The previous temporary time visit was completed October 28, 2025, and prior to today’s visit, the eighteen-month compliance history score was 100%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a temporary license effective September 16, 2025 to March 16, 2026. The information on record with DCDEE was verified as correct by Shakera Graham, and the owner’s corporation status was verified on the NC Secretary of State website as current-active. Angela Stevens, Child Care Consultant, accompanied me during today’s visit. Current Inspection is as follows: Sanitation Classification of Superior with zero (0) demerits completed August 26, 2025. S. Graham, Operator/Teacher, was present and assisted us during the visit. A. Stevens and I washed our hands and completed a walk-through of the indoor and outdoor areas. Three children, aged two to five years old, were present. The children participated in free play activities indoors, personal care routines and hand washing, and ate lunch. During the visit, the children played dress-up in the dramatic play area. They wore princess dresses and costume jewelry. Additionally, they painted and glued shapes and numbers on construction paper. S. Graham provided direct supervision while the children participated in art activities, asked open-ended questions to encourage conversation, and assisted them upon request. Drinking water was accessible to the children in the active play area. After free play activities, S. Graham and the children transitioned to the gross motor room for music, dancing, and exercises. Lunch consisted of chicken nuggets, broccoli florets, pineapple tidbits, whole wheat roll, and milk. The operator and children’s files and program records were monitored today. The most current fire drill was conducted on November 25, 2025, at 10:32 a.m. The most current outdoor inspection checklist for Family Child Care Homes was completed on November 12, 2025, at 9:25 a.m. At the completion of the visit, the Visit Summary was reviewed with S. Graham. The following violations were observed: Violation Number Comment Rule 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. A child, identified on the enrollment record, with an enrollment date of November 5, 2025, did not have a health assessment on file within thirty (30) days of enrollment. GS 110-91(1); 10A NCAC 09.1721(a)(1) 1924 Balloons were accessible to children. Three (3) mylar balloons were hanging on the wall in the child care room accessible to children. This was corrected when the operator removed the balloons from the approved space and made them inaccessible to the children. .1719(a)(19) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4)(d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The violations observed and documented today may impact the compliance history score. The violations observed and documented today must be corrected immediately. Additionally, by 5:00 p.m. on January 1, 2026, you must submit a written, dated, and signed statement to me. The statement must describe accurately and in detail, how and when the violations were corrected. If the letter states that corrections have been made when they have not, this may be considered falsification of information. Technical Assistance/Consultation/Reminders Specific to Today’s Visit: Safe Indoor/Outdoor Environment: Balloons: During today’s visit, there were three mylar balloons hanging on the birthday wall above the children’s cubbies inside the child care room. As a reminder, balloons are prohibited for children of all ages. S. Graham stated that a parent provided the balloons for a birthday celebration and she kept them in the child care for decoration. We reviewed child care requirement 09.1719 and S. Graham removed the balloons from the approved space and made them inaccessible to children in care. Standing water: While monitoring the outdoor environment, we observed standing water on the sand table, inside the plastic white container with animals, and art easel. S. Graham stated it rained overnight and the children have not gone outdoors today. S. Graham explained that the outdoor area is monitored for safety hazards prior to children playing in the outdoor environment. As a reminder, drowning is the leading cause of injury death in children 1-4. Young children can drown in as little as an inch or two of water, and it can happen quickly and silently. Additionally, standing water is a breeding ground for mosquitoes and other insects. As you complete your daily check of the outdoor environment, ensure you dump equipment and materials that hold recent rainfall prior to the children going outdoors. Children’s Files: A child, identified on the enrollment record, with an enrollment date of November 5, 2025, did not have a health assessment on file within thirty (30) days of enrollment. S. Graham stated the parents provided a copy of the child’s current immunization record, but a health assessment was not available upon request. Two (2) children, identified on the enrollment record, with an enrollment date of December 15, 2025, shall have a health assessment on file on or before January 15, 2026. -Children are required to have a health assessment completed by a health care professional and a copy of their immunization record on file no later than 30 days from their first day of attendance. Immunization records are required for all children; however, a health assessment is required for all children from birth to five, that have not started Kindergarten. A health assessment can be accepted if completed within the last 12 months prior to enrollment. -Ensure parents update contact information as it changes, and at least annually. Employment Outside the FCCH: S. Graham informed me that she works outside the FCCH providing training and consultation to early childhood professionals when the licensed program is closed and/or after operating hours. The following definitions from Chapter 9 were discussed with S. Graham: "Family Child Care Home" means a child care arrangement located in a residence where, at any one time, more than two children, but less than nine children, receive child care. Family child care home operators must reside at the location of the family child care home. "Reside" refers to any person that lives at a child care facility location. Factors for determining residence include: a. use of the child care facility address