General: Based on observation, the following was found: 1) In the Two's Classroom, there were staff drink cups on a desk that were accessible to the children. 2) In the Two's Classroom, there was a bottle of Odo Ban spray on the half wall divider of the room where children could reach it. This had a label stating, "Keep out of the reach of children." 3) The changing table in the Two's Classroom had a bottle of Glade air freshener and two (2) bottles of disinfectant spray next to the changing pad. The bottles contained the warning "Keep out of the reach of children." 4) In the Infant Classroom, there was a tube of Orajel tooth pain gel next to the changing pad. It had a warning label to "Keep out of the reach of children" on it. 5) In the Preschool Classroom, there was a staff drink bottle on a low-lying toddler table and a container of Lysol disinfecting wipes on a file cabinet with a "Keep out of the reach of children" label on it. On a blue children's table there was a staff member's Gatorade bottle and a Glade room spray with a label "Keep out of the reach of children" where children could reach them both.

General: Based on observation, in the Infant Classroom, the changing pad on the changing station had a rip in the middle of the pad, exposing the inner materials.
Based on review of documentation and interview, the child care center did not have a written reunification plan for infants, toddlers and children with special needs. The staff in charge stated that she was not aware of this requirement. Posted Documentation In Compliance
General: Based on interview and review of documentation, the child care center did not have a written procedure for maintaining ratio and group size throughout the day and during transitions. The staff in charged stated she was not aware of the regulation and also confirmed that the staff members do not have a procedure for conducting name-to-face recognition during transitions. General: Based on interview and review of documentation, the child care center did not have a staff member certified in infant, child and adult cardiopulmonary resuscitation (CPR) and infant and child first aid, employed or onsite. Staff members that had previously been certified had expired certifications including the staff in charge whose certification expired in December 2025. Children ranging in age from five (5) months to five (5) years old were present at the time of inspection.
General: Based on observation, interview and review of documentation, the child care center did not have a menu for review at the time of inspection. General: Based on observation, the ceiling tiles in the One's A Classroom, had water marks on the tiles next to the ceiling vent and adjacent to the wall. This classroom was not occupied with children at the time of inspection.
General: Based on observation, the following was found: 1) In both children's hallway bathrooms, the water temperature from the hand-washing sink read sixty-three (63) degrees Fahrenheit after running for approximately three (3) minutes. 2) In the Two's Classroom bathroom, the water from the hand-washing sink read sixty-three (63) degrees Fahrenheit.
General: Based on observation, in the Infant Classroom, a gray crib sheet on a crib mattress located next to the children's table had an accumulation of crumbs and liquid spots on it.
General: Based on observation and interview, the child care center did not have whole milk available at the time of inspection. The staff in charge stated she only had skim milk. The child care center had children between the ages of twelve (12) months and twenty-four (24) months present at the time of inspection.
General: Based on observation and interview, the childcare center provided the children with a juice drink containing only 5% juice. The drink also contained added sugars in the form of high fructose corn syrup and sucralose. The staff in charge stated that it was the juice they served the children.
General: Based on review of documentation, the following was found: 1) Eight (8) of the ten (10) children files reviewed did not have a phone number for a preferred hospital. Two (2) of those eight (8) records also did not list a preferred hospital. 2) Two (2) children's files (DOE: 12/18/18 and 10/03/22) did not have a preferred pediatrician listed and one (1) child's file (DOE: 08/05/25) did not have a phone number listed for their pediatrician. 3) One (1) child's file (DOE: 02/02/22) did not have an authorized pick-up person listed in the file.
Based on observation, interview, and review of documentation, the center did not notify the cabinet of the change in their inclement weather closing policy. A surveyor attempted to conduct an investigation on 01/29/26. An undated note was posted on the building stating the center would follow Fayette County School system regarding inclement weather closings. Review of documentation found there was no note in KICCS. Interview with the staff in charge found the center had not notified the cabinet of the change of closing policy. Programming Not In Compliance
Based on observation and interview, a two-year-old child watched television in a bouncy seat for one (1) hour in the Two's Classroom. The child was observed sitting in the bouncy seat watching television at 11:15 a.m. Interview with the staff in charge found the child was two years old. Continued observation found the child was still sitting in the bouncy seat watching television at 12:15 p.m. There was no documentation to prove this was part of the planned programming in the classroom. Based on observation, three (3) infants were in cribs with bottles at 11:23 a.m. on 02/10/26.
General: Based on observation, during a tour of the child care center on 01/22/26, the surveyor observed several cribs blocking a fire exit near the back left side of the building.
