This visit was conducted by Jennifer Davis, childcare consultant with Melissa Morales, assistant teacher, and Mavis Moore, lead teacher. Your program currently operates with a Five Star license. Restrictions include daytime care, meets enhanced space and enhanced ratios minus one. The license was observed, and the restrictions were found in compliance. The NC Secretary of State website was reviewed on 4/7/2026 and Yadkin Valley Economic Development District, Inc. was listed as current and active. The program’s compliance history was 94% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive staff/child interactions today. I observed children eating breakfast, engaged in group time, and routine care. I observed materials and activities that were available to the children in the classrooms. The last annual compliance visit was conducted on 9/4/2025. A sanitation inspection was completed 9/17/2025 with a Superior classification. The last fire inspection was conducted 1/12/2026. Program records and required postings were monitored. A fire drill was conducted on 3/26/2026. A lockdown drill was documented on 2/27/2026. An outdoor inspection was documented on 4/6/2026. Staff files and children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Medications were monitored during the visit today. The program does provide routine transportation. The mini-bus is not located on-site during the day. The following violations were cited during the visit today. Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. The surfacing near the tan slide had a larger hole and two smaller cracks that need repair. There was a deflated ball on the playground. This was removed and corrected during the visit. G.S. 110-91(6); .0601(b) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before April 29, 2026. Teacher reported examples of family engagement include: weekly calendar, monthly newsletter, quarterly head start newsletter, parent conferences, take home projects/folders, home visits (on-site) and family nights. Facility Requirements: The center currently holds a 5 Star License issued 01/11/19. An ECERS-R assessment was conducted in one of the NCPK classes on 12/4/18 and the score was 5.86. Child Health Assessment: I observed health screenings in children’s files that are enrolled in NCPK today. One enrolled child needed a dental, vision, and hearing screening. The screening agency and/or program representative reviewed all health assessment results and shared the results with families, as required by Rule .3005(b). The results are shared via verbal/written communication to the parents. Developmental Screening: All children’s files reviewed today contained developmental screenings (Dial 4 Assessment) administered within 90 days of the first day of attendance in the program or 6 months prior or IEP’s. The teachers stated that all developmental screening results were shared with families by the child’s teacher as required (Rule .3006(b), via verbal/written communication with parents. Curricula: The teacher stated Teaching Strategies Gold and NC Foundations are used in the NCPK program. Formative Assessments: Children’s files observed today contained formative assessments regarding each child's growth and skill development. . The teacher stated that assessment results are shared with the parent during conferences during the year. I observed a winter checkpoint assessment was completed for each record observed. I observed documentation in the files that assessment results were shared with parents during conferences held in the fall. A final assessment will be conducted before the end of May and results shared during a parent conference in this spring. Staff-to-Child Ratio and Class Size: The class size is 18 children or less and the staff/child ratio is 1:9 or less for both classrooms. A lead teacher and teacher are assigned in each classroom. Instructional Staff Standards (Rule .3015): Instructional staff work in direct contact with children in the Pre-K program for at least a 32.5-hour work week. In addition to these direct, day-to-day instructional experiences, the Pre-K program provides adequate additional time for the instructional staff for related instructional activities, including time for planning, scheduling and conducting home visits, meeting with children's families, or attending required professional development activities. Be reminded these related activities shall take place outside of the six and a half hour day of direct teacher-child contact. Planning time occurs after the instructional day or during the day when opportunities allow. Professional Development Requirements (Rule .3016) Professional Development opportunities such as training are logged on a training log and kept in each staff’s file. Prior to the visit, the center's compliance history was 94%. All programs are required to maintain 75% compliance. The license was observed, and the restrictions were found in compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children in center free play, eating breakfast and gross motor play. No new staff were reported at this program. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, Special Training including Health and Safety Training and Criminal Background Checks. Storage and administration of medication were monitored. Medication authorization was monitored for two emergency medications. Storage hazardous items was monitored today. A playground inspection was recorded 3/3/2025. A fire drill was conducted 3/24/2025. A shelter in place drill for the facility was recorded 3/4/2025. The most recent fire inspection was on 1/21/2025. I observed a sanitation inspection was last conducted on 9/11/2025 earning a Superior classification. The program does provide transportation. The program is providing transportation. Emergency and identifying information was monitored. The vehicles used to transport were not on site during the visit. I observed the daily transportation record today. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, Special Training including Health and Safety Training and Criminal Background Checks. Storage and administration of medication were monitored. Medication authorization was monitored for emergency medications and one prescription medication. Storage hazardous items was monitored today. A playground inspection was recorded 3/1/2024. A fire drill was conducted 2/28/2024. A shelter in place drill for the facility was recorded 1/11/2024. The most recent fire inspection was on 1/18/2024. I observed a sanitation inspection was last conducted on 2/13/2024 earning a Superior classification. The program does provide transportation. The program is providing transportation. Emergency and identifying information was monitored. The vehicles used to transport were not on site during the visit. I observed the daily transportation record today. The following violations were cited during today’s visit: Violation Number Comment Rule 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container.
