General: Based on observation, in the older one-year-old classroom at approximately 9:56 am, the surveyor found a bottle of Lysol toilet bowl cleaner, a plunger, and a toilet bowl brush next to the toilet. It should be noted, children utilize this restroom for handwashing. Therefore, the items were accessible to the children in care.

General: Based on a review of documentation and the Kentucky National Background Check Service (KARES), a staff member (DOH: 03/15/24) currently has a determination status listed as “in process.” At approximately 10:05 am, the surveyor observed this staff member and another staff member caring for nineteen (19) children ages five-ten (5-10) years old. Therefore, at no time during the renewal inspection was the staff member working alone with children. General: Based on interview and observation, the following was found: 1.) A child was able to let himself out of the gate on the playground and supervising staff on the playground did not see him leave the playground; therefore, staff were not providing adequate supervision of the child. Nine (9) children were in the hallway sitting in highchairs unsupervised; these children were ranging from the age of 11 months to 14 months old. General: Based on interview and review of documentation, this regulatory requirement was not met. During the investigation it was found that a staff member let a four-year-old child go to the bathroom alone, around 12:50 pm on 6/30/2022. After approximately five (5) minutes, when this child did not return to the room, staff member went to look for him and discovered that he was not in the bathroom. At that time, the staff in the facility started the search for this child. At approximately 12:59 pm another staff member, who was in the van with her class on the way to the field trip, spotted a child running across the main road and called the facility to make sure all their children are there, and learned that staff members are looking for a child. The staff members in the van came to the child on the side of the road and were able to return him to the facility approximately at 1:06 pm. General: Based on observation, the following was found: 1.) There was a large area of chipped paint in one of the bathroom stalls in the upstairs boys' bathroom. 2.) There was an area of frayed carpet just outside of the boys' restroom located upstairs. 3.) There was what appeared to be paint on the floor under the sensory table in the panda pals room. 4.) There was an area of the carpet that had a marker stain on it. This was located in the panda pals room. 5.) In the tiger cubs room, there was a large black stain on the carpet by the trash can in the science center. 6.) In the upstairs hallway there were numerous black stains on the carpet. 7.) In the toddler classroom, there was a cracked ceiling tile above the CD player. as well as a stained ceiling tile above the storage closet.

General: Based on Observation, it was found that the T-Birds changing table was located in the classroom. The sink is located in the bathroom in the classroom, but the door to the bathroom is kept closed, creating a barrier that must be touched prior to hand washing. It was also found that the water in the hand washing sinks throughout the facility measured one hundred and forty degrees when measured with a calibrated thermometer. General: Based on Observation, there was a roll of plastic garbage bags in an unlocked cabinet under the sink in the upstairs boys' bathroom. The bags were accessible to the children in care.

General: Based on Review of Documentation, there were no children's names listed with the March 2016 earthquake and tornado drill. The director stated that she must not have forgotten to attach a list of the children that were present the day of the drill.

