The licensing coordinator's review of the providers' file revealed they did not renew the medication administration training yearly. Corrective action: The ProSolution Medication Administration training was completed after the Licencing Staff visit and reminder. A reminder calendar yearly has been created to remind the administration staff to renew the training next year.

This Statement of Findings includes a repeat citation and failure to comply with approved Corrective Action Plans for He-C 4002.10(a) as noted herein. Corrective action: This online corrective plan is new and much more efficient. I will start using this online corrective plan moving forward. Previous years, I have been receiving emails and physical paper Statement of Findings sent through mail and email and they have requested me for written correction plan on the paper and to emailed back to the previous License Coordinators.

Any contaminated, expired, or discontinued medication, whether prescription or over-the-counter, and topical substances shall be returned to the child’s parents or, if belonging to the program, disposed of properly by authorized staff.

The licensing coordinator observed one Epi-pen that had expired in June 2023. Corrective action: Expired Epi pen was returned to parent and a new epi pen was received 12/12/24 (Expired 12/4/25). Added via email on December 19, 2024 with provider's approval, "Epi Pen expiration date will be input into my google calendar as a reminder to remind the parents for a new one."

Prior to employment or residency, as applicable, and every 5 years thereafter, all child care staff, substitutes, other employees, and volunteers who may be alone with children or are included in staff to child ratios, and household members age 10 years or older, shall submit for a background record check using NH Connections.

The licensing coordinator's review of department records revealed two household members did not renew their background record checks prior to their 5 year expiration dates of December 19, 2021 and May 27, 2023. Corrective action: Both household members have scheduled their background checks on Dec 19th 2025. Reason for the eldest background check expiration might be due to her turning 21 years old on Dec 19. Added via email on December 19, 2024 with provider's approval, "household members will not have interactions with the program and children until deemed eligible by CCLU."

Child care staff shall ensure that for each child, upon the child’s first day in attendance in the program, there is a “Child Care Registration and Emergency Information” (April 2022) form or an equivalent form provided by the child care program completed and signed by the parent, on file, which contains: 1) Full legal name of the child; (2) Child’s date of birth; (3) Child’s physical address and mailing address; (4) Child’s home telephone number; (5) Date of enrollment in the program; (6) The name, physical address, and mailing address of the parent(s) responsible for the child, if different from the child’s address; (7) Telephone numbers for the child’s parents and instructions as to how the parents can be contacted during the hours that the child is at the program; (8) Email addresses for the parents, if available; (9) Names and telephone numbers of at least one person who will assume responsibility for the child, if for any reason, the parents cannot be reached immediately in an emergency; (10) Any chronic conditions, allergies, or medications in case of sudden illness or injury; (11) Written parental permission for first aid treatment; (12) Written parental permission for emergency medical transportation and treatment; (13) The name and telephone number of each child’s physician or health care provider; and (14) Names and telephone numbers of any person(s) other than parents who are authorized to remove the child from the program.

REPEAT CITATION, PREVIOUSLY CITED ON APRIL 26,2023. The licensing coordinator's review of five child files revealed that none of the Child Registration and Emergency Information forms were the equivalent of the April 2022 form, as the forms were missing the required statement under He-C 4002.10(c). Corrective action: Latest version of the Child registration forms were printed and signed by the parents. Moving forward will check the DHHS regulations for any updates of forms and adding it to the enrollment packet.
