A bungee cord was observed to be hanging from a closet door in the daycare space as well as a second cord on the gate in the stairway, both in reach of the children.
The refrigerator temperature in the refrigerator read 46 degrees. The temperature must be within 32-40 degrees.
The children's records were reviewed, one child did not have a date of admission, three children did not have proof of lead testing, two did not have proof of vaccinations, one child had an expired physical, one child did not have proof of a physical and one child did not have a completed enrolment form.

The educator documented completing evacuation drills on a monthly basis from June 2025 through April 2026. The following dates were observed to be on either a Saturday or Sunday in which the educator confirmed they are not open; Saturday August 16, 2025, Sunday November 16, 2025, Saturday December 20, 2025, Sunday February 15, 2026 and Sunday March 15, 2026. The educator is not documenting the number of children present, meeting place or length of drill.
The educator is licensed for six, there were seven children present during the visit. The educator had a child scheduled to come in at 1 pm, allowing for eight children to be present.
The educator did not provide a written notification to the parents of the children enrolled regarding the change in the household composition.

The attendance reflected the child being signed in at 1 pm every time they attended the program. After a phone call with the child's parent, the parent confirmed the child is scheduled to attend the program at 1 pm and attends every Monday, Tuesday, and Thursday at 1 pm. This information contradicts the information the educator provided during the visit.
The educator is not documenting the date and time in/time out of the assistant when they are staffing the program.
