This visit was conducted by Jennifer Davis , child care consultant with Beth Price, on-site program coordinator. Currently this program operates with a Five (5) Star License issued 10/18/19. License restrictions include daytime care only; meets enhanced ratios and space, reduced group size by 1 in each group from 7 pt. level; playground area does not meet child care playground safety guidelines. The license was observed, and the restrictions were met. This program is operated by Yadkin County Schools. A walk-through of the facility was completed today. Indoor areas were observed. Today, the following items were monitored: • Supervision • Staff / Child Ratio • CPR / First Aid • Special Training • CBC Qualification • Emergency Medical Care Plan • Administration of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • Discipline • Adequate / Approved Space • Program Records • License Posted • Permit Restrictions Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children having snack, outside play and playing in activity centers. One new staff was reported today. A substitute was present but did not have a file. This was corrected during the visit. No medications were observed, and no medications were reported. Storage hazardous items was monitored today. Program records and required postings were monitored. A playground inspection was recorded 11/1/23. A fire drill was recorded 10/2/23. A shelter in place drill was recorded 8/17/23. An approved fire inspection was completed on 11/2/22. A sanitation inspection was conducted on 4/5/23 earning a superior classification. The program does not provide transportation at this time. The program’s compliance history was 92% prior to today’s visit. All programs are required to maintain 75% compliance. The following violations were cited during the visit. Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. The unlocked workroom had a low shelf with three containers of wipes and five bottles of hand sanitizer with warning labels. .2820(b) 1043 All staff records, except financial records, were not made available for review. One new staff person did not have a staff file on-site for review. G.S. 110-91( 9) The violations were corrected during the visit today. Technical Assistance: -Since the substitutes are working at different sites, the files should be available at each site for review. Information can be sent for new hires to all sites so records are accessible. -The workroom is locked daily. The door was open today for emergency purposes today. The room was locked during the visit today. If there is a need for the room to be open, than all hazardous items will need to be locked in a cabinet. Repeated violations, continued non-compliance or failure to submit the corrective action plan prior to the date given in this visit summary can also lead to administrative action taken by the DCDEE. Additional monitoring visits may be conducted during the year to monitor compliance with applicable child care requirements.
