General: Based on observation, the following was found: 1) The water temperature in the Toddler 1 Classroom sink was one hundred thirty (130) degrees Fahrenheit. 2) The water temperature in the Toddler 3 Classroom sink was one hundred thirty (130) degrees Fahrenheit.
General: Based on review of documentation and interview, the following was found: 1) A staff member (DOH: 12/05/24) did not have documentation of a professional development plan. Interview with the staff in charge found a professional development plan had not been completed for the staff member.

General: Based on observation, there was no indicating thermometer or other temperature measuring device in the refrigerator located in the Nursery 3 Classroom.
General: Based on observation, the following was found: 1) The ceiling vent in the downstairs boy's bathroom was detached from the ceiling and not in good repair. 2) The ceiling vent in Classroom F contained brown stains on the surface and on the ceiling tile surrounding the vent.
General: Based on observation, Classroom F had two (2) stained ceiling tiles.
General: Based on observation, the toilet seat in the downstairs boy's bathroom had peeled coating exposing a brown material underneath. Hygienic Practices In Compliance
General: Based on observation, the following was found: 1) Two (2) sheets were loosely fitted to the mattresses of cribs in the Nursery 2 Classroom. 2) Two (2) sheets were loosely fitted to the mattresses of cribs in the Nursery 3 Classroom.
General: Based on observation, the following was found: Preschool 3 Classroom 1)The blue rug in the reading center was torn and not in good repair. 2)The gray rug near the cubbies was torn and not in good repair. 3)The shelf in the dramatic play area had peeled paint exposing the particleboard. Preschool A Classroom 1) The blue chair in the reading center had several tears exposing the foam and it was not in good repair.
General: Based on interview, on 09/03/25, at approximately 10:40 a.m., a staff member in the Toddler One Classroom reacted inappropriately when a toddler sought her attention. The staff member pushed the child’s hand away in a quick, forceful motion while saying, “Stop pinching me.” The toddler, who had been crying and pinching the staff member to gain attention, began crying more intensely after the interaction. A witness noted that the staff member appeared very irritated and that her demeanor was inappropriate. Management reviewed video footage of the incident and confirmed that the staff member’s behavior was unacceptable. General: Based on review of documentation, one (1) staff member (DOH: 12/05/24) had a negative tuberculin skin test result on file that exceeded two (2) years and therefore was no longer current.
General: Based on observation, a container of spray sanitizer was placed on a toy shelf within the Toddler Three (3) Classroom. The spray was accessible to the children in care.
General: Based on observation, an air conditioning panel was loosely fitted to the unit in the Transitional Area, exposing the interior mechanisms.
General: Based on observation, the following was found: Cafeteria 1.) Several ceiling tiles were missing. Toddler 2 1.) Several ceiling tiles were missing. Toddler 3 1.) General: Based on observation, the following was found: Classroom D 1.) Contained six (6) stained ceiling tiles. Classroom F 1.) Contained three (3) stained ceiling tiles. Bathroom #1 (upstairs) 1.) Contained a missing floor tile around the drain. Bathroom #2 (upstairs) 1.) Contained a stained ceiling tile. Main Hallway 1.) Contained four (4) stained ceiling tiles.
General: Based on observation, the refrigerator in the Infant 3 Classroom did not contain a thermometer; therefore, the temperature could not be determined.
General: Based on review of documentation, there was no evidence that the facility's emergency preparedness plan had been updated/submitted to the proper authority for review at the time of inspection.
General: Based on observation, the sinks within the basement restrooms registered a temperature of one hundred and twenty-eight (128) degrees Fahrenheit when measured with a thermometer.
General: Based on observation, the following was found: Preschool A Classroom 1.) There were several areas of tape residue on the Science/ Music shelf. The tape residue had started to accumulate dirt and grime. Preschool B 1.) The Science and Puzzle shelf had areas of tape residue which had started to accumulate dirt and grime. School-Age One 1.) There were areas of tape residue on the Living Center tables. The tape residue had started to accumulate dirt and grime.
General: Based on observation and interview, during a tour of the Nursery Three (3) Classroom the surveyor observed several cans of formula. The staff-person-in-charge of the classroom described that the facility does provide some formula for the children, but a few parents will bring formula from home in the original canister. The staff person described that for the children that bring formula from home, staff will measure the formula and water. The formula was not brought to the facility prepared by the child’s guardian.
General: Based on observation, during a tour of the Toddler 1 Classroom the surveyor observed that children were served potato chips and water for their A.M. snack. These items do not meet criteria to be counted as two (2) required components.
