlet protruding from the wall, which was not flush with the surface, thereby exposing wiring and creating potential access for children.

General: Based on observation, an unfurled, green garden hose was found on the playground, next to a gray water meter. It was located in an area that was part of the playground.
