General: Based on observation, on 04/22/25, at approximately 8:31 am, a staff member under investigation by DCBS in an ongoing investigation (# 234359) greeted a child and signed the child into the facility on an I-Pad. General: Based on observation, during review of video footage from 03/11/25 it was observed that a staff member would grab children by their upper arm and yank them toward their designated seat at the table. Programming Not In Compliance
General: Based on observation, interview and review of documentation/video footage, this regulatory requirement was not met: 1.) Staff members reported that one (1) staff member would mishandle children and speak to them in a loud manner/tone with intent to frighten them. The surveyor visited this facility on 03/19/25 and heard the staff member in question yelling at the children as they ate. 2.) The surveyor reviewed video footage from 03/11/25 and observed the staff member in question dragging a child by their arms to the facility kitchen. The child exited the kitchen, and the staff member smacked the child on the back of his head. The staff member was then observed to grab the child by their arm and yank them around toward her. General: Based on interview and review of documentation, the child care center failed to maintain state regulations. At the time of survey, the staff in charge had been the facility director for six (6) months. The previous director is still an owner, though no longer works in the facility. The executive director of the facility stated on 02/13/25 that they were unaware a change application needed to be filed and would do so. It should be noted that as of 03/04/25, there is no record of a change application being submitted in the provider records system. Programming Not In Compliance
General: Based on interview and review of documentation, the child care center failed to maintain state regulations. According to the facility's documented daily schedule, afternoon snack is served after rest time from 1:00 pm until 1:15 pm. The center does not close until 5:00 pm and therefore should serve a late afternoon snack beginning between 3:15 pm to 4:15 pm to meet meal schedule requirements that a meal should be served every two (2) to three (3) hours.
General: Based on observation, interview, and review of documentation, the child care center failed to maintain state regulations. The surveyor made two (2) visits to the facility: the first on 02/10/25 beginning at 10:40 am and the second on 02/21/25 beginning at 10:30 am. All children in care at the facility were on cots or in cribs (as age-appropriate), covered, some sleeping, and the lights in the center turned out for rest time during both visits. The surveyor concluded from these observations that the children in care had already been served lunch, eaten, cleaned their areas, and prepared for rest time. When referencing the schedule, the surveyor observed rest time was scheduled to end at 1:00 p.m. indicating that the regulation regarding the rest time limit of two (2) hours was not in compliance. Interview with three (3) staff members indicated that rest time does not begin until 11:00 am following lunch, which is served at 10:30 am, per staff. However, the surveyor observed children on cots before 11:00 am, some sleeping, during both visits to the facility. General: Based on review of documentation, it was determined that two (2) staff (DOH: 07/20/23 and 09/09/19) had evidence of only fourteen (14) hours of annual training on file for the last calendar year.
General: Based on review of documentation, the following was found: 1.) Two (2) children (DOE: 09/19/24 and 03/08/21) presented for review did not have documentation of a preferred hospital listed by a parent. 2.) One (1) child (DOE: 09/19/21) did not have a physician’s name and contact information on file at the facility.
General: Based on observation, the refrigerator in the Infant Classroom needs defrosted and did not contain a thermometer.

General: Based on observation and review of documentation, this regulatory requirement was not met. A staff member hired on 12/01/2002, did not complete the mandatory training on cleaning, sanitizing, health procedures, and mandatory reporting prior to the date of the center reopening. The ECE-TRIS system was reviewed.
General: Based on observation, the following was found: 1.) There was a staff member's purse laying on the floor of the three- and four-year-old classroom. 2.) During the initial walkthrough of the facility, the kitchen door was propped open leaving cleaners and rolls of trash bags accessible to the children. 3.) The facility's laundry room was accessible to the children, as there was no safety mechanism in place to prevent access. Laundry detergent was found in this area, as well as an unlocked cabinet which contained other cleaners.
