This Statement of Findings includes repeat citations and failure to comply with approved Corrective Action Plans for He-C 4002.08(b) and He-C 4002.21(u),as noted herein. The program failed to comply with two approved corrective action plans (CAPs) for August 31, 2023 regarding staff attendance and returning of expired medications. Corrective action: Director reminded employees about signing in and checks the attendance daily.

REPEAT CITATION PREVIOUSLY CITED ON AUGUST 31, 2023. The licensing coordinator observed an expired Honest brand sprayable diaper cream in the Toddler classroom. Corrective action: The Director created a re-occurring reminder the first of each month to check all cream expiration dates.

The program shall maintain daily child attendance records, in either paper or electronic format, including the child’s full name, date of birth, and their arrival and departure times in the program and each room or group throughout the day, as applicable, recorded in real time by staff or parents.

REPEAT CITATION PREVIOUSLY CITED ON AUGUST 31, 2023. The licensing coordinator's review of staff attendance records revealed one staff member did not sign in upon entering the classroom. Corrective action: Director will continue to remind teachers to sign in first thing in the morning, each time they cover a classroom, and when they leave.

He-C 4002.08(a) · Correction status not published

In order to assure that staff and children are familiar with all of the EOP drills, programs shall: (1) Practice evacuation off-site and bomb threats once per year with all child care staff; (2) Practice at least 2 other components of their EOP with all child care staff and children each calendar year; and (3) Review all EOP response actions with all staff at least 4 times each calendar year.

Staff A stated that the program has only reviewed their EOP once in 2023, when programs must review all EOP response actions with all staff at least four times per year. Corrective action: Director created a form to be filled out signed and dated four times a year to review the EOP with staff.
