A child's prescription rescue medication was found to have expired in September 2023. A bottle of over the counter medication for the same child was also found to have expired in May 2025.

A review of BFIS accounts and staff files show that Staff A has an expired CDA and that Staff B has not completed a Fundamentals course or a three (3) college credit course within the first 12 months of working in the program.
This site visit inspection recorded no violations or advisories.
There were 13 children present and only 11 children documented as present on the attendance clipboard.
The program does not have an indentified business manager, nor has the program director completed a course including budgeting and financial management.
A review of staff BFIS Quality and Credentialing accounts and onsite staff files show that Substitute A does not have current First Aid and CPR certification.
A review of the staff BFIS Quality and Credentialing accounts shows that all six (6) staff at the program, who have worked at the program for longer than six (6) months, have incomplete BFIS Quality and Credentialing accounts. All six (6) staff are missing qualification documentation, First Aid and CPR certification documentation, annual professional development information, and mandated reporting information.
A review of the staff BFIS Quality and Credentialing accounts and onsite staff files show that Staff A, B, C, and D do not have training in mandatory reporting of child abuse and neglect.