General: Based on observation, the following was found: 1.) In the diapering area of the stingray classroom, there was missing paint and bleach streaks on the walls. 2.) In the three- and four-year-old classroom were areas of missing and chipped paint. 3.) In the bathroom off of the three- and four-year-old classroom, were bleach streaks behind the sink, as well as holes in the walls. 4.) In the school-age classroom, there were areas of missing/chipped paint throughout. 5.) In the bathroom off of the two-year-old classroom were large areas of missing paint. 6.) In the room off of the school-age room, there were areas of missing and chipped paint. 7.) In the bathroom off of the school-age classroom were bleach streaks on the wall, missing and chipped paint, and one stained ceiling tile. 8.) In the hallway outside of the school-age bathroom, there was a large crack in the drywall. This went the entire length of the wall and the wall underneath the crack was able to be moved when pushed. General: Based on observation, this regulatory requirement was not met. During a tour of the preschool classroom, the surveyor observed a cabinet full of dramatic play clothes in the children's play area. These items are considered machine washable toys; therefore, should be removed from the classroom.
General: Based on observation and interview, this regulatory requirement was not met. Upon arrival (10:37AM) the surveyor entered the infant room and observed an infant asleep in a bouncy seat. The surveyor toured the infant room a second time approximately twenty minutes later and observed the child still sleeping in the same position. The staff person working in the classroom stated that the child is a light sleeper and further stated that she did not have documentation stating that the child required an alternate sleeping position. General: Based on observation, interview and review of documentation, the following was found: 1. There were 31 children present in the facility at the time of the initial tour of the facility. A review of the facility's attendance records found that there were only 28 children signed in with an arrival time on the facility's attendance record. 2. There were 15 children signed in on the attendance form in the KKPS classroom; however, there were only 14 children present. In an interview with the staff present, it was found that one child leaves each day to go to another program then returns later in the day. General: Based on observation, the refrigerated chest being used to store milk for the children in the kitchen did not contain an indicating thermometer. It could not be determined that the interior temperature of the chest was 40 degrees or below.
General: Based on observation, the following was found: 1. There was a can of air freshener sitting on the children's table in the KKPS classroom. 2. There was a large bottle of Windex being stored in the utility closet in the KKPS classroom. The door to the closet was not locked giving the children present access to the Windex. 3. There was a large bottle of bleach being stored in the utility closet beside the hallway bathroom used by the children. A bottle of bleach was being stored in an unlocked cabinet under the sink in the hallway bathroom used by the children. 5. At the time of the visit (8:20AM) it was found that one (1) staff person was caring for a group of six (6) children (infants and toddlers) in the infant classroom. General: Based on observation, it was determined in the PreSchool Room that the surveyor found a vertical crack in the wall closest to the front window on the left side of the building. The crack ran vertically from the ceiling, down the wall. The crack was approximately twelve (12) to eighteen (18) inches in length. The crack was completely open near the top of the ceiling and had split, but became smaller in size as it ran down the wall. General: Based on observation, it was determined in the PreSchool Room that food which should have been refrigerated had been left out in the room for an unknown period of time. The surveyor found three (3) single serve packages of string cheese located on a shelf in the room. The packages were warm to touch and had been out for an undetermined amount of time. The packages stated to keep refrigerated. Upon speaking to the director, she stated string cheese and wheat crackers had been served as a snack the previous day. The director stated the packages must have been left on the shelf and not disposed of or refrigerated. The packages were in plain sight and easily reached by children in the room. General: Based on Observation, this regulatory requirement was not met. During a tour of the infant classroom the surveyor observed an infant asleep in a bouncy seat that was sitting in a crib. Staff working in the classroom identified the child as being four (4) months old. Premises Not In Compliance