General: Based on observation, upon arrival at approximately 9:04 am on 09/17/25 the surveyor observed seventeen (17) children and two (2) staff members in the Two's Classroom. The age of the children was two (2)-months old to three (3)-years-old. The maximum group size with the age of the youngest child being two (2) months old is ten (10). The group size was exceeded by seven (7) children. It was not determined as to how long the classroom had been over group size prior to the surveyor's arrival. It should be noted the staff member in charge was in the office when the surveyor arrived. Once the surveyor alerted the staff member in charge the classroom was over group size one (1) staff member took nine (9) children to the Toddler 2 classroom where there was an additional staff member but no children. General: Based on observation, upon arrival at the facility at approximately 9:07 am on 09/17/25 the surveyor observed seventeen (17) children and two (2) staff members in the Two's Classroom. The age of the children was two (2)-months old to three (3)-years-old. A cartoon program was observed playing on the television located within the classroom. Children under the age of twenty-four (24) months were present at the time. Screen time and electronic viewing is prohibited for children under the age of twenty-four (24) months old. It was not determined as to how long the television had been on prior to the surveyor’s arrival.
General: Based on observation and interview, there were three (3) loosely fitted sheets in three (3) of the cribs in the Infant Classroom. One (1) of the sheets, a gray sheet, also contained areas that were stained and soiled. It should be noted there were no children in this room at the time of the follow up inspection conducted on 09/17/25. The staff member in charge indicated the Infant Classroom has not been used for a while. Premises Not In Compliance
General: Based on review of documentation, the facility's child enrollment documentation did not contain contact information for the family's preferred hospital.
General: Based on observation, the following was found: Infant Classroom 1) Only hot water dispensed from the sink. Toddler 2 1) The sink's hot water temperature measured one hundred forty (140) degrees Fahrenheit. Bathroom next to Kitchen 1) The sink hot water measured only eighty (80) degrees Fahrenheit. One Year Old Classroom 1) The sink hot water measured seventy-nine (79) degrees Fahrenheit. Girls Bathroom 1) Only cold water dispensed from the sink. Boys Bathroom 1) Only cold water dispensed from the sink.
General: Based on observation, the following was found: Two Year Old Classroom 1) A blue vinyl cushion had rips along the edges exposing the padding inside. 2) The edge and corner of an area rug was rolled causing a trip hazard. Toddler A 1) A soft floor mat contained a large tear exposing the padding inside. Bathroom (Beside Kitchen) 1) The sink faucet was corroded and exposing sharp metal edges.
General: Based on observation and interview, the child care center failed to comply with indoor gross motor space requirements. Interview with a teacher and director found the children did not use the outdoor playground that was approved as part of the facility's licensed space. Further observation found the center failed to maintain an indoor gross motor space in lieu of an outdoor play area. The director stated she intended to change one of the classrooms into an indoor gross motor room. At the time of the survey, the aforementioned room contained classroom furniture and materials; it did not contain gross motor equipment.
General: Based on observation and interview, during a tour of the facility the following was found: Purple Preschool Classroom 1) One (1) ceiling tile did not fit securely in the metal frame used to hold the tiles. There was a gap between the frame and the tile. 2) In the corner of the room there was a hole where only a partial ceiling tile was covering the open space. Infant Classroom 1) One (1) ceiling tile with a sprinkler head attached did not fit securely in the metal frame, leaving a gap. Kitchen 1) Two (2) water-stained ceiling tiles were observed over a wall mounted wire rack located over the counter. Equipment Not In Compliance
General: Based on observation and interview, a crib located in the Infant Classroom was observed leaning to one (1) side. Closer inspection of the crib found two (2) of the wheels on one (1) side were missing. It should be noted there were no children in this room at the time of the follow up inspection conducted on 09/17/25. An interview with the staff member in charge found the classroom has not been used for a while.
General: Based on observation, the following was found: Gross Motor Room 1) The paint along the top of the half wall was chipped. 2) The drywall was crumbling along the top and edges of portions of the half wall. Bathroom by One-Year-Old Classroom 1) The drywall around the urinal was discolored. 2) The drywall was crumbled and contained holes on the wall behind the urinal. 3) There was a missing tile in the ceiling near the sink. Kitchen 1) Approximately one half (1/2) of the surface of the ceiling tile, above the microwave, was stained. Toddler A Classroom 1) A ceiling tile was sagging from its frame. 2) A hole in the drywall next to the sink.
Based on observation, the following was found: Storage Room 1) An active water leak was observed coming from the ceiling. The water was being caught in a large tote that was leaking onto the floor.
Based on observation, the following was found: Toddler A 1) One (1) light fixture was sagging from the ceiling. 2) The fluorescent light bulbs from one fixture were not shielded; the light cover was cracked.
Based on observation, there was a buildup of standing urine around the base of the toilet in the boy's restroom. Hygienic Practices In Compliance
General: Based on observation, the child care center failed to maintain food service in accordance with regulations. Observation of the kitchen found the refrigerator temperature measured forty-two (42) degrees Fahrenheit.
General: Based on observation and interview, the child care center failed to maintain food service in accordance with regulations. Review of the posted menu revealed 1% milk was scheduled to be served to children, aged two (2) years and over, for breakfast and lunch each day; however, observation of the childcare center kitchen refrigerator revealed only whole milk and chocolate 1% milk. During interview, the owner/director acknowledged the facility was out of plain 1% milk for children over the age of two (2) years in attendance on the day of the inspection.