General: Based on Review of Documentation, staff hired on 08/20/02 had only fourteen (14) hours of cabinet approved training on file and has not completed the Pediatric Abuse and Head Trauma class. This information was verified through ECE-TRIS. The director acknowledged these findings.
General: Based on Observation, it was found that a child in the older infant classroom was observed to be eating a cracker while walking around in the classroom. The staff in charge acknowledged this.
General: Based on Observation, it was found that the cover was missing off of the ceiling fan in the women's downstairs restroom used by the children for toileting. It was also found that the two (2) stepping stools in the men's restroom and the two (2) stepping stools in the women's restroom were covered in a substantial amount of dirt and grime. it was found that in the Bridging /Pre-K classroom, the smoke detector was observed to be hanging loose from the ceiling and in the three (3) to four (4) year old classroom there was observed to be a broken electrical plate accessible to the children. The staff in charge acknowledged this.
General: Based on Observation, during a tour of the facility there were hooks with bags and cloth boxes outside some of the classrooms that contained the children's belongings. A paper was observed attached to some of these bags and cloth boxes that had the child's name and information about their day attached with a clip. This information could be viewed by anyone walking through the hallway or by another parent.
General: Based on Observation, during a tour of the younger infant room three (3) water stained ceiling tiles were observed. During a tour of room # 22 a nickel size hole was observed in the wall behind the door with drywall exposed. During a tour of room # 21 there was an area of paint that had been peeled off the wall in the dramatic play area leaving the brown covering of the drywall exposed. In addition there was another area of paint in the same area that had been peeled back but had been taped down with scotch tape.
General: Based on Review of Documentation, it was found that the facility is licensed for transportation. However, there is not a job description for the van driver.
General: Based on Observation, at the time of the survey a group of children ages four (4) to five (5) years old were observed playing outside of the fenced playground in the parking lot. An interview with the director found that somtimes they allow the children to play in the parking lot when the playground is wet. The director added that the staff are supposed to put orange cones out but at the time of the survey the cones were not in use. An interview with the director also found that parental permission had not been obtained for the children to play outside of the fenced area. Written Documentation
General: Based on Review of Documentation, the surveyor found that two (2) staff (Hire dates: 4/30/13 and 5/28/13) have not completed the required six (6) hour orientation training. Also, the staff person hired on 4/30/13 has only completed seven (7) hours of training. A staff person hired on 12/14/13 had orientation traiing on file that was dated 3/12/14 which is past the ninety (90) day time frame.
General: Based on Observation, the surveyor obseved two (2) crib sheets that were soiled with what appeared to be recent yellow colored baby food or "spit up". The staff person stated the sheets were washed every week. However, the sheets that were observed at the time of the inspection were in need of immediate changing.
General: Based on Observation, the surveyor observed two (2) ceiling tiles that were stained brown from water residue in Classroom #10 and Classroom #13. (2) The wood trim around the doorway in the two (2) year old room was in bad repair. The bottom half of the trim was missing and the top part of the trim was very loose; not secured to the door frame.
General: Based on Observation, the surveyor observed a huge amount of debris on the 2004 Ford day care van that included but was not limited to the following: candy wrappers, empty water bottles, and food crumbs. It was noted that the driver of the van had checked the daily inspection log as "free of debris".
General: Based on Observation, the vinyl furniture in Classroom #10 had been repaired with tape. However, the tape was peeling and leaving a sticky residue on the pieces of furniture.
General: Based on Observation, there were damaged ceiling tiles throughtout the downstairs classrooms. The tiles had brown spots that looked like water damage.
General: Based on Review of Documentation, one (1) staff person did not have current documentation of being freen of TB. Her hire date was 11/28/08.

General: Based on Review of Documentation, it was found that some of the medication forms reviewed did not have a specific time for the medication to be given. The medication form would state a date and "give as needed". In addition, some of the medication forms reviewed contained staff member's initials as to who gave the medication. Food Service
General: Based on Review of Documentation, it was found that two (2) medication forms for a child were not signed by a parent. Some of the medication forms reviewed indicated to "give as needed". This is not a daily request for the medication. Some of the medication forms reviewed showed that the parent had requested the medication on a specific date and a specific time and the staff did not sign off on giving the medication as requested by the parent. Based on staff interviews sometimes the staff did not give the medication if they felt like the child did not need it.
General: Based on Observation, the black plastic ground cover was showing throught he mulch in several diferent areas. The exposed plastic created a tripping hazard.
T-Birds: Based on Observation, there was a one (1) year old combined with a group of two (2) year olds.

