This visit was conducted by Jennifer Davis, child care consultant with Alissa Long, on-site program coordinator. Currently this program operates with a FIVE (5) Star License issued 10/14/22. License restrictions include daytime care only; school age children only; allowed to provide care to children up to 14 years of age,meets enhanced ratios and space, reduced group size by 1 in each group from 7 pt. level; playground area does not meet child care playground safety guidelines. This program is operated by Yadkin County Schools. The license was posted, and restrictions were met. The center’s fire inspection was conducted on 9/16/2025. All indoor and outdoor areas were observed during the visit. Upon arrival staff/child ratio was observed and recorded on the attached worksheet. Supervision was adequate during the visit. I observed developmentally appropriate materials and activities were available to the children in the classrooms. I observed the children playing in activity areas during the visit. I observed positive interactions between staff and children. A current snack menu was posted. An allergy list was posted. Storage of hazardous items was monitored. No medications were reported. The program does not provide routine transportation. They will go on field trips this summer. Staff and children’s records were monitored per DCDEE procedures. The last fire drill was conducted on 2/20/2026. The last playground inspection was conducted 2/25/2026. The last shelter-in-place drill was conducted 1/22/2026. Prior to the visit, the program's compliance history was 92%. All programs are required to maintain 75% compliance. The following violation was cited during the visit: Violation Number Comment Rule 1824 The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. The EPR plan was not reviewed annually. .0607(e) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Additional information: • We discussed pathway options in October 2025 and the facility will process rated license under Pathway 1 option. Ms. Long stated that the ERS will be requested in Fall 2026. They are currently working on preparing. A three month self study will need to be completed prior to requesting the ERS. This can be found on the www.ncrlap.org website. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. Alissa Long, on-site program coordinator assisted me with the visit. Currently the program operates with a FIVE (5) Star License re-issued 10/14/22. License restrictions include daytime care only; school age children only; meets enhanced ratios and space, max group size 24; playground area does not meet child care playground safety guidelines; allowed to provide care for child up to 14 yrs of age. The program is operated by Yadkin County Schools. The program’s compliance history was 90% prior to today’s visit. Licensed programs are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least 75%. Any violation(s) documented may impact the compliance history score. Today, the following items were monitored: • Supervision • Staff / Child Ratio • CPR / First Aid • Special Training • CBC Qualification • Emergency Medical Care Plan • Administration of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • Discipline • Adequate / Approved Space • Program Records • License Posted • Permit Restrictions The license was observed, and the restrictions were observed in compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. The outdoor play area is not monitored by DCDEE. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children eating snack, engaged in an art activity and doing homework. No new staff were reported. There are currently no children on medication. Storage hazardous items was monitored today. One bottle of lens cleaner was five feet up in a cabinet but needs to be locked. This was locked during the visit today. A playground inspection was recorded 11/9/2023. A fire drill was recorded 9/30/24. A shelter-in-place drill for the facility was recorded 6/20/24. A fire inspection was conducted 9/16/2024. I observed a sanitation inspection was last conducted on 7/24/2024 earning a superior classification. The program does not currently provide transportation. The following violations were3 cited during today’s visit: Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. One bottle of lens cleaner was five feet up but was not locked. .2820(b) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff member had expired first aid training in September 2024. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff member had expired CPR training in September 2024. .1102(d) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The storage closet with hazardous cleaners was unlocked. .2820(b) 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. Playground inspections were not completed for 2024. .0605(q) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. Emergency drills were not recorded every three months. .0604(u);.0302(d)(8) 1824 The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. The EPR plan was not updated in the portal annually. .0607(e) 1921 Staff working in part-time, full-day or track out school age care programs required to complete BSAC training, did not complete it within three months of employment. One staff member did not complete BSAC within three months of employment. .2510(j) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Alissa Long, on-site program coordinator assisted me with the visit. Currently the program operates with a FIVE (5) Star License re-issued 10/14/22. License restrictions include daytime care only; school age children only; meets enhanced ratios and space, max group size 24; playground area does not meet child care playground safety guidelines; allowed to provide care for child up to 14 yrs of age. The program is operated by Yadkin County Schools. The program’s compliance history was 89% prior to today’s visit. Licensed programs are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least 75%. Any violation(s) documented may impact the compliance history score. Today, the following items were monitored: • Supervision • Staff / Child Ratio • CPR / First Aid • Special Training • CBC Qualification • ITS – SIDS • Emergency Medical Care Plan • Administration of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • Discipline • Adequate / Approved Space • Program Records • License Posted • Permit Restrictions The license was observed, and the restrictions were observed in compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. The outdoor play area is not monitored by DCDEE. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children engaged in a special snack activityand in free play, I also observed staff reading with children. Two new staff has been assigned to this program. Files for new staff were reviewed. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, Special Training including Health and Safety Training and Criminal Background Checks. There are currently no children on medication. Storage hazardous items was monitored today. A playground inspection was recorded 11/9/2023. A fire drill was recorded 11/27/23. A shelter-in-place drill for the facility was recorded 8/2/23. Ms. Long has planned her November emergency drill for tomorrow. A fire inspection has been conducted this school year. Ms. Long will email me the copy of the report. I observed a sanitation inspection was last conducted on 5/1/202 3earning a superior classification. The program does not currently provide transportation.
