In the kitchen, the following hazards were observed in an unlocked drawer; loose wire, tools,nails and batteries. In the living room, the television stand had sharp edges which were not protected.
The educator does not have the required dog license for both dogs in the home.

Six of the children's records were reviewed, one child did not have a date of admission, one child needs an annual enrollment form and one child needs proof of vaccinations and lead testing.
Two wading pools in the outdoor space were observed to have sitting water. The pools were not emptied immediately after use or sanitized.
Two drawers in the kitchen had locks but were not functioning allowing access to super glue, loose charging wires and small objects.

All of the children's records were reviewed. Four children did not have a documented date of admission, three children did not have vaccination records, two children did not have a current physical and one child did not have their lead testing.
The children and provider were observed not to wash their hands after eating or diaper changing.

The attendance was observed to not have documented departure time for the children for 11/6/23 and did not have a documented arrival time for the children for today 11/7/23.
The edge of fireplace did not have a complete border covering the sharp edges. The gate at the bottom of the stairwell was not secured. In the kitchen, a sharp knife was accessible on the kitchen counter, there were several drawers which had the safety lock but they were not functioning allowing access to a cork screw, small plastic objects and plastic bags. In the living room, the entertainment center did not have the edges covered exposing sharp edges. The living room was not set up with space to accommodate a variety of activities for the children enrolled or to upgrade the amount of children.
A portable heater was observed to be in use during child care hours.
This change of location visit inspection recorded no violations or advisories.

Provider had two slides over 18 inches without an appropriate fall zone.
In the kitchen space, two cabinets were unlocked allowing access to plastic gloves, bags, trash bags and cleaning materials to the children. One shelf used for toys and books in the play area was no secured causing a tipping hazard. The shampoo in the bathroom tub was accessible and a plastic bag in the bathroom was accessible. In the master bedroom space, several charging wires were accessible causing s strangulation hazard.
The water temperature in the bathroom measured at over 120 degrees.

First aid kit was missing instant cold pack, non latex gloves, tape, tweezers, and the children's emergency cards.
There was a grill without a cover, electric wires, and candles on the table in the deck that leads to the outdoor play area. There was a truck bed hard cover with scaffolding with exposed corners.
There was a plug-in air freshener accessible to children in the bathroom.
