This visit was conducted with Roxanna Steelman, owner/operator. Your program currently operates with a THREE (3) Star license effective 11/8/19. Restrictions include 1st shift daytime care only; maximum of five (5) preschool children at any time.; no more than two (2) infants under one (1) year; Donnie Steelman and Amanda Steelman shall not be on the premises while children are in care. The license was observed, and the restrictions were found in compliance. Ms. Steelman is the sole proprietor of this family childcare home. The program’s compliance history was 100% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the home was completed today. Indoor and outdoor areas were observed. Upon arrival, capacity was monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children having free choice center play in the designated play area, having lunch, and preparing for nap/rest time. I observed developmentally appropriate materials and activities were available to the children in the licensed space. The last annual compliance visit was conducted on 7/18/24. A Verification of Required Information for Operator and Additional Caregivers form was sent to the owner prior to the visit. It is being printed for the operator by the Yadkin County partnership for Ms. Steelman to complete and scan back to me. A FCCH and Centers Located in a Residence Operator’s Statement of Responsibility form was sent to the owner prior to the visit and is being printed by the Yadkin County partnership for Ms. Steelman to complete and scan to me. We discussed that you are the sole proprietor of this family childcare home. Ms. Steelman stated the only people living in the home with her are her granddaughter. Be reminded that you and your granddaughter must have criminal record checks conducted every five (5) years. You may go to the Division of Child Development & Early Education's website at https://ncchildcare.ncdhhs.gov and complete this process six (6) months prior to the expiration date on your qualifying letter. Program records and required postings were monitored. A fire drill was conducted on 6/4/25 at 4:02 pm with forty-eight (48) seconds to evacuate by R. Steelman. A lockdown emergency drill was documented on 6/4/25 at 3:46 pm with twenty-nine (29) seconds to lockdown by R. Steelman. The last outdoor inspection was documented on 2/4/25 at 3:54 pm by R. Steelman. There are two (2) pets inside the home and all vaccinations and parent acknowledgement forms were monitored. Staff files and children’s records were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Storage and administration of medication were monitored. Medication authorization was monitored. The program does not provide transportation. The following violations were cited during today’s visit: Violation Number Comment Rule 923 Operator did not keep records of monthly fire drills, quarterly lockdown or shelter-in-place drills, incident reports, incident logs, accurate attendance, emergency numbers, emergency preparedness and response plan, field trip/transportation permission, pet vaccinations records and/or playground inspections monthly outdoor safety checks for a minimum of one year. Ms. Steelman did not have record of playground inspections for the months of March, April, May or June 2025. The last playground inspection was documented in February 2025. .1721 (e)(7) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before Tuesday, July 22, 2025. All new information regarding the new QRIS will be coming out in the Raise NC newsletters going forward. • Make sure you are registered on the ABCMS portal. If you have questions, you may contact the Criminal Background Check Unit at (919) 814-6401 and someone will assist you. • The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new childcare providers working who were hired or moved into the childcare facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted.
