General: Based on review of documentation, the following was found: 1) Three (3) children’s files contained immunization records that were no longer current as of: 05/30/25, 10/08/24, and 04/04/25. 2) One (1) child’s file contained an immunization record that did not have a date of expiration; therefore, the surveyor was unable to determine whether the documentation was current. General: Based on observation and a review of documentation, the following was found: 1) The childcare center exceeded capacity by two (2) children on 09/11/25. Fourteen (14) children were present during the inspection process, exceeding the approved capacity by two (2) students. 2) The surveyor reviewed the sign in and out logs for the children in care and found that sixteen (16) students were signed in on 09/09/25. One (1) school-aged child had a documented arrival time of 7:05 am and again at 2:45 pm. It is unknown what time this child had left for school. By 8:45 am sixteen (16) children were signed into the program. The sixteenth child arrived at 1:00 pm. The first child was not picked up by their guardian until 3:00 pm. The childcare center exceeded approved capacity on this date by four (4) students.
General: Based on observation, on 09/11/25 the surveyor entered the childcare center 10:21 am and observed fourteen (14) children in the care of two (2) staff members. The youngest child in the group was seven (7) months-old. The ratio was exceeded by four (4) children for the duration of the inspection. General: Based on observation and review of documentation, the surveyor entered the childcare center at 10:21 am and observed the two (2) staff members and fourteen (14) children playing on the driveway of the home and riding on toys on the sidewalk by the road. There were no barriers in place to prevent the children from accessing the road. It is important to note that this area was is not licensed for childcare and written permission was not granted for the children to utilize this area.

The surveyor entered the facility at 1:35PM and observed the staff person in charge sitting in the infant/toddler classroom watching Judge Mathis and eating her lunch. Two (2) children were present in this classroom. One (1) child was asleep in a crib and the other was awake in a crib. The staff person in charge stated that she thought it was okay for her to watch T.V. since she was in the infant room.

The surveyor entered the facility at 1:35PM and observed the staff person in charge sitting in the infant/toddler classroom watching Judge Mathis and eating her lunch. It is important to note that the three (3) children asleep in the preschool classroom were not visible from where the staff person in charge was sitting and the lights were off in both classrooms. The surveyor then opened the door and entered the facility which caused a doorbell to ring. When the doorbell rang a staff person from the adjacent preschool room entered the infant/toddler room. The staff person in charge stated during the exit interview that the staff person working in the infant/toddler room must have been relieving her for a bathroom break when the infant was left alone in the infant/toddler classroom. General: Based on Review of Documentation, and interview, this regulatory requirement was not met. The facility had an emergency preparedness plan; however, the staff person in charge stated during interview that she had not submitted the plan to the local emergency management official.