General: Based on Review of Documentation, observation and interview, the following was found: 1. The long wall near the classroom table in the extra classroom (previous toddler classroom) contained approximatley fifty (50) staples. 2. The transition strip was missing that separates the tile floor from the carpeted area in the preschool classroom. An excessive amount of dirt particles and grime had collected in the area. The carpet in door way between the preschool classroom and the cubby room was also found in dis-repair. The carpet in this area was missing and frayed. 3. The surveyor observed three (3) ceiling tiles with dark stains in the preschool cubby room and one (1) ceiling tile with a dark stain near the exterior door that leads outside to the playground. A review of Division of Regulated Child Care files found that these regulatory violations were previously issued as a result of a relicensure survey conducted on 11/7/17. The director had indicated on a Plan of Correction submitted 1/3/18 that the staples were removed from the wall on 11/10/17, ceiling tiles will be replaced by 1/8/18 and she stated that she was in process with the landlord to repair/replace the carpet by 2/1/18. Two (2) staff files (hire dates: 4/18/17 and 5/22/17) did not contain evidence of the six (6) hours of cabinet-approved training being completed within the first three (3) months of employment. 2. General: Based on Review of Documentation, the following was found: 1. Two (2) civil penalty notices that were issued to the facility on 12/7/15 and 11/22/16 were not posted. 2. General: Based on Review of Documentation and interview, this regulatory requirement was not met. A review of the transportation logs found no evidence that daily inspections of the 2003 Chevrolet vehicle used to transport children had not been documented. The staff person in charge stated that children are transported on a daily basis in this vehicle and indicated that the daily inspection sheets have either been misplaced or accidentally thrown away. General: Based on Observation, this regulatory requirement was not met. An inspection of the facility's 2003 Ford van (fifteen (15) passenger) found numerous candy wrappers, dirt and debris scattered on the floor from the front to the back. Transportation Not In Compliance
General: Based on Interview and a review of documentation, on 12/12/16 a staff member in the infant room made statements such as: "I'm going to strangle that one directly" and "If you scream again I'm going to put you in the bouncy". General: Based on Observation, 1. Upon arrival there were fifteen (15) children in the eating area being cared for by one (1) staff person. The children in the group ranged in age from six (6) months to five (5) years old. The required ratio for these ages is one (1) staff per five (5) children with a maximum group size of ten (10) children. 2. There were eight (8) children with one (1) staff person in the Ones classroom. The staff person indicated that all of the children were between twelve (12) and twenty-four (24) months old. The required ratio for this age group is one (1) staff per six (6) children. General: Based on Observation, upon arrival at 7:50 a.m. there were two (2) infants sitting in car seats on the floor of the eating area. In an interview with staff it was found that the children had arrived at 7:40 a.m. Another infant arrived at the facility at 8:05 a.m. and was placed on the floor of the eating area in his car seat. The children remained in their car seats until 8:10 a.m. when they were taken to the Infant classroom. At that time a staff person took the children out of the car seats. General: Based on Observation, at the time of the survey lunch was observed. During lunch children in the 1's and toddler room were placed in feeding tables. One (1) child was observed reaching for a carton of milk but the staff person quickly moved the carton of milk and told the child they could not have the milk until she ate her food. Cartons of milk were observed sitting in the center of each table and were not given to the children until the children's plates were cleared away.
General: Based on Observation, during a tour of the 1's room there were several zebra print vinyl mats sitting on a shelf that were torn. An interview with the staff person in the room found that the mats are used for sitting on when reading. There was also a wooden shelf in the room that had numerous areas of chipping and peeling paint. The changing table located in the bathroom located off of the 1's room had an end piece that had come loose from the top of the changing table. During a tour of the toddler room there was a changing table that had a drawer that had been repaired with blue duct tape. In addition one of the legs had been repaired and secured to the rest of the changing table with clear packing tape. An interview with the staff person in this room indicated that the changing table was like that when she started working in the room. Also during an inspection of the vehicle with the name of the facility on it it was found that one (1) of side windows did not shut all of the way. During an inspection of both of the vehicles used to transport children it was found that the floors were covered in crumbs, food wrappers and other debris.

