At the time of the visit the following hazards were accessible and within reach of the children: sunblock, diaper cream, educator's bag handle hanging on edge for the children to reach, gate between classrooms was broken, hand lotion in children's bathroom, Aquaphor, and a wire longer than 6 inches.

At the time of the visit the designated educator was not able to communicate essential information to EEC during the visit while the Licensee was absent.
At the time of the visit the attendance in the program did not accurately reflect the number of children present in each classroom. The toddler classroom had 10 children signed in however 7 children were present and in the Preschool classroom 6 children were signed in however 9 children were present. The program was not following the toddler grouping of a max of 9 children at any time as 10 children were signed in on the attendance. While on the play space the educator was out of ratio as there were 7 toddlers with 1 educator not following the ratio for the toddler grouping.

At the time of the visit the program did not have evidence of three educators with the EEC 2.0 training.
At the time of the visit some of the children's records were expired.
At the time of the visit one educator's file was not on site and several of the staff files did not have all of the required information.
At the time of the visit the Licensee had expired fingerprints not making the BRC current.
At the time of the visit one educator who is not teacher qualified was in the Preschool classroom with 9 children alone.
At the time of the visit the program had all of the toddler shelves turned around facing the wall and in the preschool several of the shelves were turned around against the wall not allowing the children to access the equipment.
