On February 2, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding use of a substitute. An investigation was conducted on February 7, 2024. This complaint is substantiated because the date and time when Substitute #1 was present with the children was not documented and the parent notification provided was photocopied signatures. However, Ms. Harvey stated the parents trust her and do not want to be informed when she leaves Substitute #1 alone with the children.

On September 6, 2023, the Office of Child Care Licensing (OCCL) received a complaint regarding lack of supervision, accident injury procedures, and use of a substitute. An investigation was conducted on September 9, 2023. This complaint is substantiated for accident/injury procedures and substitute use because Substitute #1 did not report to the parent that Child #1 had been bitten on the forehead after providing assistance to Child #1, and the parents were not notified before Ms. Harvey's absence that Substitute #1 would be used. In addition, the date and time of when Substitute #1 was present with the children was not documented. There is no evidence to substantiate the allegation of lack of supervision because Substitute #1 saw the incident but was changing a child and could not intervene. Additional concerns noted were an ointment was applied to a child without a Medication Administration Record, and child attendance was not being documented and was not available for review.

Fence off or restrict access to the area with the dog kennel, kerosene can, and other miscellaneous items on the side of the home.
