    Name of Operation: BEREAN BAPTIST ACADEMY PRESCHOOL Facility ID: 26001238 Consultant: NANCY STRATFORD Operation Type: Center Case Number: Visit Date: 6/11/2025 Number Present: 22 Completed Date: 6/11/2025 Age: From 3 To 5 Total Minutes: 165 Time In: 09:00 AM Time Out: 11:45 AM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today's visit was to monitor compliance with applicable child care requirements pertinent to a routine unannounced visit. The previous Annual Compliance Visit was completed November 14, 2024, and prior to today’s visit, the eighteen-month compliance history score was at 99% which is above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a Notice of Compliance issued August 11, 2008. The information on record with DCDEE was verified as correct by Sandra Adams, and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of Superior completed September 4, 2024. Fire Classification of Approved completed January 28, 2025. A violation was observed for the fire inspection. Ms. Adams was present and accompanied me while observing the indoor areas. In Space #1, the children were playing in a variety of activities, relating to Space. The classroom was set up to give the children a visual for the different planets and items in Space. The children played in the spaceship, walked around and explored their space, and played in the sensory area. The children were able to find items that belong in Space. After playing, the children cleaned up their things, and the children ate snack. Snack served was fruit loops and milk. Lunch served was cheeseburger, potatoes, mixed fruit, bread, and milk. The last fire drill was completed June 5, 2025. The last shelter in place was completed March 12, 2025. The last monthly playground inspection was completed June 5, 2025. At the completion of the visit, the Visit Summary was reviewed with Ms. Adams. The following violation was observed: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection was completed January 16, 2024, and the current fire inspection was completed on January 28, 2025. This was corrected when the operator scheduled and completed a fire inspection. 10A NCAC 09 .0304(a) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The violation observed and documented today may impact the compliance history score. The violation observed and documented today was corrected. A compliance letter is not required. Facility Specific TA/Consultation/Reminders : Observation/Technical Assistance (TA): Fire Inspection: A violation was observed for the fire inspection. The last fire inspection was dated for January 16, 2024, and the current fire inspection is dated for January 28, 2025. The fire inspection is required to be completed one year from the last inspection date. The administrator stated that she made advance notice for the inspection, and was informed that as long as it was completed within the month on January, the inspection would be in compliance. However, Child Care Rules stated it must be completed one year from the last date. This would need to be completed annually. Please refer to Child Care Rules for additional rule requirements. An observation was observed in the preschool classroom three dimensional objects of the planets. The children were able to select an activity. The activity included the spaceship, sensory table with planets, and blocks. The children were able to explore and expand on their knowledge of the planets. I recommend adding different types of books about the different planets and stars. The children can even pretend to create a star and name a star.
