The previous Annual Compliance Visit was completed October 6, 2025, and prior to today’s visit, the eighteen-month compliance history score was at 96% which is above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a one-Star License issued January 15, 2020. The information on record with the DCDEE was verified as correct by Louvina Bertie. Current Inspections are as follows: Sanitation Classification of Superior completed 5/16/2025 L. Bertie was present while I observed the indoor and outdoor areas. Upon entering the facility, one child arrived and two children present were observed participating in center play such as blocks, and puzzles. L. Bertie assisted children in care by stacking the blocks to create a rocket ship. Breakfast served were a banana, cereal and milk. Lunch documented were rice, green beans, baked chicken, garlic bread, and milk. At the completion of the visit, the Visit Summary was reviewed with L. Bertie. The following violation(s) was observed. The facility uses redirection to assist with keeping the learning environment safe. NCDHHS Responds to Increase in Measles Cases: Measles is on the rise globally and in North Carolina. As a highly contagious, airborne disease, even one case is a major priority for public health. NCDHHS is taking action by launching new outreach resources for child care providers, families, and community stakeholder to help curb the spread. Child care providers can share information about vaccinations with the families served and be informed with the most current information. The previous Annual Compliance Visit was completed November 07, 2022 and prior to today’s visit, the eighteen-month compliance history score was 100 % which is at or above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a One Star License issued January 15, 2020. The information on record with DCDEE was verified as correct. Inspections are as follows: Sanitation inspection was completed, November 10,2022 Louvina. Bertie, Owner/Operator, was present while I observed the indoor and outdoor areas. Upon my arrival, I observed one child inside, participating in handwashing and toileting routines. L. Bertie assisted the child with completing handwashing and toileting. L. Bertie assisted the child with identifying colors, letters, and shapes during morning group time. L. Bertie and the child sang along with a musical CD to identify the letters and sounds. Free choice materials were available. Lunch documented on menu, chicken nuggets, corn on the cob, rice, applesauce and milk. At the completion of the visit, the visit summary was reviewed with L. Bertie. The following violation(s) were observed. Violation Number Comment Rule 1301 Operator did not complete the required number of on-going training hours as specified in rule. Operator did not complete required number of on-going training hours, operator only completed 6hrs out of 12hrs required. GS 110-91(11); 10A NCAC 09 .1705(b)(5) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violations observed and documented today may impact the compliance history score. The violation(s) observed and documented today must be corrected immediately. Additionally, by 5:00pm on November 7, 2023, you must submit a written, dated, and signed statement to me. The statement must describe accurately and in detail, how and when the violations were corrected.
