The children’s records of the four children present were reviewed. Two children’s records did not have the date of admission documented and one child’s enrollment packet was missing sections of page 2 and 4.

The educator did not document an evacuation drill for the month of November.
On the kitchen counter a bottle of ibuprofen and a sharp knife were in reach of the children. The baseboard heater in the bathroom was rusted.
This investigation inspection recorded no violations or advisories.

In the kitchen, one drawer and one cabinet had a broken child lock which allowed access to a pair of scissors, screw driver and a blender. In the bathroom, the floor heater had rust. In the living room on the windowsill there were several dead bees.
A lawnmower was observed to be in the outdoor play space uncovered and accessible.
This investigation inspection recorded no violations or advisories.

There was one electrical outlet uncovered in the playroom. One of the cabinets in the kitchen was unlocked allowing access to hazardous items and there was a kitchen drawer unlocked allowing access to a knife. In the bathroom, the flat iron was unplugged but accessible to the children.

There were several items incomplete or missing from the children’s records upon review including,updated physicals, general permission forms were signed but not completed and children’s pediatrician information and their day and times of attendance. Provider to complete children’s record checklist.

There was a rusted metal rod in the backyard that places a hazard to children’s safety.