General: Based on Review of Documentation and interview, this regulatory requirement was not met. Interviews with staff during an investigation revealed that staff-to-child ratios were not in compliance on 8/17/15 while a staff person cared for an eight (8) month old child in the preschool classroom. Staff stated that the staff person was not clocked-in at the time; however, went into the infant classroom to get the eight (8) month old child and then brought the child into the preschool classroom and cared for the child in the preschool classroom approximately fifteen (15) minutes from approximatley 10:00AM - 10:15AM. Staff further stated during interview that this staff person (hire date: 1/11/11) often carries the eight (8) month old child around the facility because the child is very attached to the staff person. A review of facility attendance records found that eighteen (18) children (three (3) and four (4) year olds) were present in the preschool classroom on 8/17/15 from 10:00AM - 10:15AM and the eight (8) month old child made a total of nineteen (19) children. Two (2) staff that were on the clock and two (2) staff that were not currently clocked in were identified as being present in the room during this time frame. Director Requirements
General: Based on Review of Documentation and interview, the facility director failed to provide for the safety and comfort of a child. A visit to the facility was made to investigate allegations regarding two (2) staff getting into an argument in front of the children on 8/17/15 around 10:00AM, and a four (4) year old child being injured as a result of the incident. The staff person in charge identified the two (2) staff involved in the arguement during interview (hire dates: 9/11/14 and 1/1/11) and stated that both staff were escorted out of the facility immediately after the incident and they were placed on a two (2) day suspension. Interviews with administrative staff and staff present in the preschool classroom at the time of the incident and written statements provided by the staff involved, revealed that a male staff person working in the classroom and a female staff person that was visiting in the room with an infant at the time got into an argument and were heard screaming/yelling at each other and using profanity. During the argument the male staff person swiped a container of crayons off the table he was sitting at, slammed his hands on top of the table and kicked a classroom shelf over. The shelf and toys displayed on the shelf, fell over onto a four (4) year old child playing in the dramatic play area and the child received a bruise under his left eye as a result of the incident. Staff stated that ice was applied to the child's injury; however, his parents were not contacted. A facility incident report was given to the child's parent when the parent arrived to pick up the child at 3:30PM; however, the incident report contained information that was not true. The incident report stated that the male staff person was exercising with the children and kicked the shelf over while doing a kicking exercise. Staff further stated that several children in the classroom were crying and upset as a result of the incident. A DCBS Worker examined the child the evening of the incident and observed a small bruise under the child's left eye.
General: Based on Review of Documentation, it was found that the most current lesson plan for the preschool classroom was dated for 04/27/2015 to 05/01/2015.

General: Based on Review of Documentation, observation and interview, this regulatory requirement was not met. An inspection of the main vehicle (Chevy fifteen (15) passenger van) found numerous pieces of debris (dirt particles, food wrappers and chunks of foam from seat cushions) all over the carpet. The carpet also contained numerous dark spots and stains. Three (3) bench seats in this vehicle were grossly in dis-repair. The seats were torn from end to end on the area where the children sit which exposed the foam filling. The arm rest on the front bench seat was missing which created a hole in the side of the seat. The director stated that she purchased covers for the seats; however, indicated that they will not stay on. During a tour of the infant classroom the surveyor observed three (3) cans of Glade air freshener that was labeled "Keep Out of Reach of Children" and one (1) can of Great Value Spray Disinfectant labeled "Keep Out of Reach of Children" being stored in an unlocked cabinet under the sink; therefore, these items were accessible to the children in the classroom.
General: Based on Observation, the following was found: 1. The surveyor observed sticky tape residue all over the top surface of a red classroom table in the preschool classroom. 2. An inspection of the back up vehicle (Ford fifteen (15) passenger van) found numerous pieces of debris (dirt particles, food wrappers and pieces of paper) all over the carpet. 3. An inspection of the main vehicle (Chevy fifteen (15) passenger van) found numerous pieces of debris (dirt particles, food wrappers, empty drink containers and pieces of paper) all over the carpet. The carpet also contained numerous dark spots and stains. Three (3) of the bench seats in this vehicle were grossly in dis-repair. The seats were torn from end to end on the area where the children sit which exposed the foam filling. The arm rest on the front bench seat was missing which created a hole in the side of the seat. The van driver observed transporting children during the visit stated, "We have seat covers, but they won't stay on". The surveyor observed seat covers laying in the floor under the seats.
General: Based on Observation, this regulatory was not met. The following was found: 1. During a tour of the two (2) year old classroom, the surveyor observed two (2) electrical cords from the refrigerator and a radio were observed hanging loose and accessible to the children. 2. During a tour of the infant classroom the surveyor observed a pile of approximatley six (6) dirty tissues lying on a shelf beside the sink. The staff working in the classroom stated that they did not have a trash can/bag because they had just transitioned into the room the morning of the visit.

General: Based on Review of Documentation, the following was found: 1. One (1) staff person (hire date: 9/12/17) did not have T.B. documentation on file. 2. Three (3) staff files (hire dates: 11/15/15, 11/27/01 and 5/22/17) contained results of negative T.B. tests that are no longer current. The tests were dated 11/5/15, 9/15/15 and 11/2/15 which exceeds the two (2) year time frame.
