On 1/16/2024, a child was found at the end of the hallway on the first floor, by the door alone. The child had walked out of their classroom during toileting/diaper changing time. Staff were unaware the child had been missing for up to two minutes. In addition, on the date of 6/13/2023, the program self reported a similar incident and the Division did not cite a violation at that reported time. A violation is being cited at this time as there is now a pattern of a break down in supervision. This is a serious violation.

Staff X brought four children inside to use the bathroom. One of the children refused to put back on their outdoor winter clothing, including boots, before returning outside to the playground. Staff X brought the four children back outside and onto the sidewalk while carrying one of the children's outdoor winter clothing and boots, and had the child walk with no shoes or boots on. Staff X talked to Staff Y for at least 30 seconds to two minutes, while the child screamed that their feet were cold and were hurting. Staff Z, on an adjacent playground, went and spoke to Staff Y of their concern of the child being outside without boots on and asked for Staff X to pick up the child. Staff X offered the child their boots, but the child refused, and Staff X did not pick up the child. Staff Y decided to bring all the children inside. Staff X had the child walk back inside. Making a child walk outside without boots and outdoor clothing on in the wintertime is not an acceptable response to a child's behavior and development.
The evacuation drill record reflected that drills were not conducted in October and November of 2022.
There are three staff present who have not submitted to the required fingerprint supported background check.
An audit of BFIS Quality and Credential Accounts and on site staff files show that Staff A and Staff B, who have been employed for over 30 days, have not completed an approved orientation training as required.
On February 6th, 2020, two staff in the Koala Bear classroom were sitting on the floor in the play area with five infants while three infants slept in the nap area. Two infants were in cribs and one was on a mat on the floor. Staff could not see the infant sleeping on the mat because of the closed solid gate and wall that is three feet in height separating the nap area from the play area. An eleven-month old infant sleeping on a mat woke up, left the classroom which had an open door and gate, and was found by a staff member of another classroom in the hallway approximately two feet from the entrance to the restroom. Staff stated the time the infant left until the time the infant was found in the hallway was between 11:05 and 11:10. Staff stated they were not aware the infant had woken up and left the room. The door to the classroom was closed and the gate was open when the infant was found in the hallway. Because staff were not able to see the sleeping infant on the nap mat on the floor in the nap area, were not aware the infant woke up, left the classroom, and was found in the hallway near the restroom, this is a lack of supervision. This is a serious violation.
Staff stated an eleven-month old infant in the Koala Bear classroom has been sleeping on a nap mat and the infant had not attempted to climb out of the crib prior to transitioning to the nap mat. Infants under the age of 12 months are to sleep in a crib or port-a-crib unless able to climb out on their own.
As the school age classes were preparing to leave the center to go to a local beach to swim, the licensor asked a staff person if she and other staff had access to the program's aquatic plan. The staff person indicated she had not seen the written plan and neither had the other school age staff. When the licensor asked the director if there was an aquatic plan, the director presented an aquatic plan that was written in 2014 and had not been updated nor shared with staff. The director said staff were trained on aquatic safety during staff meetings and through written communication from the child care director and aquatic director. When the licensor asked the director and school age staff person if the program took a copy of the aquatic plan with them on swimming field trips, both the director and staff person said no. The director and school age staff decided not to take the children swimming on the day of the visit and said they would not take any swimming field trips until the aquatic plan was updated and shared with staff.
A three-year-old child left his classroom, walked down the hallway, down the stairs and exited the double doors to the parking lot without his teachers noticing his absence. The child was found by another parent in the parking lot. The classroom teachers were not aware the child left the classroom until he was brought back by the director. It was confirmed he was out of his classroom for approximately 5 to 6 minutes. A child being unsupervised and leaving the building without staff knowing he is missing is a serious violation.

Staff A who was hired in September 2021 and is in a position to be left alone with children, has not complied with the full background clearance screening as required. A reminder email was sent to Staff A on 11/29/2021. Staff B who was hired in August 2019 and is in a position to be left alone with children, has not complied with the full background clearance screening as required. Staff B was sent background clearance forms to complete in August 2021.
A review of the staff BFIS Quality and Credentialing accounts shows that multiple staff at the program, who have worked at the program for longer than six (6) months, have incomplete BFIS Quality and Credentialing accounts.
