When the surveyor arrived at the home, the certified provider's mother answered the door and reported that the provider had gone to an appointment. It was reported that the provider's mother was an assistant in the home; however, through a review of paperwork, it was found that the staff member did not have a staff file and none of the required background checks (Criminal Records Check or Central Registry Check) had been initiated or completed on this staff member upon hire. General: Based on observation, it was found that the porch area off the back of the house leading into the backyard was dirty with various types of debris. This debris included an orange electrical cord, an empty vacuum cleaner box, a cigarette butt, and paper towels. In addition, it was found that there was an uncovered grill in the backyard, which was accessible to the children in care. General: Based on observation, the following items were found accessible to the children in care: 1. An internet modem with numerous wires attached to it. 2. An iron/metal fire poker as well as other accessories used for a fireplace.

General: Based on observation and review of documentation, the following was found: 1.) The certified provider arrived at the home approximately 10-15 minutes into the survey. She was not wearing a mask when she arrived. The provider did not wear a mask for the remainder of the inspection. 2.) When the surveyor arrived at the home, the certified provider's mother answered the door and reported that the provider had gone to an appointment. The provider's mother was watching the children alone. It was reported that the provider's mother was an assistant in the home; however, through a review of paperwork, it was found that the staff member did not have a staff file and neither of the required background checks had been initiated or completed on this staff member. This staff member was alone with the children for at least 10 to 15 minutes until the provider arrived back to the home.
General: Based on review of documentation, it was found that the assistant in the certified home did not have evidence of a negative TB skin test on file at the facility.
General: Based on review of documentation, it was found that the assistant was not certified in CPR/First Aid. This staff member was left alone with the children for a period of time while the provider was at an appointment; therefore, there was no one at the certified home with CPR/First Aid certification during that timeframe. It was reported by the certified provider that the refrigerator is not currently in proper working order and the part needed should be arriving soon.
