In the bathroom the door under the sink was unlocked allowing access to cleaning materials. There was a make up bag accesible on the sink with make up items and a prescription medication bottle with medication inside. In the playroom an extension cord with electrical outlets was hanging from the chair accessible to the children.
There were two slides in the outdoor space over 18 inches which did not have a playground fall zone.
Provider did not have exit signs posted on either of her exits.
There were several items obstructing the back exit, not allowing a clear path to the exit.
