General: Based on review of documentation, the following was found: 1. Staff member (DOH: 06/27/20) had a pediatric abusive head trauma training on file that exceeded five (5) years and therefore is no longer current. 2. Staff member (DOH: 08/26/19) completed nine (9) out of fifteen (15) hours of cabinet- approved early care and education training for the 07/01/24-06/30/25 training year. A review of ECE-TRIS supports these findings.
General: Based on observation, the Three Year Old's #1 Classroom contained a can of aerosol Clorox sanitizing spray within a basket on top of a wooden shelf holding toys that was within reach of children in care. It should be noted that a child was reaching for an item within this basket while the surveyor was present in the room.
General: Based on observation, in the Two Year Old #2 Classroom, the surveyor witnessed the staff member change two (2) children's diapers and did not sanitize the changing pad between diaper changes, nor after the second diaper change.

General: Based on observation, in the Infant #3 classroom, the surveyor found an inadequate number of toys. The surveyor noted that there were no blocks and no dramatic play materials. Furthermore, the books were stored out of reach of the children in care.

General: Based on interview and review of documentation, on 5/28/2021, the staff person in charge received information and video footage alleging that a staff person touched a three-year-old child in an inappropriate manner. The incident was not reported to the cabinet (DCBS or DRCC).

This outlet was located high on the wall and the CD player was plugged into the outlet. 2.) An insect glue trap was located in the bathroom in the Toddler 2-A Room. General: Based on observation and interview, there were seven 2-year-old children being cared for in a classroom with three 3-year-olds in the Twos II classroom. The the facility had not obtained the signature and approval from the 2-year-olds' parents for a transition plan.

At approximately 9:23 a.m., the surveyor observed two female infants, both eight weeks old, sleeping on top of loose crib sheets. General: Based on observation, this regulatory requirement was not met. The following was found: 1. During a tour of the three (3) year old classroom, the surveyor observed a lamp positioned on a low corner shelf that was turned on and plugged into a wall outlet. The cord was not securely attached to the wall and it was accessible to the children. 2. During a tour of the middle one (1) year old classroom, the surveyor observed a large commercial size roll of toilet paper positioned on top of the toilet tank in the children's bathroom area. The staff person working in the classroom identified two (2) children present in the room that are currently potty-training. The large roll of toilet paper was the only toilet paper accessible to the children because the toilet paper holder on the wall was found to be empty. Storing toilet paper in this manner is not sanitary and allows for the spread of communicable illnesses. 3. At 9:25AM the surveyor entered the Pre K Classroom (four (4) - five (5) year olds) and observed a staff person (hire date: 6/27/00) braiding a child's hair. The surveyor observed the staff person jerk her left elbow up in the air while braiding the child's hair and yell at a child standing next to her, "Get off my arm". The staff person was not aware that the surveyor had entered the classroom at the time. 4. Two (2) staff were observed changing ceiling tiles in the School Age dining area. It should be noted that this space is an open area that also includes the School Age # 1 classroom which was being used by some of the children from the 4's # 1 room. These same staff were also observed changing ceiling tiles in the hallway in front of the bathrooms located across from the preschool dining area. The staff placed a tall ladder in the hallway while children were using the bathroom. This ladder was accessible to the children. 5. In School Age Room #3, two (2) plastic containers filled with blankets were observed on a shelf in the book area. An interview with staff found that the children use the blankets to cover themselves while in the book area. The staff person also stated that the blankets are washed once a week. Because the blankets are not desingated for individual children and are stored in a manner where they are touching, this allows for the cross contamination of germs. 6. In the entry way of the facility located beside the office which also enters into School Age Room #2, there were two (2) wheelbarrows and adult sized gardening tools. Interviews with staff during the exit conference found that these items had been used the day prior to the survey to spread mulch on the playground and were placed inside the facility to keep them from being stolen. 7. In School Age Room #2, a black portable heater was also observed in the book area. Interviews with staff during the exit conference found that the heater had been used once this year when it got very cold. It was stated that there were no children in the area when the heater was in use. However, the heater has remained in the area which is part of the children's play space making the heater accessible to the children. Employee Records Not In Compliance

