The second exit leading to the backyard was not free and clear of clutter. The pathway to the door was not clear and the door was not able to open/close properly due to the clutter.
The following hazards were observed in the backyard: bottle caps that are a chocking hazard were accessible in the yard. There was no gate at the bottom of the stairs leading to the side door preventing the children from accessing the stairs. There was no covering around the brick for the bar area.
The educator has a slide in her yard that is over 18 inches tall and there is no ground surface covering. The educator removed the slide during the visit at the licensors request.
The Educator’s license was not posted in an area easily visible to parents
The Educator did not have current CPR or First aid. CPR expired 1/2026 and First Aid expired 2/2026
The Educator is taking the children off the premises for walks and to the library. A review of the children’s enrollment packets show that none of the children have written permission to leave the premises.
The licensor observed the following hazards: the bathroom cabinets were not locked and the following hazards were accessible: several bottles of face cream, deodorant, leave in conditioner and other hazards. There was also a plug in air freshener accessible.

The Educator's diaper surface is not in compliance. The Educator is using a blanket to cover the diaper surface which prevents the diaper surface from being disinfected.
The Educator did not sweep the kitchen floor after snack time. The children played with blocks on the kitchen floor after snack and several children were observed to be eating Doritos off the floor.
Although the Educator has started tracking attendance the attendance for the day was not complete. The Educator completed it during the visit at the licensor's request.
The Educator did not ensure that the pack and play used to nap an infant had a properly fitted mattress. The mattress was not installed properly. The infant was awake during the visit
The licensor observed the following; the cabinet under the kitchen sink was not locked and the following were accessible, Comet, 2 bottles of rubbing alcohol, a bottle of Windex, air freshener and a can of Raid. The bathroom cabinets had the following accessible: a can of air freshener, various face creams, Vaseline, a candle and an air freshener plug in accessible.

The Educator does not have enough appropriate nap materials for the ages of the children in care. The Educator has 1 pack and play, 1 bassinet and 5 cots. The cots are only approved for children ages 2 and older. During the visit, there were 3 children under the age of two and only 1 appropriate place for napping.
The following hazards were accessible inside: playroom 1: Swiffer, Dyson vacuum, two brooms. Playroom 2: there is a shelf that is a tipping hazard. In the kitchen there is a cord from the fish tank accessible. The following hazards were accessible outside: a ladder, a tarp, a dolly for moving, a heater, paint cans and vinyl spray.

The licensor observed a (REDACTED) to be drinking a juice box that was not theirs, that was picked up off the floor, the Educator removed the juice box at the licensors request. A toddler was playing in playroom 1 during lunch time and the Educator was in the kitchen unable to see the child at this time.

A review of six children's records show that: 5 general permissions need completed, 1 child needs an updated medical, 1 child does not have a medical on file, 1 child needs proof of a lead test, 1 child did not have a developmental history completed, 1 child did not have the parent visit notice signed, 2 children did not have the parent handbook acknowledgment signed, one parent wrote they did not receive it and 1 child needs authorized pickup person.
The Educator did not promote good hygiene when she used one child's nap blanket as a diapering surface, then used the same blanket on a different child a few minutes later at nap time.
The Educator did not use appropriate child guidance with a two year old child in care, when the child was upset the Educator did not respond appropriately. The Educator told the child that she would call the child's parent and have her sent home.
The Educator has not completed her attendance records since May 2025.
The Educator did not ensure there was a distance of at least two feet between children's faces while resting.
The Educator was using an unapproved outdoor space. The Educator was using her garage space for the children in care, it is not approved space and contained hazards: wires and a toolbox were accessible.
The educator is using the children’s nap blankets for diapering. The blankets are not able to be disinfected between uses and the blankets are then used for nap time. There is no surface that can be disinfected used for diapering at this time.
The licensor observed the Educator lay the (REDACTED) on a cot and give them a bottle to drink while going to sleep. Additionally there is a two year old in care who was placed in a high chair at lunch time but was not given lunch, he was only given milk.
