Cited finding

Children’s records were reviewed. One file was missing educator’s name for permissions on page 6 and 7 of enrollment packet. Two files need annual update forms. One file was missing parent signature dates on page 3 of enrollment packet. One file was missing the first page of the enrollment packet and the emergency card. One file did not have lead screening results. It was reviewed with the educator that she cannot have paperwork that indicates child care system name since she is no longer part of that system.