as a permanent address for personal identification or mail delivery; b. use of the child care facility to store personal belongings such as furniture, clothing, and toiletry items; and c. names listed on official documents such as criminal records or property tax records. "Operator" means the owner, director, or other person having responsibility for operation of a child care facility subject to licensing. I explained to S. Graham that she may work outside the FCCH as long as when children are enrolled and in care, she is the operator, and the children receive care at the address where the license is issued. A qualified additional caregiver or substitute provider who meets the requirements in child care rule 09.1729 may provide care for enrolled children for you to attend classes, medical appointments, and/or to complete routine tasks in excess of two hours per week. Rated License Assessment: During the TTP visit completed October 28, 2025, an overview of section .3200 of the child care rules were provided. Each of the following pathway options were discussed: Program Assessment Pathway, Classroom and Instructional Quality Pathway, and Accreditation and Head Start Pathway. Education requirements were also reviewed. As discussed on October 28, 2025, the FCCH must transition to a 3-5 Star Rated License to continue to be eligible to receive subsidized child care assistance after six (6) months. Based on today’s conversation, S. Graham stated she plans to achieve compliance with Classroom and Instructional Quality: Pathway 2 for a five-star license, and that Kathleen Hoffler, Region 5 Family Child Care Consultant, is her Coach/Mentor. A TA application was completed and submitted to the Partnership for Children of Cumberland County for technical assistance and consultation regarding the Family and Community Engagement Standards, CQI, and Professional Development Plan. Currently, S. Graham is using an approved curriculum (The Creative Curriculum for Family Child Care, 2nd edition Vols 1 & 2) along with the formulative assessment: Desired Results Developmental Profile. Additionally, S. Graham is LT qualified with a master’s degree in early childhood education and over twenty-five (25) years’ ECE experience. As a reminder, a child care facility is eligible for a rated license of two through five-stars upon application and assessment of compliance with the requirements. Graham has four (4) months from the date of licensure (January 16, 2026) to go online at https://rmp.nc.gov/portal/portal.aspx and complete the EPR Plan for this location. As a reminder, you shall review the EPR Plan annually or when information in the plan changes to ensure all information is current. Additional caregivers and substitutes shall review the EPR Plan during orientation and/or on an annual basis with S. Graham. Documentation of the review shall be maintained on file. Graham: "Family Child Care Home" means a child care arrangement located in a residence where, at any one time, more than two children, but less than nine children, receive child care. Family child care home operators must reside at the location of the family child care home. "Reside" refers to any person that lives at a child care facility location. Factors for determining residence include: a. use of the child care facility address as a permanent address for personal identification or mail delivery; b. use of the child care facility to store personal belongings such as furniture, clothing, and toiletry items; and c. names listed on official documents such as criminal records or property tax records. "Operator" means the owner, director, or other person having responsibility for operation of a child care facility subject to licensing. I explained to S. Graham that she may work outside the FCCH as long as when children are enrolled and in care, she is the operator, and the children receive care at the address where the license is issued. A qualified additional caregiver or substitute provider who meets the requirements in child care rule 09.1729 may provide care for enrolled children for you to attend classes, medical appointments, and/or to complete routine tasks in excess of two hours per week. Rated License Assessment: During the TTP visit completed October 28, 2025, an overview of section .3200 of the child care rules were provided. Each of the following pathway options were discussed: Program Assessment Pathway, Classroom and Instructional Quality Pathway, and Accreditation and Head Start Pathway. Education requirements were also reviewed. As discussed on October 28, 2025, the FCCH must transition to a 3-5 Star Rated License to continue to be eligible to receive subsidized child care assistance after six (6) months. Based on today’s conversation, S. Graham stated she plans to achieve compliance with Classroom and Instructional Quality: Pathway 2 for a five-star license, and that Kathleen Hoffler, Region 5 Family Child Care Consultant, is her Coach/Mentor. A TA application was completed and submitted to the Partnership for Children of Cumberland County for technical assistance and consultation regarding the Family and Community Engagement Standards, CQI, and Professional Development Plan. Currently, S. Graham is using an approved curriculum (The Creative Curriculum for Family Child Care, 2nd edition Vols 1 & 2) along with the formulative assessment: Desired Results Developmental Profile. Additionally, S. Graham is LT qualified with a master’s degree in early childhood education and over twenty-five (25) years’ ECE experience. As a reminder, a child care facility is eligible for a rated license of two through five-stars upon application and assessment of compliance with the requirements. S. Graham stated that she is working on the Facility’s Continuous Quality Improvement Plan and her Professional Development Plan to review established goals and to document progress. Training: S. Graham has four (4) months from the date of licensure (January 16, 2026) to go online at https://rmp.nc.gov/portal/portal.aspx and complete the EPR Plan for this location. As a reminder, you shall review the EPR Plan annually or when information in the plan changes to ensure all information is current. Additional caregivers and substitutes shall review the EPR Plan during orientation and/or on an annual basis with S. Graham. Documentation of the review shall be maintained on file.