General: Based on Review of Documentation, it was found that there was not a current lesson plan in the school age classroom. The last dated lesson plan was for 03/21/2016 thru 03/25/2016. It was also found that there was not a current lesson plan in the Bridging/Pre-K classroom. The last lesson plan was dated for 04/25/2016 to 04/29/2016. The staff in charge acknowledged this.
General: Based on Observation, it was found that in the men's downstairs restroom used by the children for toileting, There was observed to be bottles of Windex window cleaner and Lysol toilet bowl cleaner in a low cabinet that was accessible to the children as the lock was not secure. In the women's bathroom that is used by the children for toileting, there was observed to be a bottle of glass cleaner, Lysol, Tilex, Clorox wipes and toilet bowl cleaner accessible in a low unlocked cabinet. General: Based on Observation, it was found that in the three (3) to four (4) year old classroom and in both Bridging/Pre-K classrooms there was found to be a substantial amount of dust on the ceiling vents. it was also found that in the two (2) to four (4) year old classroom there was observed to be several areas of chipped pain on the walls as well as in one of the Bridging/Pre-K classrooms. In the downstairs women's bathroom used by the children for toileting, there was observed to be a brown water stain on the ceiling with peeling paint. Also in the women's bathroom there was a missing tile on the floor and the rubber trim around the bottom of the floor was observed to be torn. In the men's bathroom the rubber trim was observed to be coming loose from the wall. In the two (2) year old classroom, there was observed to be a substantial amount of dirt and debris on the rug. The staff in charge acknowledged this.
General: Based on Observation, it was found that the water at the sink in the upstairs restroom used by the children for handwashing reached a temperature of 130+. The staff in charge acknowledged this.
General: Based on Review of Documentation, child enrolled on 01/18/15 did not have a preferred hospital listed on file. The director acknowleged this finding.
General: Based on Review of Documentation, there were no children's names listed with the fire drill for March 2016. The director stated that she must have forgotten to print off a copy of the children who were present the day of the drill.
General: Based on Review of Documentation, a staff person hired 5/18/15 did not have the results of a child abuse or neglect check nor was there evidence the application for the results had been submitted. In addition there were no results of a criminal records check for this same person submitted for review at the time of the survey nor was there documentation to verify the application for the results of the criminal record check had been submitted. At the time of the survey this staff person took five (5) school age children outside to wait for the bus to pick them up and take them to school. This staff person was supervising the children alone. It could not be determined how long this staff person was outside with the children. In addition this same staff person was observed sitting in the eating area for lunch with a group of ten (10) children ages three (3) to five (5) years old. Although there were other groups of children with other staff in the same area this staff person was given the responsibility and supervision of a group of children and did not have the results of both records checks back.
General: Based on Review of Documentation, it was found that neither the van driver nor monitor on the Chevy Express are currently certified in CPR/first aid. An interview with the director found that these staff have been responsible for picking children up from school and bringing them to the facility the majority of the school year up until 5/26/15 which is the day before this survey was conducted.
General: Based on Review of Documentation, there was one (1) staff person with a hire date of 9/3/14 who had the results of a criminal records check dated 9/8/14 and another staff person with a hire date of 1/12/15 who had the results of a criminal records check dated 1/27/15. There was no documentation available to verify that the application for the results of the criminal records check had been submitted prior to the date of hire or on the date of hire. General: Based on Observation, at the time of the survey the kitchen door had been proped open. Three (3) bottles of bleach, a can of Lysol spray disinfectant and a bottle of heavy duty degreaser were observed sitting on the floor under the sink. These items could be seen from the hallway. Children were observed using the bathrooms located across from the kitchen. The labels on these items indicate to "Keep out of the reach of children".
General: Based on Observation, during a tour of the boys' and girls' bathrooms located upstairs uncovered light bulbs were observed in the light fixtures over the sinks. An interview with the director found she could not verify that the light bulbs were shatter proof.
General: Based on Observation, upon arrival there were children playing in the fellowship hall waiting for breakfast to be served. Once breakfast was served the children were placed at the table without washing their hands.
General: Based on Observation, during an inspection of the first aid supplies on the Chevy Express used to transport children there was not a working flashlight or a thermometer. On the Ford van used to transport children an inspection of the first aid supplies found there was no gauze or a thermometer.
General: Based on Observation, during a tour of the older infant room (room # 11) and the younger infant room (room # 12) a crib with two (2) mattresses stacked on top of each other was observed in each room. The top mattress did not fit snuggley in the crib and had gaps around the edges.

General: Based on Observation, the surveyor observed a portable hand-washing sink in the two (2) year old classroom next to the diapering area. The surveyor tested the hot water temperature with a calibrated thermometer and found the hot water temperature to be sixty five (65) degrees.
General: Based on Observation, the surveyor found that both First Aid Kits on the 2004 Ford Van and the 2005 Chevorlet Van did not contain CPR mouthpieces. The surveyor found the Fire Extinguisher on the 2005 Chevorlet Van was empty. 825 - Transit/School Bus Not Applicable 922 KAR 2:120. Section 12. Transportation. (5) Transportation provided by licensed public transportation or a school bus shall comply with subsections (1) and (2) of this section.
General: Based on Observation, the surveyor observed torn diaper changing pads in the following classrooms: Classroom #11 and Classroom #12. There was also a torn diaper changing pad located in the girls' restroom upstairs.
General: Based on Observation, the surveyor observed four (4) uncovered light bulbs in the boys' restroom and in the girls' restroom. The staff person in charge stated the restrooms were painted recently and the painters forgot to put the sheilds back over the light bulbs. The exposed light bulbs did not appear to be shatterproof.

General: Based on Interview, the surveyor found that a permanent wall was added in February 2014 in order to enclose the two (2) year old classroom that is located on a former stage adjacent to the cafeteria. The staff person in charge stated that there use to be temporary walls in place prior to the renovation. The renovation was not reported to the cabinet prior to construction.
General: Based on Interview, the surveyor found that three (3) staff who drive the day care vans do not have a driving history record on file that has been requested through the state transportation agency. Programming
General: Based on Observation, and interview, the daily planned activities was not posted in Classroom #11. The staff person stated that she was "filling in" for the regular teacher and did not know if she had a lesson plan.
General: Based on Observation, the surveyor observed a staff person's purse on top of the children's cubbies in the two (2) year old classroom. The purse strap was within reach of the children in care.
General: Based on Observation, at approximately 8:40 a.m., the surveyor observed a staff person change a child's diaper and not wash his hands afterwards. The staff person returned the child to the group.