General: Based on Observation, the following was found: 1. The long wall near the classroom table in the toddler classroom contained approximately fifty (50) staples. 2. The wall beside the refrigerator in the two (2) year old classroom contained excessive tape residue. 3. Three (3) different sections of carpet (approximately ten (10) - twelve (12) feet long each) was observed in dis-repair. These areas had threads that were missing and/or frayed. Duct tape was being used to cover the areas, but it was curling and peeling up which left a sticky residue. 4. The transition strip was missing that separated the tile floor from the carpeted area in the preschool classroom. An excessive amount of dirt particles and grime had collected in the area. The carpet in door way between the preschool classroom and the cubby room was also found in dis-repair. The carpet in this area was missing and frayed. Furthermore, several areas of the wall to wall carpet in the preschool classrroom had colored Duct tape to define different sections of the room. Most of the tape had peeled off and left a sticky residue. 5. The surveyor observed three (3) ceiling tiles with dark stains in the preschool cubby room and one (1) ceiling tile with a dark stain near the exterior door that leads outside to the playground. 6. The door frame entering the infant classroom contained numerous areas of chipped/peeling blue paint.
General: Based on Observation, this regulatory requirement was not met. The water temperature at the sink in the two (2) year old classroom was measured with a calibrated thermometer and found to be 130 degrees Fahrenheit, which exceeds the maximum allowable temperature of 120 degrees Fahrenheit. General: Based on Review of Documentation, this regulatory requirement was not met. A review of the emergency drill log in the transportation binder found that the facility had not practiced monthly emergency procedures since April 2017.
General: Based on Review of Documentation and interview, this regulatory requirement was not met. Based on review of documentation the facility did not have written safety procedures for transporting a child, loading/unloading a child and providing adequate supervision of a child during transportation. The staff person in charge stated that she thought she had this documentation somewhere; however, was not able to locate it during the visit.
General: Based on Observation and interview, this regulatory requirement was not met. The surveyor observed the facility van pull up to the facility with six (6) children and two (2) staff on board (driver and monitor). The monitor escorted the children off the vehicle and into the building while the van driver remained on the vehicle to assist with the inspection. The van driver identifed the ages of the children observed getting off the vehicle as an eight (8) month old, three (3) children that are two (2) years old and two (2) children that are three (3) years old. The surveyor observed the eight (8) month old getting off the vehicle in a car seat; however, only observed one (1) other car seat inside the vehicle during the inspection. The van driver stated that one (1) of the two (2) year old children rode in the car seat observed present on the van and indicated that the other four (4) children (two (2) - three (3) year olds) were transported with a regular seat belt. Therefore, these children were not provided with a safety seat.
General: Based on Review of Documentation, this regulatory requirement was not met. Record review revealed no written documentation indicating that a daily inspection of the facility’s Chevy fifteen (15) passenger van was performed on 11/3/17. On this day, the vehicle was used to transport children.
General: Based on Observation, this regulatory requirement was not met. During a tour of the classroom the surveyor observed one (1) infant sitting in a low rocker chair while drinking a bottle and another infant sitting on the floor while drinking from a sippy cup. An interview with the staff person in charge found she was unaware the certificates had been altered. During an interview with the staff person hired 9/1/15 indicated that she graduated from Henry Clay High School in 2012. However, there is a copy of a GED on file for this person. An interview with the other two (2) staff indicated that they both have a GED.
General: Based on Observation, upon entry into the one (1) year old room there was only one (1) staff person visible with nine (9) children ages one (1) year old. The children were observed playing with toys. A few minutes later a door opened and a staff person was observed walking out of the room behind the closed door. An interview with the staff person found that the room she came from is a bathroom.
General: Based on Observation, there was a small tear in the changing mat in the toddler room. During a tour of the infant room a staff person was observed changing a child's diaper. The staff person did not sanitized the table after changing the child's diaper.
General: Based on Observation, during a tour of the one (1) year old room the wall where a tall cabinet is located had numerous areas of chipping and peeling paint. In the early 3's room the wall with the alphabet across the top had numerous areas of chipping and peeling paint. During a tour of the toddler room there was a large run in the carpet.
