The file did not contain evidence of a central registry check ever being completed. This employee has been employed for longer than the allowed ninety (90) days. The director stated that she had not noticed the central registry check not being complete or that it needed to be resubmitted with the missing information. This employee has reportedly never worked alone with the children; 2) it was found that the files for employees hired on 5-20-15, 5-20-15 and 8-29-15 contained evidence of requests for central registry checks being signed and submitted on 8-25-15, 8-25-15 and 2-29-16 respectively. Interviews with staff found that there were two (2) classrooms. One (1) of those classrooms had ten (10) children who were two (2) years old with two (2) staff responsible for that group. The other group had seventeen (17) children ages two (2) to three (3) years old with one (1) staff person responsible of the group. Groups were mixed together while playing on the playground. The maximum group size with the age of the youngset child being two (2) years old is twenty (20) and the staff to child ratio with the age of the youngest child being two (2) years old is ten (10). The groups size was over by seven (7) children and the staff to child ratio was over by seven (7) children. There was another staff person inside the building who was supposed to be with the group of seventeen (17) children but she was inside at the time and came outside with the surveyor.

General: Based on Review of Documentation, it was found that the files for employees hired on 4-26-13, 8-20-13, 2-5-14 and 2-12-14 did not contain evidence of a high school diploma, GED or Commonwealth Childcare Credential.

General: Based on Interview, it was confirmed that the child care director failed to provide for the health, safety, and comfort of each child. On 6-24-13 a child was injured after she got her hand stuck in between the elevator door and the frame at the facility.

General: Based on Review of Documentation, it was found that two (2) staff members had TB skin tests on file that were no longer current. These TB tests were dated 8/11/09 and 11/02/09. These two (2) staff were hired on 8/20/12 and 8/18/04. Another staff member hired on 4/15/13 did not have negative TB documentation on file.

General: Based on Review of Documentation, it was found that three (3) staff members did not have fifteen (15) hours of training on file for their completed training year. These three (3) staff wre hired on 2/15/11, 1/4/11, and 8/17/11. In addition, three (3) staff did not complete orientation training within their the first three (3) months of employment. One (1) staff hired 8/20/12 completed orientation on 4/23/13. Another staff hired on 4/11/12 completed orientation on 8/6/12.