General: Based on Observation, a stained ceiling light tile was observed in the girls' bathroom in the front preschool dining area. The ceiling light tile contained a rusty red substance inside the area.
General: Based on Observation, there were several purple nap mats observed in the Three-Year-Old room # 3 and the Four-Year-Old room # 1 that had worn corners that exposed the foam. This does not allow for the mats to be properly sanitized.
General: Based on interviews, (it was found that there was a physical altercation between a parent and a teaching staff in the facility which created imminent danger to children present. Parent was throwing items off a desk that included water bottles and office equipment. Parent and staff started hitting each other after a water bottle thrown by parent hit staff. In addition both parent and staff were cussing each other during conflict. One child was reportedly kicked in the face during the altercation.
General: Based on Observation, in the three (3) and four (4) year old classroom with only one (1) staff there were sixteen (16) children resulting in four (4) over ratio limit. Staff stated that the children had not been separated into their other rooms yet. Based on interview, facility failed to notify of changes to facility hours. Interviews with staff indicated there were multiple reoccurring occasions that parents picked children up after the stated time of 6pm.
Based on interview, it was found that a teaching staff was aware of possible neglect/abuse issues with a child that attends the facility. Staff stated that children frequently arrived at center smelling of pot. Staff stated that one of the children reportedly is made to sit in a high chair for excessive times by the parent. Based on interview, it was found that sections of the metal chain link fence were removed the week prior so that they could get the playground renovated. General: Based on Observation and interview, the director failed to keep all equipment in good repair. The staff person in charge revealed during the investigation that one (1) of their air conditioner units was not working and indicated that it had not been working for approximately three (3) weeks. The surveyor requested the file for a specific staff person during the investigation. The staff person in charge stated that this staff person was no longer employed at the facility and identiifed her last day as 9/12/16. The only documentation on file at the facility for this staff person was her educational documentation and her completed child abuse/neglect check (CAN); therefore, she did not have a complete file at the facility. General: Based on Review of Documentation and interview, this regulatory requirement was not met. The surveyor requested the file for a specific staff person during the investigation. The staff person in charge stated that this staff person was no longer employed at the facility and identiifed her last day as 9/12/16. The only documentation on file at the facility for this staff person was her educational documentation and her completed child abuse/neglect check (CAN); therefore, she did not have a complete file at the facility. It is important to note that administrative staff had difficulty identifying this person's hire date because it had not been documented. After reviewing facility time cards, the staff person in charge stated, "Guessing from the amount on her first paycheck, her first day would have been around the first of September". General: Based on observation, staff interview, and written documentation the facility employed an individual on 5/6/16 who has a substantiation of child neglect on the central registry (CA/N check). On 5/11/16 at approximately 1:10 p.m., this individual was observed working in the infant classroom. The individual told the surveyor that she had been working in the infant classroom since she was hired on 5/6/16. It is imporant to note that at the time of the observation, a qualified staff person was working in the classroom with this individual. The co-director told the surveyor that the paperwork that she had received did not reveal a substantiation. However, after looking at the CA/N check document again in the presence of the surveyor, the co-director found that the results revealed that the individual did indeed have a substantiation of child neglect. The co-director stated that she had not noticed it before. The co-director stated that she received the CA/N check on 5/5/16 and the individual started working on 5/6/16. The surveyor found that the central registry check was signed by the individual and the co-director on 4/28/16. The document was received in the Division of Child Care on 5/2/16 and the check was completed on 5/3/16. The date of the substantiation was found on 8/28/15.
General: Based on Observation, in the infant 2 room the green seat cover on the rocking chair cushion was torn and the foam was exposed. In the infant 3 room there was a spill down the side of the cabinet that had left a brownish residue.
General: Based on Observation, a row of lights in the younger two (2) year old classroom were not working. The staff person tried turning on the lights but they would not come on. A section of lights in the older two (2) year old classroom were flashing. The director stated that maintenance person was on vacation but would look into the issue.
General: Based on Observation, the bathroom in the school age room #4 had what appeared to be several yellow urine spots on top of the toilet seat. The director stated that she would make sure the toilet was cleaned. Hygienic Practices

After looking up information on the computer, the staff person in charge presented the surveyor with start dates for the staff. The staff person in charge stated that the staff person responsible for tracking staff records is currently out on leave.

General: Based on Observation, there was a shelf white shelf in the Threes I classrom that had numerous areas of chipping paint.
General: Based on Observation, during a tour of the facility there were four (4) bathrooms that contain the automatic sinks for handwashing. The water temperature could not be tested at these sinks due to the water not staying on long enough. There was a girls' bathroom located in the hallway where the preschool classrooms are located that had manuel handles for the sink. The water temperature was tested at this sink with a calibrated thermometer. The water temperature measured approximatelly seventy (70) degrees Fahrenheit.
General: Based on Interview, it was determined that the staff person in the Infant II classroom was preparing formula in bottles for the children in care.
General: Based on Review of Documentation, a staff person rehired 1/16/15 had a criminal records check dated 3/4/15. There was no evidence of when the check was submitted as evidence it was done prior to employment. Also, a staff person hired 2/16/15 had no evidence that a child abuse and neglect check had been submitted; she was not working alone.

General: Based on Observation, the surveyor tested the water temperature with a caliberated thermometer at three (3) hand washing sinks in the school age area and across the hall from the preschool classrooms and found that the hot water was only registering at sixty five (65) degrees Fahrenheit.

In the School Age #4 room, there was a white built-in bookshelf located in the book area. There were several areas on the shelf where the paint had chipped and peeled away from the surface. 5. In the School Age #2 room, two (2) wooden rocking chairs with cushions were observed. The surface of the cushions had areas that were stained and soiled. In the same room a teal colored cloth couch was observed in the book area. There were areas on the surface that were stained and discolored with dirt. In the School Age room #3, the wall where the television is located had several areas of chipping and peeling paint. Several pieces of masking tape were also observed on the wall where a bulletin board is located. The blue wall in the book area had areas of chipping and peeling paint. The orange wall in the block area also had several areas of chipping and peeling paint. A blue wall with hooks also had areas of chipping and peeling paint. Three (3) dusty ceiling vents were also observed in this classroom. 7.) In School Age room #4, the wall where dress up clothes were observed hanging and the wall where a raised platform is located has areas of chipping peeling paint. Several ceiling vents with an accumulation of dust were also observed in this classroom. A blue wall located in a small hallway in this classroom had thre (3) wall vents that were bent inward. Numerous dirt particles were observed on the carpet in the classroom. 8.) In School Age room #2, dried orange particles of dirt were found on the floor and on a burgandy colored rug in an area that contained a coffee table and two (2) chairs. During a tour of the middle one (1) year old classroom, the surveyor observed a large commercial size roll of toilet paper positioned on top of the toilet tank in the children's bathroom area. The large roll of toilet paper was the only toilet paper accessible to the children because the toilet paper holder on the wall was found to be empty. General: Based on Observation, the bathroom located in the two-year-old #1 room contained a vent (in the ceiling) with a large accumulation of dust. General: Based on Observation, it was found that a six-feet long section of the chain-link fence as well as the gate to the playground had been taken down to allow for playground maintenance. The fencing was reportedly taken down on 4-6-17.
General: Based on Observation, the outdoor play area contained rolls of metal chain link fence, fencing brackets, hardware, and large concrete pieces (on the base of the metal fence polls) that were accessible to the children. There were also several weeds on the playground. In addition, the paint on the side of the building that was accessible to the children on the playground was peeling off in multiple places. General: Based on Observation, it was found that the statements of deficiencies issued and plans of corrections submitted for allegation investigations conducted on 5-11-16 and 9- 29-16 as well as the notice of a civil penalty issued on 8-4-16 were not posted in the facility.
General: Based on Observation and interview, this regulatory requirement was not met. The surveyor toured the facility with the staff person in charge during the visit and observed three (3) staff caring for a group of twenty-six (26) children in the two (2) - three (3) year old classroom. The youngest child in the group was two-years-old, making the maximum group size twenty (20) children. The facility exceeded the group size by six (6) children. The surveyor and the staff person in charge walked into the classroom at the same time as a staff person arriving at 8:30AM. At this time, staff began dividing the group by taking a group of children to the bathroom to wash their hands. General: Based on Observation and interview, the director failed to provide for the safety of each child. The staff person in charge escorted this HSS on a tour of the facility during the visit. A very large rotating fan was observed sitting on a table in the preschool eating area and it was plugged into an outlet on the other side of the room with an extension cord. The fan was turned on and was pointed in the direction of an adjacent classroom. Children were not using the preschool eating area during the first walk-thru at 8:10AM; however, the surveyor walked through the room again at approximately 9:15AM and observed nine (9) preschool children eating breakfast in this room. The fan was still on and the electrical cord was stretched through the middle of the room in the space the children were using. The staff person in charge stated that the fan was being used to keep a few rooms cool because one (1) of their air conditioner units was not working and indicated that it had not been working for approximately three (3) weeks. General: Based on Observation, there was a glass of milk, a cup of fruit and a honey bun sitting on the table in the school age dining room. The director stated that a staff person must have forgotten to throw the items away after the children left the area.
General: Based on Observation, the sink in the infant 1 classroom did not have adequate hot water. The water temperature did not get above 70 degrees Fahrenheit when tested with a calibrated thermometer. Also, the water in the infant 2 classroom did not get hot. The sink in this room operated on a motion sensor and this made getting a accurate temperature difficult. During the exit interview the staff person in charge said the maintenance person would check on the water.
General: Based on Observation, there was a kitchen counter top and a kitchen cabinet stored in the library room. The staff person stated there was some remodling in the kitchen. These items were in the children's play space as they did activities in this room. General: Based on Observation, a staff purse was found sitting out on a shelf in the three (3) year old classroom. The purse was accessible to the children. The director stated that staff are aware that personal items are not to be left out in the classroom. General: Based on Observation, in the toddler 2 room there was a bottle of all purpose cleaner under the bathroom sink. On the changing table in this room other cleaners like Lysol spray, window cleaner, a spray bottle with a green liquid, and Array disinfectant spray.
General: Based on Observation, in the infant 2 room there was an area of the baseboard missing from the wall near the changing table. The staff person in charge stated that the changing table had recently been moved and there were plans to repair the baseboard. In the infant 2 room there were four (4) ceiling tiles with brownish spots. The staff person in charge stated it could be water damage from the hair salon above the center.
General: Based on Observation, the bathroom in the school age #4 room had several pieces of toilet paper on the floor and sink. The director stated that she would make sure the bathroom was cleaned.
General: Based on Observation, there were scrapes of paper, plastic bottles, and other debris on the playground.
General: Based on Observation, a portion of the playground was bordered by the concrete block building. The paint on the buidling was chipping off in pieces.
General: Based on Observation, several mats in the four (4) year old classroom were torn.
General: Based on Review of Documentation, it was found that the transportation binder for the 2003 Chevy van used to transport the children did not contain evidence of a daily inspection being performed during the months of January, February and March of 2016. The van is used daily to transport school age children. The staff person in charge stated that the documents had been misplaced and she was unable to locate them.
General: Based on Observation, the thermometer in one (1) of the refrigerators in the preschool dining area did not work properly. The director stated that she thought it was working but would look into it.
General: Based on Review of Documentation, it was found that one (1) child's file out of ten (10) children's file reviewed did not contain information regarding the parent's preferred hospital. The staff person in charge stated that she would have the parents fill in the information.
General: Based on Observation, it was found that the statement of deficiencies issued and the plan of correction submitted for the 2015 relicensure survey was not posted in the facility. General: Based on Review of Documentation, a staff person hired 11/17/14 had TB skin test dated 3/9/13. The two (2) year time frame had just recently lapsed.

General: Based on Observation, a platform located in the school age room # 4 was found to have numerous areas of chipping and peeling paint. The foam squares that covered the platform had numerous areas that had an accumulation of dirt and grime.

General: Based on Observation, during a tour of school age room # 4 a refrigerator with a freezer was observed. Inside the freezer there was a bag of bagels and a box of English muffins. There was not a thermometer in the freezer.