General: Based on Review of Documentation, there were two (2) staff with hire dates of 4/30/15 and 9/1/15 who have evidence of an altered GED certificate on file. There was an additional staff person hired 8/21/15 who is still enrolled in high school but there was no evidence on file to verify this staff person is currently enrolled in high school. An interview with the staff person in charge found that she was unaware the GED certificates had been altered. General: Based on Observation, at the time of the survey a staff person was observed changing a child's diaper in the infant room. The staff person then took the child a child and sat down with the child on her lap. The staff person then wiped the child's nose and touched another child sitting in a bouncey seat in front of her on the floor. The staff person did not wash her hands after changing the child's diaper and she did not wash her hands after wiping the child's nose. The other staff person in the room told the her to wash her hands. The staff person went and washed her hands and then came back and got the dirty tissue she had used to wipe the child's nose and threw it away. As she walking to the trash can to throw the dirty tissue away she picked a toy up off the floor and then went and washed her hands again after touching the toy.
General: Based on Observation, during a tour of the facility there were numerous overhead lights that were either out or flickering on and off. During an interview with the staff person in charge it was found that some of the lights had been repaired recently.
General: Based on Review of Documentation, there were three (3) staff with hire dates of 8/21/15, 8/20/15 and 4/30/15 who did not have evidence of having completed six (6) hours of orientation training.
General: Based on Observation, during a tour of the three (3) to five (5) year old room it was found that the planned program of activities posted was dated for 9/23/15-9/26/15. In the 1's room, the early 3's room and the infant room the word November was listed on the planned program of activities but there were no dates.
General: Based on Observation, during an inspection of one (1) of the vehicles used to transport children a can of Lysol spray was observed in the open console between the driver and front passenger seats. The van had been used to transport children that morning. The label on the can indicates to "Keep Out of the Reach of Children".
General: Based on Observation, during a tour of the eating area there was a large wall vent that was covered in dried food. In the hallway where there is a back door located there was a water stained ceiling tile. There was also a water stained ceiling tile in one of the bathrooms located off of the 3-5 year old room.

During a tour of the 3-5 year old room there were several blue and red nap mats that were torn with foam exposed. It was also found that these red and blue mats were apporximately one (1) inch thick. An interview with staff found that children nap on these mats. During a tour of the toddler room there were three (3) torn nap mats with foam exposed.
General: Based on Observation, during an inspection of one (1) of the facility vans it was found that the first aid supplies did not include band aids or gauze. In addition this same vehicle did not have reflective triangles.
General: Based on Review of Documentation, there were three (3) children's files that did not contain information regarding the name and phone number of the child's physician. In addition one (1) of those children's files did not contain the name of a preferred hospital.
General: Based on Review of Documentation and interview, this regulatory requirement was not met. Interviews with administrative staff and staff present in the preschool classroom on 8/17/15 revealed that a four (4) year old child was injured during an argument between two (2) staff when a male staff person kicked a shelf containing toys and it fell over onto the child. Staff described the child having a mark on his face from where the shelf fell on him. The child's parent was given an incident report when the parent arrived to pick up the child at 3:30PM that day; however, the incident report contained information that was not true. The incident report stated that the male staff person was exercising with the children and kicked the shelf over while doing a kicking exercise. A staff person identified as working in the classroom at the time of the incident and identified as the staff person that wrote the incident report stated during interview, "I didn't want to write it in a way that would make the parent mad". The incident report was signed by this staff person and an administrative staff person. General: Based on Review of Documentation and interview, this regulatory requirement was not met. A visit to the facility was made to investigate allegations regarding two (2) staff getting into an argument in front of the children on 8/17/15 around 10:00AM, and a four (4) year old child being injured as a result of the incident. The staff person in charge identified the two (2) staff involved in the arguement during interview (hire dates: 9/11/14 and 1/1/11) and stated that both staff were escorted out of the facility immediately after the incident and they were placed on a two (2) day suspension. Interviews with administrative staff and staff present in the preschool classroom at the time of the incident and written statements provided by the staff involved, revealed that a male staff person working in the classroom and a female staff person that was visiting in the room with an infant at the time got into an argument and were heard screaming/yelling at each other and using profanity. During the argument the male staff person swiped a container of crayons off the table he was sitting at, slammed his hands on top of the table and kicked a classroom shelf over. The shelf and toys displayed on the shelf, fell over onto a four (4) year old child playing in the dramatic play area and the child received a bruise under his left eye as a result of the incident. Nineteen (19) children were identified as being present in the classroom at the time of the incident. Staff stated that several children in the classroom were crying and upset as a result of the incident. A DCBS Worker examined the child the evening of the incident and observed a small bruise under the child's left eye.
