This visit was conducted by Jennifer Davis, child care consultant, with Ashley Wilbanks, lead teacher. Currently this program operates with FIVE (5) star license issued 4/30/19. Restrictions include daytime care only; meets enhance space; meets enhanced ratios; meets enhanced ratios minus one; Certified Developmental Day. The license was observed, and the restrictions were met. This program is operated by Yadkin County Schools. There is one classroom that is designated NCPK. Currently, classrooms are not designated Developmental Day. The license was posted, and restrictions were met. The program’s compliance history was 97% prior to today’s visit. All programs are required to maintain at least 75% compliance. The last annual compliance visit was conducted on 12/12/2024. A sanitation inspection was completed on 10/30/2025 with a Superior classification. A fire inspection was conducted on 3/6/2025. All indoor and outdoor areas were observed and monitored during the visit. Upon arrival staff/child ratio was observed and recorded on the attached worksheet. Supervision was adequate during the visit. I observed developmentally appropriate materials and activities were available to the children in the classroom. I observed the children having free play in activity areas and in gross motor. I observed positive interactions between staff and children. Storage of hazardous items was monitored. I observed medications during the visit. The program does provide transportation for limited children at this time. DPI offsite verification forms for transportation (7/30/25) was observed. Program records and required postings were monitored. A fire drill was documented on 11/24/2025. A lockdown drill was documented on 9/29/2025 . A playground inspection was documented on 10/30/2025 . Staff records were monitored per DCDEE procedure. Children’s records were monitored per DCDEE procedures. DPI offsite verification forms for staff records and children’s records were observed. NCPK Requirements Staff Education Administrator: Christopher Petree - MA/MS Education Leadership w/ PL, Lev 3 adm. Lead Teacher: Ashley Wilbanks – Lead Teacher, BA/BS ECE w/ BK License; AAS ECE Asst. Teacher: Sarah Tweed – Long term sub; BA/BS Social Work-currently enrolled in ECE courses. The program's instructional day is 7:30 a.m. to 2:30 p.m. Attendance is taken daily and if a child is absent for three consecutive days stated that the teacher will contact the parent. Teacher reported examples of family engagement include monthly newsletters/calendars, open house and parent conferences (in person or phone), DOJO, family projects and daily take home folders. Facility Requirements: If the operator fails to correct any documented violations within the established time period The center currently holds a 5 Star License issued 4/30/19. An ECERS-R assessment was conducted in the NCPK class on 4/4/19 and the score was 5.53. You must maintain all requirements at all times. Child Health Assessment: Records of children enrolled in the NCPK classroom were reviewed today. For any on-site screenings held at the program, the screening agency and/or program representative reviewed all health assessment results and shared the results with families, as required by Rule .3005(b). The results are shared via verbal/written communication to the parents. Developmental Screening: Children were confirmed to have had a developmental screening administered within 90 days of the first day of attendance in the program or 6 months prior. The teachers stated that developmental screening results were shared with families by the child’s teacher as required (Rule .3006(b), via verbal/written communication with parents. Curricula: The teacher stated Creative Curriculum and NC Foundations are used in the NCPK program. During the observation, children were engaged in free play in the activity centers, routine care and gross motor play. Throughout the classroom, items were in activity areas were consistent with the lesson plan. Formative Assessments: The teacher stated that the formative assessment the center is using is Teaching Strategies Gold. I observed entries in Teaching Strategies Gold. A checkpoint is currently being finished up. The teacher stated that assessment results were shared with the parents, and conferences offered. A Wilbanks reported that an additional assessment will be completed by 2/2025 and parent conferences will be offered. Staff-to-Child Ratio and Class Size: The class size is 18 children or less and the staff/child ratio is 1:9 or less for the classroom. A lead teacher and teacher are assigned to the classroom. Instructional Staff Standards (Rule .3015): Instructional staff work in direct contact with children in the Pre-K program for at least a 32.5-hour work week. In addition to these direct, day-to-day instructional experiences, the Pre-K program provides adequate additional time for the instructional staff for related instructional activities, including time for planning, scheduling and conducting home visits, meeting with children's families, or attending required professional development activities. Be reminded these related activities shall take place outside of the six-and-a-half-hour day of direct teacher-child contact. Planning time occurs after the instructional day or during the day when opportunities allow. A Wilbanks stated that an open house was conducted on 8/11/2025. Professional Development Requirements (Rule .3016) Professional Development opportunities such as training are logged on a training log and kept in each staff’s file. Staff training was monitored today. The following violations were cited during the visit: Violation Number Comment Rule 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. One expired over-the-counter medication was not returned to the parent or discarded. .0803(12) 1769 The health assessment did not include a dental screening. One enrolled child did not have a dental screening on file. .3005 (a)(5) 1834 Application did not have a medical action plan attached