The previous Annual Compliance Visit was May 30, 2024 and prior to today’s visit, the eighteen-month compliance history score was at 96% which is at or above 75%, as required by North Carolina General Statute 110-90(4)(c-d).This program currently operates with a three- Star License issued June 15, 2023. The License was based on the program earning 6 points in education standards, 2 points in program standards, and one quality point by choosing to meet a programmatic option infant capacity restricted to no more than two children under one year of the age. The information on record with DCDEE was verified as correct. Sanitation Classification of Superior completed March 12, 2026. Xiomara Flores, Owner/Operator, was present and accompanied me while observing the indoor and outdoor areas. I observed three (3) preschool children in the child care area of the home. Upon arrival, children were engaged in playing in interest centers. I observed children manipulate connecting child-shaped and bristle blocks, toy vehicles, puppets, framed knobbed puzzles, and shape sorters. The operator reviewed the alphabet, days of the week, and counting in Spanish and English. The operator engaged children in conversations by asking open-ended questions which led to rich conversations and positive interactions. The operator transitioned children to complete personal care and hand washing routines located at the bathroom in the hallway of the home. Lunch was not observed however, the operator stated children would be served spaghetti, with diced tomatoes, strawberries, and milk. A checklist was used to note the requirements monitored during today's visit. The Verification of Required Information for Operator and Additional Caregivers and the Operator’s Statement of Responsibility forms were completed. Two children’s files were monitored. At the completion of the visit, the Visit Summary was reviewed with X. Flores. The previous Annual Compliance Visit was May 13, 2025, and prior to today’s visit, the eighteen-month compliance history score was at 94%, which is at or above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a three- Star License issued June 15, 2023. The License was based on the program earning 6 points in education standards, 2 points in program standards, and one quality point by choosing to meet a programmatic option infant capacity restricted to no more than two children under one year of the age. Current Inspections are as follows: Sanitation Classification of Superior was completed on September 26, 2024. X. Flores, Operator, and one child were present while I observed the indoor and outdoor areas. The child had access to blocks, fine motor, dramatic play, creative art, and literacy materials. The child was observed participating in free choice activities. The operator and child identified colors in English and Spanish while the child played with various colored blocks. The child was also observed solving various knob puzzles and identifying the picture on each puzzle piece. When the child solved the puzzles, the operator said to the child, “Good job!” while giving him a high five. The operator and child also lined up toy cars and counted them in English and Spanish. The outdoor learning environment was monitored for health and safety requirements. Routine care and handwashing practices were observed. Lunch consisted of stewed beef, rice, carrots, potatoes, applesauce, and milk. Program files were reviewed. The most recent fire drill was completed on December 02, 2025. The most recent quarterly (lockdown) drill was completed on October 02, 2025. The most recent outdoor inspection was completed on December 02, 2025. At the completion of the visit, the Visit Summary was reviewed with X. Flores. The following violation was observed: Violation Number Comment Rule 1544 The child care operator did not comply with all State laws, federal laws and/or local ordinances that pertain to child health, safety, and welfare, as required by General Statute 110-91. Sanitation Inspection. Family Child Care Homes shall comply with all State laws and federal laws and local ordinances that pertain to child health, safety, and welfare. The Commission for Public Health shall adopt rules which establish minimum sanitation standards for child care centers and their personnel. The sanitation rules adopted by the Commission for Public Health shall cover such matters as the cleanliness of floors, walls, ceilings, storage spaces, utensils, and other facilities; adequacy of ventilation; sanitation of water supply, lavatory facilities, toilet facilities, sewage disposal, food protection facilities, bactericidal treatment of eating and drinking utensils, and solid-waste storage and disposal; methods of food preparation and serving; infectious disease control; sleeping facilities; and other items and facilities as are necessary in the interest of the public health. The operator stated that she attempted to call the health department to schedule an inspection but received no answer and left a voice message. No one has returned her call. I encouraged the operator to contact the Cumberland County Environmental Health Department at (910)433-3618 to schedule and obtain a sanitation inspection as soon as possible. For more information about sanitation inspection requirements, refer to NC General Statute 110-91 and 15A NCAC 18A .2834. Preschool Age Group Observation I discussed the results from my observation of the caregiving space with X. Flores. I encouraged the operator to continue to connect activities and experiences with previously learned concepts. Children’s thinking skills can be promoted throughout the school day in many classroom activities including centers, whole group instruction, outdoor play time, meal and snack time, as well as during transitions. It is important for opportunities to intentionally think about finding opportunities to do this at different times during the day, and not just during a structured instructional activity. No infants, toddlers, or school age children were enrolled in the program. QRIS Modernization: Pathway Discussion During today’s visit an overview of section .3200 of the child care rules was provided. Each of the following pathway options were discussed: Program Assessment Pathway, Classroom and Instructional Quality Pathway, and Accreditation and Head Start Pathway. Education requirements were also reviewed. Based on today’s conversation, the facility is thinking about pursuing the Classroom and Instructional Quality Pathway. Annual License Fees Invoices for the annual license fees were emailed to facilities subject to license fees on November 3, 2025. Please note that annual license fees do not apply