General: Based on Observation, the bathroom used by the preschool children did not have any paper towels.
General: Based on Observation, the preschool children's individual blankets that were kept in their cubbies were all touching keeping them from being stored in a sanitary manner.
General: Based on Observation, one (1) infant was awake in a crib with a bottle. There was also one (1) infant in a bouncy seat in a crib holding a bottle.
General: Based on Review of Documentation, sibling children enrolled on 09/08/14 did not have the preferred hospital information documented on their enrollment forms.
General: Based on Review of Documentation, a review of seventeen (17) staff files found that nine (9) of those staff who had been employed for more than one (1) year had not completed fifteen (15) hours of training for the licensure year.
General: Based on Observation, it was found that several of the children's coats and bags were observed touching one (1) another outside of the one (1) year-old room on the hooks provided to hang the children's personal belongings. General: Based on Review of Documentation, there was one (1) staff person hired 10/21/13 who had the results of a criminal records check dated 11/7/13. However, there was no documentation on file to verify the application for the results of the criminal records check was submitted prior to employment or on the date of hire.
General: Based on Review of Documentation, there is a staff person who is reportedly a senior in high school. The documentation on file to verify enrollment in school was for the 2010-2011 school year.
General: Based on Review of Documentation, there was one (1) staff person hired 7/22/13 who did not have the results of a T.B. skin test read until 7/24/13 which is after the staff person's hire date. There was an additional staff person hired 10/21/13 who had the results of a T.B. skin test dated 11/4/13 which is after the staff person was hired.
General: Based on Observation, it was found that the door to the furnace contained a hole near the bottom. This furnace door was located adjacent to the preschool room.
General: Based on Observation, it was found in the preschool bathroom the light bulb over the sink was missing a cover and the bulb was exposed. The exposed bulb did not appear to be shatter proof.
General: Based on Observation, it was found that the wall in the 2 year-old bathroom next to the sink contained a crack. An area of chipped paint was observed on the wall in the infant room.
General: Based on Observation, it was found that a staff member wiped a child's nose in the one (1) year-old Dolphin room and the staff member did not wash her hands after cleaning the child's nose.
General: Based on Review of Documentation, found that out of the ten (10) children's files reviewed there was not a preferred hosptial listed in any file.
General: Based on Observation, it was found that a medication bottle with some type of ointment inside it was located under the changing table in the one (1) year-old bathroom. The label was torn off of the bottle. The medication was not in a separate locked place and it was not properly labeled with a child's name. Outdoor Play Area
General: Based on Observation, it was found that the facility had four (4) cirbs that were not in compliance with the federal safety standards issued by the Consumer Product Safety Commission established in CFR 1219-1220. The cribs were not being used for the children to nap, but they were assembled and present in the facility.
General: Based on Observation, it was found that approximately six (6) mats were observed in the preschool room that were not two (2) inches thick. The mats were measured and they were one (1) inch thick. In addition, in the two (2) year-old room eleven (11) mats were less than two (2) inches thick. During the relicensure the children were napping on these mats.
General: Based on Observation, it was found that on the 2003 Ford van used to transport the children one (1) of the seat belts was broken. In addition, a car seat in this van was in disrepair.
General: Based on Observation, it was found that in the one (1) year-old room (Dolphin room) the sink in the bathroom was not draining properly. Standing water was observed in the sink.
General: Based on Review of Documentation, it was found that earthquake and tornado drills are being conducted monthly along with the fire drills. However, the same time the fire drill was conducted is being documented as the same time fo the earthquake and tornado drill. There is not a separation of each drill documenting the time they were conducted.
General: Based on Review of Documentation, there were two (2) staff who did not have a TB skin test on file; the hire date was 6/25/12 for both staff.