for any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services. Two enrolled children did not have a medical action plan on file. .0801(b) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Additional Information: Begun in February 2023, the Child Care Commission’s work to modernize North Carolina’s QRIS is now complete. Pathways to the Stars, Section .3200, Standards for Two through Five Star Rated Licenses became effective July 1, 2025. Effective February 1, 2025, ECERS-3, ITERS-3, and FCCERS-3 — also known as the "Threes" will be used for program assessments and reassessments. We discussed the pathway options during the visit today. The county has a planned training in January to review ECERS-3 information. • Additional climbing equipment is being added to the playground by donation from Lowes. Additional Information/Consultation: • You must notify your consultant 30 days prior to changing the ownership status of your facility. • As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. Restrictions include daytime care only; meets enhance space; meets enhanced ratios; meets enhanced ratios minus one; Certified Developmental Day. The license was observed, and the restrictions were met. This program is operated by Yadkin County Schools. There is one classroom that is designated NCPK. Currently, classrooms are not designated Developmental Day. The license was posted, and restrictions were met. The program’s compliance history was 86% prior to today’s visit. All programs are required to maintain at least 75% compliance. The last annual compliance visit was conducted on 1/11/2024. A sanitation inspection was completed on 9/11/2024 with a Superior classification. A fire inspection was conducted on 2/19/2024. All indoor and outdoor areas were observed and monitored during the visit. Upon arrival staff/child ratio was observed and recorded on the attached worksheet. Supervision was adequate during the visit. I observed developmentally appropriate materials and activities were available to the children in the classroom. I observed the children having free play in activity areas and in gross motor. I observed positive interactions between staff and children. Storage of hazardous items was monitored. No medications were observed or reported. The program does provide transportation for limited children at this time. DPI offsite verification forms for transportation (8/13/24) was observed. Program records and required postings were monitored. A fire drill was documented on 11/19/2024. A lockdown drill was documented on 9/24/2024 . A playground inspection was documented on . The program's instructional day is 7:30 a.m. to 2:30 p.m. Attendance is taken daily and if a child is absent for three consecutive days stated that the teacher will contact the parent. Teacher reported examples of family engagement include monthly newsletters/calendars, open house and parent conferences (in person or phone), DOJO, family projects and daily take home folders. Facility Requirements: The center currently holds a 5 Star License issued 4/30/19. An ECERS-R assessment was conducted in the NCPK class on 4/4/19 and the score was 5.53. You must maintain all requirements at all times. Child Health Assessment: Records of children enrolled in the NCPK classroom were reviewed today. For any on-site screenings held at the program, the screening agency and/or program representative reviewed all health assessment results and shared the results with families, as required by Rule .3005(b). The results are shared via verbal/written communication to the parents. Developmental Screening: Children were confirmed to have had a developmental screening administered within 90 days of the first day of attendance in the program or 6 months prior. There were eight(8) children that need a follow-up vision screening and eight(8) children that need a follow-up hearing screening. The teachers stated that developmental screening results were shared with families by the child’s teacher as required (Rule .3006(b), via verbal/written communication with parents. Curricula: The teacher stated Creative Curriculum and NC Foundations are used in the NCPK program. During the observation, children were engaged in free play in the activity centers throughout the room and a group activity. Throughout the classroom, items were in activity areas were consistent with the lesson plan. Formative Assessments: The teacher stated that the formative assessment the center is using is Teaching Strategies Gold. I observed entries in Teaching Strategies Gold. A checkpoint was conducted in Fall 2024. The teacher stated that assessment results were shared with the parents, and conferences offered. A Wilbanks reported that an additional assessment will be completed by 6/2025 and parent conferences will be offered. Staff-to-Child Ratio and Class Size: The class size is 18 children or less and the staff/child ratio is 1:9 or less for the classroom. A lead teacher and teacher are assigned to the classroom. Instructional Staff Standards (Rule .3015): Instructional staff work in direct contact with children in the Pre-K program for at least a 32.5-hour work week. In addition to these direct, day-to-day instructional experiences, the Pre-K program provides adequate additional time for the instructional staff for related instructional activities, including time for planning, scheduling and conducting home visits, meeting with children's families, or attending required professional development activities. Be reminded these related activities shall take place outside of the six-and-a-half-hour day of direct teacher-child contact. Planning time occurs after the instructional day or during the day when opportunities allow. A Wilbanks stated that an open house was conducted on 8/12/2024 . I observed a signed attendance roster, confirming parent participation was on file. Professional Development Requirements (Rule .3016) Professional Development opportunities such as training are logged on a training log and kept in each staff’s file.
