A substitute staff member (DOH: 08/08/18) completed none of the required fifteen (15) cabinet approved early care and education training hours between 07/01/22 and 06/30/23. General: Based on observation and interview, this regulatory requirement was not met. Upon the surveyor's arrival into the cafeteria at approximately 2:57 PM, all of the children's backpacks, coats, and personal belongings were sitting on the floor against the wall in the cafeteria. The items were stored touching one another. This promotes the spread of germs. At approximately 3:09 PM, the children took their personal belongings to another part of the building and stored them in laundry baskets. The staff in charge indicated that usually from 2:35 PM, (dismissal) the children's personal belongings are stored in the floor of the cafeteria until they separate into groups at around 3:10 PM and at that time the personal belongings are moved to the laundry baskets outside the media center. General: Based on observation, this regulatory requirement was not met. At approximately 3:04 PM, a five-year-old child exited the cafeteria to go to the restroom. The child went to the restroom alone. The restroom is located out the cafeteria door and around the corner from the cafeteria. The staff members in the cafeteria were helping children prepare to move to other areas of the building and separate into smaller groups. The child in the bathroom was not within sight or sound of the staff members in the cafeteria. At approximately 3:07 PM, a staff went to the restroom to check on the child. The five-year-old child was unsupervised for approximately three minutes. The child and staff returned back to the cafeteria at approximately 3:08 PM together. General: Based on Review of Documentation, eight (8) staff members hired on 8-13-14 did not have an annual evaluation on file. Based on interview with the staff in charge, she stated she wasn't sure where they were kept, and thought they were located with staff files.
General: Based on Review of Documentation, and as referenced by ECE-TRIS, nine (9) staff members with the following hire date do not have up to date annual training as required: 8-13-14; eight (8) of these staff members do not have orientation training. Based on interview with the staff person in charge, she stated she was unaware these trainings were to occur on a regular, annual basis.
General: Based on Review of Documentation, there were no employees on file with a current certification in CPR/First Aid. (7) An individual who has been convicted of, or entered an Alford plea or a plea of guilty to, a nonviolent felony or misdemeanor not specified in this section shall be handled on a case-by-case basis by the licensee with consideration given to the: (a) Nature of the offense; (b) Length of time that has elapsed since the event; and (c) Individual’s life experiences after conviction, Alford plea, or guilty plea.

General: Based on interview and review of documentation, upon review of personnel files, the surveyor observed one staff member hired 8/13/15, to be certified in CPR/FIRST aid. This individual left the facility at approximately 3:15 pm on the date of the survey. The surveyor did not observe documentation in the personnel files reviewed for the staff members currently present at the facility to be certified in CPR/First aid training. During the exit conference, the surveyor questioned an individual with an unknown date of hire whether any staff members were currently certified in CPR/FIRST aid. The individual stated that he was certified with CPR and first aid, yet this individual was unable to present any documentation to verify this statement.
General: Based on interview and review of documentation, the staff member in charge stated that the facility receives childcare assistance payments. The individual was unable to present a DCC-94E documenting the children participating in this program.
General: Based on interview and review of documentation, the surveyor asked the staff member in charge to identify current staff members listed from the KARES portal list of current employees. The staff member in charge provided names of two individuals who were not listed on the KARES background check list for the facility. There was no evidence in the employee files indicating a date of hire for these two individuals, nor evidence of submitting fingerprints for review. The surveyor searched the first individual on KARES upon return to the office and did not observe that the background check process was started for this staff member. This staff member was observed to be alone with one group of children at approximately 3:40 pm. Upon KARES review for the second individual listed as currently employed by the staff member in charge, the portal verifies that the background check process was started on 11/7/18, by the staff member in charge but was closed on 11/22/18, for incomplete application.
