The previous Annual Compliance Visit was completed March 06, 2025, and prior to today’s visit, the eighteen-month compliance history score was at 100%. This program currently operates with a Three-Star License issued March 25, 2020. The License was based on the program earning four points in education standards, two points in program standards, and one quality point by choosing to meet a programmatic option for: Staff benefit package and Infrastructure of parent involvement. The information on record with DCDEE was verified as correct by Ms. Andria Smith, Staff Member, and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: The facility is exempt from meeting requirements for Sanitation and Fire Inspections. Ms. Smith was present and accompanied me while observing the indoor areas. The outdoor area was not monitored due to the license restriction: “Playground area does not meet child care playground safety standards. Upon arrival, the children washed their hands, completed toileting activities, and put their things in their cubbies. The group leaders completed their attendance for their assigned group. The snack served was corn chips and chilled milk. After eating their snack, the group leaders talked to the children about the next activity scheduled. All materials were open and available for children. A group of children went outdoors to play. The groups alternated so each group has outdoor time. The staff assisted the children with free choice play activities. A violation was observed today for storage of hazardous items. A checklist was used to note the requirements monitored during the visit. A complete Staff and Training worksheet was updated, and all staff files were monitored. The Children’s Record form was completed, and six (6) files were monitored. The last fire drill was completed on January 09, 2026. The shelter in place drill was completed on November 25, 2026. The last monthly playground check was completed on September 29, 2025. At the completion of the visit, the Visit Summary was reviewed with Ms. Smith. The following violation was observed: Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In an unlocked cabinet, greater than 5 feet from the floor, there was one aerosol can of Lysol. This was corrected when the staff member removed the aerosol and placed the aerosol can in a locked cabinet. .2820(b) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The previous Annual Compliance Visit was completed March 12, 2024, and prior to today’s visit, the eighteen-month compliance history score was 100% which at or above 75%, as required by North Carolina General Statute 110-90(4) (c-d). This program currently operates with a Three-Star License issued February 24, 2020. The License was based on the program earning four (4) points in education standards, two (2) points in program standards, and one (1) quality point by choosing education option the child care administrator has at least 10 years of documented child care administration work experience in a licensed program. The information on record with DCDEE was verified as correct by Andria Smith. Current Inspections are as follows: Sanitation Classification of Superior completed September 5, 2024. Fire Classification of Superior completed September 19, 2024. A. Smith, Program Coordinator, was present while I observed the indoor area. The outdoor area was not monitored due to the license restriction: “Playground area does not meet childcare playground safety standards.” Upon arrival, children were divided into groups and attendance was documented by a staff member. One staff was observed serving snack to one group of children while one staff member escorted a group of children to restrooms in the adjacent hallway. Activity centers were set up throughout the space approved for childcare—activity centers (blocks, manipulatives, reading, tabletop games, art, and homework). Staff members assisted children with whole group activities. Children were offered to participate in painting posters with tempera paints and making homemade slime. I observed toileting and hand-washing routines, eating snacks, engaging in free choice activities, and transitioning to outdoors. The snack served today were baked Cheetos and 100% chilled juice. Staff engaged in frequent conversations with individuals and groups of children. Staff were observed greeting parents during departure and sharing important information. A checklist was used to note the requirements I monitored during today’s visit. I received an updated Staff and Training Worksheet and all staff files were monitored. A Children’s Record form was completed for six (6) files monitored during today's visit. I observe one staff member leave the facility for the day during the visit. The most recent fire drill on file was completed October 8, 2024. No documentation was on file of lockdown or shelter-in-place drills completed for the 2024-2025 school year. At the completion of the visit, the Visit Summary was reviewed with Ms. Smith. The following violations were observed: Violation Number Comment Rule 415 A current schedule was not posted for each group of children for reference. A current daily schedule was not posted in the space approved for child care. The violation was corrected when the program coordinator placed the information board with the current daily schedule at the entrance of the facility. GS 110-91(12);.0508(a) 428 A current activity plan was not posted for each group of children for reference. No activity plans were available for reference for all children enrolled. GS 110-91(12); .0508(a) 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. A current menu was not posted where easily seen by parents. The violation was corrected when a staff member placed the current menu on the parent information board. 10A NCAC 09 .0901(b) 805 Fire drills were not practiced monthly and/or the drill record was incomplete. The most recent fire drill on file was conducted on October 8, 2024. .0604(t); .0302(d)(5) 1792 Staff did not model appropriate eating behaviors by consuming food or beverages that meet the nutritional requirements specified in the Meal Patterns for Children in Child Care Programs in the presence of children in care. A bag of McDonald's and a beverage in a McDonald's cup that belonged to a staff member was stored on a table and accessible to children in care. The violation was corrected when the staff removed the items from the child care area. .0901(i) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The shelter-in-place and lockdown drill record on file was not completed with drills that had been conducted. .0604(u);.0302(d)(8) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Additionally, by 5:00pm on March 20, 2025, you must submit a written, dated, and signed statement to me. A Children’s Record form was completed for six (6) files monitored during today's visit. I observe one staff member leave the facility for the day during the visit. The most recent fire drill on file was completed October 8, 2024. No documentation was on file of lockdown or shelter-in-place drills completed for the 2024-2025 school year. At the completion of the visit, the Visit Summary was reviewed with Ms. Smith. The violation was corrected when a staff member placed the current menu on the parent information board. 10A NCAC 09 .0901(b) 805 Fire drills were not practiced monthly and/or the drill record was incomplete. The most recent fire drill on file was conducted on October 8, 2024. .0604(t); .0302(d)(5) 1792 Staff did not model appropriate eating behaviors by consuming food or beverages that meet the nutritional requirements specified in the Meal Patterns for Children in Child Care Programs in the presence of children in care. A bag of McDonald's and a beverage in a McDonald's cup that belonged to a staff member was stored on a table and accessible to children in care. The violation was corrected when the staff removed the items from the child care area. .0901(i) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The shelter-in-place and lockdown drill record on file was not completed with drills that had been conducted. .0604(u);.0302(d)(8) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The previous Annual Compliance Visit was completed March 12, 2024, and prior to today’s visit, the eighteen-month compliance history score was 100% which at or above 75%, as required by North Carolina General Statute 110-90(4) (c-d). This program currently operates with a Three-Star License issued February 24, 2020. The License was based on the program earning four (4) points in education standards, two (2) points in program standards, and one (1) quality point by choosing education option the child care administrator has at least 10 years of documented child care administration work experience in a licensed program. The information on record with DCDEE was verified as correct by Andria Smith. Current Inspections are as follows: Sanitation Classification of Superior completed September 5, 2024. Fire Classification of Superior completed September 19, 2024. A. Smith, Program Coordinator, was present while I observed the indoor area. The outdoor area was not monitored due to the license restriction: “Playground area does not meet childcare playground safety standards.” Upon arrival, children were divided into groups and attendance was documented by a staff member. One staff was observed serving snack to one group of children while one staff member escorted a group of children to restrooms in the adjacent hallway. Activity centers were set up throughout the space approved for childcare—activity centers (blocks, manipulatives, reading, tabletop games, art, and homework). Staff members assisted children with whole group activities. Children were offered to participate in painting posters with tempera paints and making homemade slime. I observed toileting and hand-washing routines, eating snacks, engaging in free choice activities, and transitioning to outdoors. The snack served today were baked Cheetos and 100% chilled juice. Staff engaged in frequent conversations with individuals and groups of children. Staff were observed greeting parents during departure and sharing important information.
