The following were observed: 1.Stairs not clear of obstructions. 2.A fan within children reach in one of the bedrooms on the second floor. 3.Medicine (Mucinex)within children reach on top of a table on the second floor in one of the bedrooms.

The following were observed: 1.Four of the children records did not list the assistant name on the general permission. 2.One of the children record did not list days and time of attendance for the child. 3.One of the children record did have a physical description or a picture of the child. 4. One of the children record did not specify where the child can go when off the premises.
The licensor observed that the educator is not documenting time of arrival or departure for the children enrolled.
The first aid kit did not have the following: family contact information, CPR mouth guard, and gauze rollers.

The educator did not have the list of allergies posted for one child allergic to cashews and pecans.
The licensor observed that the emergency plan is not complete when the following written detailed procedures are not included in the project: Meeting potential emergencies, including but not limited to missing children, the evacuation of children from the program in the event of a fire, natural disaster, loss of power, heat or hot water or other emergency.
The indoor space is not free from immediate safety risks when the following were observed: 1. Loose electrical cords in two bedrooms on the second floor. 2. Clothes iron on top of a table in one of the bedrooms on the second floor 3. Two uncovered heat radiators on the second floor in two bedrooms.
