All center directors, agency administrators, site coordinators, or site directors, and all other child care staff who are responsible for the supervision of children, or who are necessary for the staff to child ratios, shall keep on file documentation of completion of a minimum of 6 hours of professional development, which shall be completed in accordance with the following: (1)  Within 90 days of the first date of employment; (2) Within 2 weeks for programs operating 3 months of the year or less; or (3) By providing documentation of previous completion.

REPEAT CITATION PREVIOUSLY CITED ON APRIL 22, 2022, AND OCTOBER 2, 2024. The licensing coordinator's review of four staff files revealed that three full time staff members did not complete 18 hours of professional development in 2024, and one staff member had not completed all of the health and safety trainings within 90 days of employment. This constitutes the second repeat citation and failure to comply with two approved Corrective Action Plans regarding completion of annual professional development hours, one repeat citation and failure to comply with one approved Corrective Action Plan regarding completion off health and safety trainings. Corrective action: I understand this is a repeat violation and it solely rest on me as a director. I instructed my staff to have all education hrs. needed for 2025 by Feb 28th as I was submitting for renewal. they did! However I did not realize with this visit those hr. cannot count. still a tad confused. however, i have created a monthly log going forward to ensure staff gets a minimum of 4 hrs. each monthly going forward. As for myself i need to be more proactive as well on self recording

Child care staff shall ensure that for each child, upon the child’s first day in attendance in the program, there is a “Child Care Registration and Emergency Information” (April 2022) form or an equivalent form provided by the child care program completed and signed by the parent, on file, which contains: 1) Full legal name of the child; (2) Child’s date of birth; (3) Child’s physical address and mailing address; (4) Child’s home telephone number; (5) Date of enrollment in the program; (6) The name, physical address, and mailing address of the parent(s) responsible for the child, if different from the child’s address; (7) Telephone numbers for the child’s parents and instructions as to how the parents can be contacted during the hours that the child is at the program; (8) Email addresses for the parents, if available; (9) Names and telephone numbers of at least one person who will assume responsibility for the child, if for any reason, the parents cannot be reached immediately in an emergency; (10) Any chronic conditions, allergies, or medications in case of sudden illness or injury; (11) Written parental permission for first aid treatment; (12) Written parental permission for emergency medical transportation and treatment; (13) The name and telephone number of each child’s physician or health care provider; and (14) Names and telephone numbers of any person(s) other than parents who are authorized to remove the child from the program.

REPEAT CITATION PREVIOUSLY CITED ON APRIL 22, 2022. The licensing coordinator's review of 10 child files revealed that the Child Registration and Emergency Information form had not been updated annually for five children. This constitutes one repeat citation and failure to comply with one approved Corrective Action Plan regarding child registration forms. Corrective action: we have gone through each enrolled child's file and update with signature. we have selected to have all child's files be renewed or updated the First week in January of every new year .

In order to assure that staff and children are familiar with all of the EOP drills, programs shall: (1) Practice evacuation off-site and bomb threats once per year with all child care staff; (2) Practice at least 2 other components of their EOP with all child care staff and children each calendar year; and (3) Review all EOP response actions with all staff at least 4 times each calendar year.

Two licensing coordinator's review of program documents revealed that the Emergency Operation drills required for only staff had not been conducted. The provider confirmed that the staff only drills were not completed. Corrective action: I have assigned another staff member the task to read and go through with each staff member to assure they understand the process, as well as created a sign off log. We do discuss this often and done a walk through, however never conducted. we have added to our fire log (back) a log to track when we conduct this. broken down to every 4 months. Also Oct 26th all staff will be attending a mandatory lock down & shelter in place event in our town.

A written record of a health screening for all child care staff, household members and other individuals who have 5 or more hours per week of contact with children shall: (1) Be on file at the program and available for review by the department for all child care staff within 60 days of the date of hire or the date the household member or individual begins having daily contact with children; and (2) Include, at a minimum, the information on the “Child Care Personnel Health Form” (11/2017) provided by the department, or its equivalent.

Two licensing coordinator's review of five staff files revealed that four did not contain documentation of a health screening for staff who had been employed for longer than 60 days. This constitutes the second repeat citation, and failure to comply with two approved corrective action plans. Corrective action: I have created all new folders for current staff to bring current. as well as awaiting on staff appoints to finish adding documentations to their said folder. we will also be starting bright wheel Nov 4th and I'm hoping this will help us track needed info
