The center does not close until 5:00 pm and therefore should serve a late afternoon snack beginning between 3:15 pm to 4:15 pm to meet meal schedule requirements that a meal should be served every two (2) to three (3) hours.

General: Based on observation, interview, and review of documentation, the child care center failed to maintain state regulations. The surveyor made two (2) visits to the facility: the first on 02/10/25 beginning at 10:40 am and the second on 02/21/25 beginning at 10:30 am. All children in care at the facility were on cots or in cribs (as age-appropriate), covered, some sleeping, and the lights in the center turned out for rest time during both visits. The surveyor concluded from these observations that the children in care had already been served lunch, eaten, cleaned their areas, and prepared for rest time. When referencing the schedule, the surveyor observed rest time was scheduled to end at 1:00 p.m. indicating that the regulation regarding the rest time limit of two (2) hours was not in compliance. Interview with three (3) staff members indicated that rest time does not begin until 11:00 am following lunch, which is served at 10:30 am, per staff. However, the surveyor observed children on cots before 11:00 am, some sleeping, during both visits to the facility. General: Based on Review of Documentation, observation and interview, the following was found: 1. The long wall near the classroom table in the extra classroom (previous toddler classroom) contained approximatley fifty (50) staples. 2. The transition strip was missing that separates the tile floor from the carpeted area in the preschool classroom. An excessive amount of dirt particles and grime had collected in the area. The carpet in door way between the preschool classroom and the cubby room was also found in dis-repair. The carpet in this area was missing and frayed. 3. The surveyor observed three (3) ceiling tiles with dark stains in the preschool cubby room and one (1) ceiling tile with a dark stain near the exterior door that leads outside to the playground. A review of Division of Regulated Child Care files found that these regulatory violations were previously issued as a result of a relicensure survey conducted on 11/7/17. The director had indicated on a Plan of Correction submitted 1/3/18 that the staples were removed from the wall on 11/10/17, ceiling tiles will be replaced by 1/8/18 and she stated that she was in process with the landlord to repair/replace the carpet by 2/1/18. General: Based on Observation, during a tour of the 1's room there were several zebra print vinyl mats sitting on a shelf that were torn. An interview with the staff person in the room found that the mats are used for sitting on when reading. There was also a wooden shelf in the room that had numerous areas of chipping and peeling paint. The changing table located in the bathroom located off of the 1's room had an end piece that had come loose from the top of the changing table. During a tour of the toddler room there was a changing table that had a drawer that had been repaired with blue duct tape. In addition one of the legs had been repaired and secured to the rest of the changing table with clear packing tape. An interview with the staff person in this room indicated that the changing table was like that when she started working in the room. Also during an inspection of the vehicle with the name of the facility on it it was found that one (1) of side windows did not shut all of the way. During an inspection of both of the vehicles used to transport children it was found that the floors were covered in crumbs, food wrappers and other debris.

General: Based on Review of Documentation and interview, this regulatory requirement was not met. Interviews with staff during an investigation revealed that staff-to-child ratios were not in compliance on 8/17/15 while a staff person cared for an eight (8) month old child in the preschool classroom. Staff stated that the staff person was not clocked-in at the time; however, went into the infant classroom to get the eight (8) month old child and then brought the child into the preschool classroom and cared for the child in the preschool classroom approximately fifteen (15) minutes from approximatley 10:00AM - 10:15AM. Staff further stated during interview that this staff person (hire date: 1/11/11) often carries the eight (8) month old child around the facility because the child is very attached to the staff person. A review of facility attendance records found that eighteen (18) children (three (3) and four (4) year olds) were present in the preschool classroom on 8/17/15 from 10:00AM - 10:15AM and the eight (8) month old child made a total of nineteen (19) children. General: Based on Review of Documentation and interview, the facility director failed to provide for the safety and comfort of a child. A visit to the facility was made to investigate allegations regarding two (2) staff getting into an argument in front of the children on 8/17/15 around 10:00AM, and a four (4) year old child being injured as a result of the incident. The staff person in charge identified the two (2) staff involved in the arguement during interview (hire dates: 9/11/14 and 1/1/11) and stated that both staff were escorted out of the facility immediately after the incident and they were placed on a two (2) day suspension. Interviews with administrative staff and staff present in the preschool classroom at the time of the incident and written statements provided by the staff involved, revealed that a male staff person working in the classroom and a female staff person that was visiting in the room with an infant at the time got into an argument and were heard screaming/yelling at each other and using profanity. During the argument the male staff person swiped a container of crayons off the table he was sitting at, slammed his hands on top of the table and kicked a classroom shelf over. The shelf and toys displayed on the shelf, fell over onto a four (4) year old child playing in the dramatic play area and the child received a bruise under his left eye as a result of the incident. Staff stated that ice was applied to the child's injury; however, his parents were not contacted. A facility incident report was given to the child's parent when the parent arrived to pick up the child at 3:30PM; however, the incident report contained information that was not true. The incident report stated that the male staff person was exercising with the children and kicked the shelf over while doing a kicking exercise. Staff further stated that several children in the classroom were crying and upset as a result of the incident. A DCBS Worker examined the child the evening of the incident and observed a small bruise under the child's left eye.

Staff described the child having a mark on his face from where the shelf fell on him. The child's parent was given an incident report when the parent arrived to pick up the child at 3:30PM that day; however, the incident report contained information that was not true. The incident report stated that the male staff person was exercising with the children and kicked the shelf over while doing a kicking exercise. A staff person identified as working in the classroom at the time of the incident and identified as the staff person that wrote the incident report stated during interview, "I didn't want to write it in a way that would make the parent mad". The incident report was signed by this staff person and an administrative staff person. A visit to the facility was made to investigate allegations regarding two (2) staff getting into an argument in front of the children on 8/17/15 around 10:00AM, and a four (4) year old child being injured as a result of the incident. The staff person in charge identified the two (2) staff involved in the arguement during interview (hire dates: 9/11/14 and 1/1/11) and stated that both staff were escorted out of the facility immediately after the incident and they were placed on a two (2) day suspension. Interviews with administrative staff and staff present in the preschool classroom at the time of the incident and written statements provided by the staff involved, revealed that a male staff person working in the classroom and a female staff person that was visiting in the room with an infant at the time got into an argument and were heard screaming/yelling at each other and using profanity. During the argument the male staff person swiped a container of crayons off the table he was sitting at, slammed his hands on top of the table and kicked a classroom shelf over. The shelf and toys displayed on the shelf, fell over onto a four (4) year old child playing in the dramatic play area and the child received a bruise under his left eye as a result of the incident. Nineteen (19) children were identified as being present in the classroom at the time of the incident. Staff stated that several children in the classroom were crying and upset as a result of the incident. A DCBS Worker examined the child the evening of the incident and observed a small bruise under the child's left eye.
