You, Clarice Evans, Operator, assisted me with the visit at this family childcare home. This program currently operates with a One (1) Star license effective 7/24/07. Restrictions include Daytime care only; Maximum of five (5) preschool children at one time; Fireplace/Woodstove not in use during operating hours; Other: Blow-up Slide cannot be used by children under the age of three (3). The program’s compliance history was 100% prior to today’s visit. All programs are required to maintain at least 75% compliance. Today, the following items were monitored: • Supervision • Capacity • CPR / First Aid • ITS-SIDS • CBC Qualification • General Health • Administering of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • General License Requirements • Program Records • License Posted • Fire Safety • Sanitation The license was observed, and the restrictions were found in compliance. Ms. Clarice Evans stated today the only person living in the home with her husband, Gary Evans. You must have criminal record checks conducted every five (5) years. You may go to the Division of Child Development & Early Education's website at https://ncchildcare.ncdhhs.gov and complete this process six (6) months prior to the expiration date on your qualifying letter. Mr. Gary Evans was at the home at the time of the visit. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Upon arrival, capacity was monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children having free choice play in the play area. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training including Health and Safety Training and Criminal Background Checks. No prescription medications were observed nor reported. Ms. Evans stated that she does not give prescription medication. I observed a diaper cream today with no medication permission form. Storage hazardous items were monitored today. A playground inspection was recorded on 1/2/26 by C. Evans. A fire drill was recorded on 1/2/26 at 8:30 am with one (1) minute, forty-five (45) seconds to evacuate by C. Evans. A shelter -in- place emergency drill for the facility was recorded on 1/2/26 by C. Evans. There are no pets reported inside the home. The program does not provide transportation. Ms. Evans stated that no one in the home smokes. The following violations were cited during today’s visit: Violation Number Comment Rule 729 Appliances with heating elements, such as bottle warmers, crock pots, curling irons, and electrical cords were accessible to children. On the day of the visit, a space heater was in operation and in the play area accessible to children. 10A NCAC .1719(a)(28) 903 When medication is administered, authorization for the operator to administer the specific medication according to the parent's or health care professional's instructions was not on file and available. I observed an Aquaphor ointment for a toddler with no permission to administer form completed and/or on file by the parent. 10A NCAC 09 .1720(c)(1&2) & .1721(a)(4) 1986 Breast milk, formula, and other bottled beverages sent from home were not fully prepared, dated, and labeled with individual child's names. I observed breast milk in the refrigerator for the toddler with no name or date labeled on the bottles. .1706(l) You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before Wednesday, February 4, 2026. This visit was conducted by Karen Young, childcare consultant with Clarice Evans, owner/operator. Your program currently operates with a ONE (1) Star license. Restrictions include 1st shift daytime care only; maximum of five (5) preschool children; fireplace/woodstove not to be used during hours of operation; Blow up slide cannot be used by children under three (3) years of age. The license was observed, and the restrictions were not found in compliance. The program’s compliance history was 86% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the home was completed today. Indoor and outdoor areas were observed. There were no violations cited on the outdoor play area. Upon arrival, capacity was monitored as indicated on the attached worksheet. Supervision was adequate during the visit. Ms. Evans was over her capacity today. There were eleven (11) children present upon my arrival. Ther were six (6) preschool children and five (5) school age children. Ms. Evan stated that Yadkin County schools have a teacher work day today. Ms. Evans began to text three parents to come pick up their children. Two sets of siblings and one other school age child went home during my visit today. This left four (4) preschool children and two (2) school age children at the family child care home. I observed positive interactions today. I observed children having free play center time, coloring, and building with magnetic tiles. I observed developmentally appropriate materials and activities were available to the children in the licensed space. The last annual compliance visit was conducted on 11/17/22. A Verification of Required Information for Operator and Additional Caregivers form and the FCCH and Centers Located in a Residence Operator’s Statement of Responsibility form was scanned for the operator to complete, sign and scan back to me. We discussed the owner status of this facility. Ms. Evan is the sole proprietor of this family child care home. Ms. Evans stated the only people living in the home with her are her husband. Remember that you and your husband must have criminal record checks conducted every five (5) years. You may go to the Division of Child Development & Early Education's website at https://ncchildcare.ncdhhs.gov and complete this process six (6) months prior to the expiration date on your qualifying letter. Program records and required postings were monitored. A fire drill was conducted on 10/23/23. A shelter in place drill was documented on 10/23/23. An outdoor inspection was documented on 10/7/23. There was no First Aid poster displayed in the family child care home today. The Emergency Preparedness and Response Plan (EPR) was viewed today, but it was dated 2016 and stated DRAFT on the copy. There are no pets reported in the home. Staff files and children’s records were monitored per DCDEE procedures. I monitored two (2) children’s files and Ms. Evans’ staff file today. Storage of hazardous items was monitored today. No medications were observed nor reported. The program does not provide transportation. The following violations were cited during today’s visit: Violation Number Comment Rule 101 Number of children exceeded number allowed. I observed eleven (11) children at the family child care home upon arrival today. Per the permit, Ms. Evans cannot exceed eigth (8) children. GS 110-91(7)(b) & 110-86(3)(b) 102 Number of preschool children exceeded five children, including the provider's own preschool children. I observed six (6) preschool children in attendance during the visit today. GS 110-91(7)(b) 1603 The home was not in compliance with permit restrictions. I observed Ms. Evans caring for eleven (11) children upon arrival today. During the visit five (5) children left with their parents including two (2) sets of siblings and one other school age child. This left six (6) children in the home including four (4) preschool children and two (2) school age children. GS 110-91 1876 The operator did not review the EPR Plan annually or when information in the plan changed to ensure all information is current. The last date on the EPR plan was 2016. The EPR plan viewed today also stated it was a DRAFT. .1714(e ) 1919 A First Aid Information sheet, complete with required information, was not posted for quick referral by staff. I observed that there was no First Aid poster displayed in this FCCH today. .1719(a )(14) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before Friday, November 10, 2023. The following information must be included in your signed and dated compliance letter: • the name of your center • facility ID number • date • title of the person who signs the letter • each violation number(s) • describes accurately and in detail how and when you corrected each violation You may email the letter to karen.young@dhhs.nc.gov, or mail two copies of the letter to: Karen Young P.O. Additional monitoring visits may be conducted during the year to monitor compliance with applicable childcare requirements. Technical assistance provided during today’s visit: • A First Aid poster will be scanned to you for you to post in your FCCH. • The EPR Plan must be reviewed annually and available for review by DCDEE representatives on the visit. You must go into the portal to review and update. If you are locked out of the portal, email me for assistance. • The capacity on your permit must be followed and allows for a maximum of five (5) preschool children. • The capacity on your permit must be followed and allows for a maximum of eight (8) children, five (5) preschool children and three (3) school age children for a total of eight (8) children. Additional Information: You must notify your consultant 30 days prior to changing the ownership status of your facility. You are not in a Cohort since you are a one-star licensed center. I recommend that you get a large notebook with tabs to file your paperwork and to help keep it organized. At the completion of the visit, this visit summary was reviewed with and provided to you.
