General: Based on observation, the refrigerator in the Dogwood Classroom had a temperature reading of 45 degrees Fahrenheit.
General: Based on review of documentation, three (3) staff members (DOH: 09/22/23, 11/27/23, and 08/04/21) had their last TB test dated as (08/18/23, 11/17/23, and 07/17/23) which was no longer valid.
General: Based on review of documentation and interview, the childcare center had their last Fire Marshal inspection on 08/18/23 and the director was not sure if a new one had been requested.
General: Based on observation, the following was found: 1) In the Honeysuckle Classroom, the ceiling air vents located in the middle of the room and near the children's cubbies had an accumulation of dust on them. 2) In the Willow Classroom, the air vent located in the middle of the room had an accumulation of a black substance around its perimeter. 3) In the Willow Classroom, the ceiling tile located above the cabinet on the outside wall had a brown stain on it. 4) In the Redbud Classroom, the air vent located on the ceiling in the middle of the classroom, had an accumulation of a black substance around its perimeter. 5) In the Dogwood Classroom, the air vent located on the ceiling in the middle of the classroom, had an accumulation of dust and a black substance around its perimeter.

General: Based on observation, a tour of the facility found the following: 1) in Room 123, a Keurig coffee maker was observed sitting on a low counter. The coffee maker was not turned on at the time. A Starbucks cup and various other adult drink cups were also observed sitting on the counter with an unidentified liquid inside. The cups of liquid were accessible to the children in care at the time of the survey. 2) In Room 122, there were several adult drink cups that contained an unidentified liquid sitting on a low counter top inside the classroom. An adult backpack and other personal bags were observed under the counter. The cups of liquid, the backpack, and other bags were accessible to the children in care at the time of the survey. 3) In Room 116, there were several adult drink cups that contained an unidentied liquid sitting on the low counter top. The drink cups were accessible to the children in care at the time of the survey. 4) In Room 117, there was a Keuring coffee maker on the low counter and several adult drink cups that contined an unidentified liquid. The coffee maker was on at the time. The surveyor felt the sides and the top of the coffee maker and observed the surface was warm but was not hot to the touch. The adult cups were accessible to the children in care at the time of the survey. This regulation requires that staff members personal belongings, which includes food and drink, not be accessible to the children. General: Based on observation, interview, and review of documentation during a tour of the facility, a posted snack menu was not observed. Interviews with staff members found that the children bring their own snack, with the exception of the children in the two-year-old rooms. It was stated by staff that the facility provides the daily snack for the children in the two-year-old rooms. Further interview with staff found that they do not post a menu that contains what will be served each day for those children who receive snack provided by the facility. A weekly menu for all meals provided by the facility are required to be dated and posted. General: Based on observation, review of documentation, and interview, this regulatory requirement was not met. On 2/22/19 the facility reported to DRCC that on that same day at 1:22PM they had to administer an Epipen to a two (2) year old child that was having an anaphylaxis reaction from eating sweet potato fries at lunch. An interview with staff during the investigation and a review of documentation found that staff are aware of the child's food allergies and there is a procedure in place to monitor food served to the child each day; however, on 2/22/19 an error was made by the cafeteria staff. The cafeteria staff communicated with the child's parent and the staff that work directly with the child that the sweet potato fries did not contain gluten; however, a gluten ingredient was listed on the box. General: Based on observation, interview and review of documentation, the child care center failed to comply with regulatory programming requirements. Observation of the Pre- Kindergarten Two’s classrooms #124 and #125, revealed eleven (11) children and two (2) staff in each room. Interview with staff revealed the youngest child in the classrooms were two (2)-years-old and the oldest child was three (3)-years-old. Upon request, the child care center director failed to provide documented evidence of a transition plan for each toddler present with the older children, in the classrooms, as it was indicated the children were combined for more than one (1) hour each day. General: Based on observation and interview, the child care center failed to maintain the premises in accordance with regulatory requirements. Observation of the Pre-Kindergarten Two classroom #125, revealed three (3) bottles of an unknown liquid stored on the back of a classroom easel. The easel was within accessible reach of children who played in the classroom. Interview with the staff revealed one (1) of the bottles contained a disinfectant that should have been kept out of reach of children. Observation of the Pre-Kindergarten Two classroom #125, revealed milk served to children in the classroom, stored in a small refrigerator. Review of the temperature measuring device located in the refrigerator revealed a temperature of fifty (50) degrees Fahrenheit. The temperature was exceeded by ten (10) degrees Fahrenheit, based on regulatory requirements.
