A household member has not completed the fingerprint supported background clearance as required. The fingerprint authorization certificate was issued on 6/19/2019 and 7/27/2022. This violation was previously cited on 2/17/2022 and 2/10/2023. Further regulatory action may be taken should non-compliance continue.
A review of the Provider's BFIS account and conversation with the provider confirms that no professional development was completed for the year 2023. It is required that 15 hours of professional development be completed annually.

A review of the provider's BFIS Quality and Credential Account and a conversation with the provider confirms that CPR and First Aid certificate and Protecting Vermont's Children: Reporting Abuse & Neglect training have not been sent to Northern Lights at CCV to be entered into BFIS Quality and Credential Account as required.
