The licensing coordinator's review nine children's files revealed that one child's registration and emergency information form was not the equivalent of the April 2022 form, as the forms were missing the required statement under He-C 4002.10(c). The program was previously cited for non-compliance with this rule following a June 21, 2023 visit. A corrective action plan submitted by the program and approved by the department stated, "I have sent home the April 2022 form with all the parents asking them to return the new forms ASAP. I have noted when all registration forms are to be reviewed, signed, and dated. I will be using the most updated form going forward." Corrective action: The registration and emergency form for this child was emailed to the parent on January 23, 2024. The parent filled out and returned the form the next day. I have noted in my planner when each child's forms need to be updated or renewed. When checking child's folders are will do a more thorough job so children will not be missed. I will be sure to use the 2022 or most recent version of the form.
The licensing coordinator's review of nine children's records revealed there was no record of physical examination on file for one child who has been enrolled and attending the program for more than 60 days. The program was previously cited for non-compliance with this rule following the March 11, 2021, April 14, 2022, and June 21, 2023 visits. A corrective action plan submitted by the program and approved by the department stated in part, "I have also noted when children need to renew/update physical examination records to prevent this from happening again." The licensing coordinator's review of nine children's records also revealed there was no updated record of physical examination on file for one child younger than 6 years of age. The program was previously cited for non-compliance with this rule following the April 14, 2022 and June 21, 2023 visits. A corrective action plan submitted by the program and approved by the department stated in part, "I have noted when children are due for a new physical form in hopes of prevention of this violation." Corrective action: On January 23, 2024 a health assessment form has been sent home with the parents. I have changed my enrollment requirements to include physical examination, along with immunizations must be on file on first day of attendance.
The licensing coordinator's review of the program's written fire drill log revealed that the program had conducted one component of its Emergency Operations Plan in the 2023 calendar year when two are required. The program was previously cited for non-compliance with this rule following a June 21, 2023 visit. A corrective action plan submitted by the program and approved by the department stated in part, "I will continue to practice the required number of components of the EOP. I will mark all EOP drills on my calendar, set reminders, and include them in my curriculum planning." Corrective action: On Friday, January26 we conducted a drop and cover drill. I understand that I have been cited for this rule at a prior visit as well, but I misunderstood my licensing coordinator and was under the impression that the off site drill I conducted on January 12, 2023 also counted. I will continue to practice the required number of components of the EOP. I will mark all EOP drills in my planner and include them in my curriculum planning. I will use a different logging sheet each year. I will hold 3 components of the EOP in 2024.
The licensing coordinator's review the program's written fire drill log revealed that the program did not conduct a fire drill during the months of August 2023 through December 2023. The program was previously cited for non-compliance with this rule following the March 11, 2021, April 14, 2022, and June 21, 2023 visits. A corrective action plan submitted by the program and approved by the department stated in part, "I will conduct a fire drill every month. I will mark fire drills on my calendar, set reminders, and include them in my curriculum planning." Corrective action: On Tuesday, January 30 I conducted a fire drill at 9:30 am. I will conduct and document monthly fire drills. I will do 2 fire drills 4 months in the year 2024, in addition to conducting one fire drill per month. I will note monthly fire drills in my planner.
This Statement of Findings includes repeat citations for He-C 4002.10(b), He-C 4002.10(h), He-C 4002.10(i), He-C 4002.17(a)(1) and He-C 4002.17(f)(2), which constitutes failure to comply with approved corrective action plans for five repeat citations, as noted herein. Corrective action: I understand that I have been cited multiple times for the rules. I admit my paperwork and documentation skills need some work. I have written in my planner the dates each child needs a new or updated emergency registration plan, health assessment or immunizations and when I will conduct drills and what drill I conduct, as well as, document the drill conducted . I continue to work on bettering these skills. I am confident that these rules will not be cited at my next annual visit.
The licensing coordinator's review of two staff files revealed that one staff was not certified in CPR and First Aid within their first 90 days of employment with the program. Corrective action: On February 6, 2024 this staff completed CPR and first aid. I have noted in my planner when this staff CPR and first aid will expire to be sure CPR and first aid is always current. I will update my employee handbook to reflect the requirement that CPR and first aid must be completed within 90 days of hire date to continue employment to remain employed at Way To Grow Preschool.
The licensing coordinator reviewed the staff and household list provided by the program and found that three household members had not been determined eligible by the Unit. Corrective action: I am working on correcting this. I have been told by my licensing coordinator they all have NHCIS please do not register again. The 3 household members have never registered for an account. When we try to log in using their emails and reset a password It states "no account found". I have submitted a support ticket on NHCIS today (January 30, 2024) to try to resolve this issue. Karleigh did do a background check and fingerprinting in July 2022. I followed the directions from the NH licensing bureau site however it seems I must've done something wrong since nothing was ever received. I received a response to my support ticket on January 30, 2024. I will have each household member register for a NHCIS account, submit a background check application and schedule a fingerprinting appointment ASAP.
The licensing coordinator's review of two staff files revealed that one staff did not complete the health and safety training regarding the medication administration, emergency preparedness and response planning, building and safety of physical premises, and handling and storage of hazardous materials within 90 days of hire. Corrective action: This staff worked from only worked from June26, 2023 until August 4, 2023. This staff then had football training, return to school and football games and practices and was not working again until November 27, 2023. Since this staff had the first initial 6 health and safety trainings I assumed she would be good until February 27, 2024. On January 25, 2024 this staff completed the administering medication in childcare settings. On February 6, 2024 this staff completed the building and safety of physical premises and half the handling and storage of hazardous materials. This staff will complete the rest of this training February 7, 2024.
