General: Based on observation, this regulatory requirement was not met. The following was found: 1. During a tour of the three (3) year old classroom, the surveyor observed a lamp positioned on a low corner shelf that was turned on and plugged into a wall outlet. The cord was not securely attached to the wall and it was accessible to the children. 2. During a tour of the middle one (1) year old classroom, the surveyor observed a large commercial size roll of toilet paper positioned on top of the toilet tank in the children's bathroom area. The staff person working in the classroom identified two (2) children present in the room that are currently potty-training. The large roll of toilet paper was the only toilet paper accessible to the children because the toilet paper holder on the wall was found to be empty. Storing toilet paper in this manner is not sanitary and allows for the spread of communicable illnesses. 3. At 9:25AM the surveyor entered the Pre K Classroom (four (4) - five (5) year olds) and observed a staff person (hire date: 6/27/00) braiding a child's hair. The surveyor observed the staff person jerk her left elbow up in the air while braiding the child's hair and yell at a child standing next to her, "Get off my arm". The staff person was not aware that the surveyor had entered the classroom at the time. 4. Two (2) staff were observed changing ceiling tiles in the School Age dining area. It should be noted that this space is an open area that also includes the School Age # 1 classroom which was being used by some of the children from the 4's # 1 room. These same staff were also observed changing ceiling tiles in the hallway in front of the bathrooms located across from the preschool dining area. The staff placed a tall ladder in the hallway while children were using the bathroom. This ladder was accessible to the children. 5. In School Age Room #3, two (2) plastic containers filled with blankets were observed on a shelf in the book area. An interview with staff found that the children use the blankets to cover themselves while in the book area. The staff person also stated that the blankets are washed once a week. Because the blankets are not desingated for individual children and are stored in a manner where they are touching, this allows for the cross contamination of germs. 6. In the entry way of the facility located beside the office which also enters into School Age Room #2, there were two (2) wheelbarrows and adult sized gardening tools. Interviews with staff during the exit conference found that these items had been used the day prior to the survey to spread mulch on the playground and were placed inside the facility to keep them from being stolen. 7. In School Age Room #2, a black portable heater was also observed in the book area. Interviews with staff during the exit conference found that the heater had been used once this year when it got very cold. It was stated that there were no children in the area when the heater was in use. However, the heater has remained in the area which is part of the children's play space making the heater accessible to the children. Employee Records Not In Compliance

General: Based on observation, staff interview, and written documentation the facility employed an individual on 5/6/16 who has a substantiation of child neglect on the central registry (CA/N check). On 5/11/16 at approximately 1:10 p.m., this individual was observed working in the infant classroom. The individual told the surveyor that she had been working in the infant classroom since she was hired on 5/6/16. It is imporant to note that at the time of the observation, a qualified staff person was working in the classroom with this individual. The co-director told the surveyor that the paperwork that she had received did not reveal a substantiation. However, after looking at the CA/N check document again in the presence of the surveyor, the co-director found that the results revealed that the individual did indeed have a substantiation of child neglect. The co-director stated that she had not noticed it before. The co-director stated that she received the CA/N check on 5/5/16 and the individual started working on 5/6/16. The surveyor found that the central registry check was signed by the individual and the co-director on 4/28/16. The document was received in the Division of Child Care on 5/2/16 and the check was completed on 5/3/16. The date of the substantiation was found on 8/28/15.

None of these instances was reported to the proper authorities as suspected abuse or neglect of a child. Several ceiling vents with an accumulation of dust were also observed in this classroom. A blue wall located in a small hallway in this classroom had thre (3) wall vents that were bent inward. Numerous dirt particles were observed on the carpet in the classroom. 8.) In School Age room #2, dried orange particles of dirt were found on the floor and on a burgandy colored rug in an area that contained a coffee table and two (2) chairs. General: Based on Observation and interview, the director failed to provide for the safety of each child. The staff person in charge escorted this HSS on a tour of the facility during the visit. A very large rotating fan was observed sitting on a table in the preschool eating area and it was plugged into an outlet on the other side of the room with an extension cord. The fan was turned on and was pointed in the direction of an adjacent classroom. Children were not using the preschool eating area during the first walk-thru at 8:10AM; however, the surveyor walked through the room again at approximately 9:15AM and observed nine (9) preschool children eating breakfast in this room. The fan was still on and the electrical cord was stretched through the middle of the room in the space the children were using. The staff person in charge stated that the fan was being used to keep a few rooms cool because one (1) of their air conditioner units was not working and indicated that it had not been working for approximately three (3) weeks.
