The licensing coordinator’s review of 12 injury reports revealed that six did not contain the time and method by which the parent/guardian was contacted, and one was not signed by a parent/guardian. Corrective action: We have reviewed the injury/incident reports, ensured completion and reviewed the forms and how to complete them correctly with all staff. We have reviewed in detail with all staff that when notifying families via our school app of an injury/incident, we also must note that information on the injury/incident form. We reviewed that all forms of communication must be noted on the injury/incident report form with the date and time notified. We will ensure compliance with these rules and applicable statues in the future by the following: We have highlighted areas on the forms and made copies for staff to use and reviewed them with all staff to ensure they are fully completed. We have implemented a two-step review process. Injury/incident report forms will be given to the Co-Director for review and then to the Owner/Director for final review, completion and filing. The citation was corrected immediately following the licensing visit on 5/23/24.