to state or public school-operated child care facilities, or to religious-sponsored facilities operating under a Notice of Compliance. The previous Annual Compliance Visit was May 13, 2025, and prior to today’s visit, the eighteen-month compliance history score was at 94%, which is at or above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a three- Star License issued June 15, 2023. The License was based on the program earning 6 points in education standards, 2 points in program standards, and one quality point by choosing to meet a programmatic option infant capacity restricted to no more than two children under one year of the age. The information on record with DCDEE was verified as correct by Xiomara Flores, Operator. Current Inspections are as follows: Sanitation Classification of Superior was completed on September 26, 2024. X. Flores, Operator, and one child were present while I observed the indoor and outdoor areas. The child had access to blocks, fine motor, dramatic play, creative art, and literacy materials. The child was observed participating in free choice activities. The operator and child identified colors in English and Spanish while the child played with various colored blocks. The child was also observed solving various knob puzzles and identifying the picture on each puzzle piece. When the child solved the puzzles, the operator said to the child, “Good job!” while giving him a high five. The operator and child also lined up toy cars and counted them in English and Spanish. The outdoor learning environment was monitored for health and safety requirements. Routine care and handwashing practices were observed. Lunch consisted of stewed beef, rice, carrots, potatoes, applesauce, and milk. Program files were reviewed. The most recent fire drill was completed on December 02, 2025. The most recent quarterly (lockdown) drill was completed on October 02, 2025. The most recent outdoor inspection was completed on December 02, 2025. Preschool Age Group Observation I discussed the results from my observation of the caregiving space with X. Flores. I encouraged the operator to continue to connect activities and experiences with previously learned concepts. Children’s thinking skills can be promoted throughout the school day in many classroom activities including centers, whole group instruction, outdoor play time, meal and snack time, as well as during transitions. It is important for opportunities to intentionally think about finding opportunities to do this at different times during the day, and not just during a structured instructional activity. No infants, toddlers, or school age children were enrolled in the program. QRIS Modernization: Pathway Discussion During today’s visit an overview of section .3200 of the child care rules was provided. Each of the following pathway options were discussed: Program Assessment Pathway, Classroom and Instructional Quality Pathway, and Accreditation and Head Start Pathway. Education requirements were also reviewed. Based on today’s conversation, the facility is thinking about pursuing the Classroom and Instructional Quality Pathway. The previous Annual Compliance Visit was May 30, 2024 and prior to today’s visit, the eighteen-month compliance history score was at 96% which is at or above 75%, as required by North Carolina General Statute 110-90(4)(c-d).This program currently operates with a three- Star License issued June 15, 2023. The License was based on the program earning 6 points in education standards, 2 points in program standards, and one quality point by choosing to meet a programmatic option infant capacity restricted to no more than two children under one year of the age. The information on record with DCDEE was verified as correct. Sanitation Classification of Superior completed September 26, 2024. Xiomara Flores, Owner/Operator, was present and accompanied me while observing the indoor and outdoor areas. I observed three (3) preschool children in the child care area of the home. Upon arrival, children were engaged in playing in interest centers. I observed children manipulate connecting child-shaped and bristle blocks, toy vehicles, puppets, framed knobbed puzzles, and shape sorters. The operator reviewed the alphabet, days of the week, and counting in Spanish and English. The operator engaged children in conversations by asking open-ended questions which led to rich conversations and positive interactions. The operator transitioned children to the complete personal care and hand washing routines located at the bathroom in the hallway of the home. Making good choices, sharing, and being safe were discussed with the children throughout the visit. Lunch was not observed however, the operator stated children would be served chicken soup with rice and vegetables, fresh bananas, and milk. A checklist was used to note the requirements monitored during today's visit. The Verification of Required Information for Operator and Additional Caregivers and the Operator’s Statement of Responsibility forms were completed. Two children’s files were monitored. The most recent monthly fire drill and outdoor inspection were both completed on May 2, 2025. The most current quarterly lockdown drill on file was completed on April 4, 2025. At the completion of the visit, the Visit Summary was reviewed with X. Flores. Record keeping is one of the most time-consuming tasks associated with administering a child care program. It is an important part of licensure. Review children's files often to ensure the program is maintaining accurate licensing records. Organization and completeness of records is important to ensure easy access to information in emergency situations. For the files that contained the checklist, the form was incomplete and not updated to reflect dates of submission for required documentation and files were missing required paperwork. As a reminder, current health assessments and immunization records are required to be on file within thirty (30) days of enrollment. A copy of the Children’s File Checklist was provided and discussed with you today. This resource from the DCDEE website will be useful in tracking required information by checking off the items on the form as they’re received and setting reminders or creating a task list for items due after the first day of enrollment. EPR/Ready to Go File: A violation was cited for the Ready to Go File during today’s visit. All licensed ECE programs are required to have a Ready to Go File to ensure that all necessary information is available during an emergency. I provided a copy of the Ready to Go File Facility Checklist to you which can also be found on the DCDEE website underneath Provider Documents.
