During the inspection of the outdoor play space, the licensor observed a slide with a fall height of 21 inches that did not meet the required fall zone of at least 6 feet in all directions from its perimeter and lacked the required impact-absorbing surface underneath.

Licensor reviewed all of the children's records. Several of the children's general permissions page was not fully completed, one child's emergency card was not completed and one child did not have an annual update form completed.
Provider’s CPR/First Aid expired on 11/20/23. The provider is scheduled to complete her course on 11/25/23. The program will be closed on 11/23 and 11/24 for holiday.
In the bathroom, a bottle of Lysol disinfecting spray was accessible. In the living room, a section of the baseboard heater was broken.
In the outdoor space, a slide which measures over 18 inches did not have an approved fall zone.

Safe Sleep
One child was missing an enrollment packet. One child was missing lead testing and some forms on the enrollment packet were missing on three children.
Educator had four children under the age of two.
Licensor was able to evacuate all of the children in approximately over 2 minutes.
The outdoor space was determined it is unable to be used due to being on a busy street and not having appropriate fencing. Please provide pictures once fencing is installed.
One child was viewed to be sleeping in the pack and play with no safe sleep concerns. Safe sleep course was completed 11/14/22. Educator did not have the safe sleep form signed by the parent. Please provide a picture of the signed letter.

The carbon monoxide/smoke detectors were tested and functioning. The diapering surfaces were appropriate. The first aid kit had all of the medical necessities but was missing the children’s emergency medical cards. The base heaters in the living room were also broken and need covers, there was a plastic bag with toys in which the educator removed during the visit. There was a fragrance plug in the living room which was accessible to the children. The cabinet doors in the kitchen were not locked making the items accessible to the children. The educator locked these doors throughout the visit.
All children’s records were reviewed, One of the children’s medical physical was missing, one child was missing a lead testing and one child did not have an enrollment packet. During the visit, the parent came and brought the child’s enrollment packet. One of the children needed an updated annual enrollment form.

Safe Sleep
The educator was unable to locate the evacuation log for 2022.
The first aid kit had all medical materials but was missing the children’s emergency contact cards.
Licensor asked educator to complete a fire drill during this visit. Educator was unable to evacuate all four children present. The carbon monoxide/smoke detectors were tested and functioning.
The outdoor space was determined it is unable to be used due to being on a busy street and not having appropriate fencing. There were also hazards in the back yard including a piece of gutter, broken wood, broken glass and a grill which is a tipping hazard.
During the visit, the educator laid down an(REDACTED)old infant in the pack and play for a nap with a blanket. Licensor discussed safe sleep regulations with them.

Four child care records records were reviewed and found to be incomplete missing pages that are not completed by parents and missing parental signatures.
The Licensor observed an outdoor slide inside the home. The slide measured more than 18 inches.
During today's visit, the Licensor observed mostly free play. The Educator was asked for her daily schedule which includes mostly free play. The Licensor has recommended that the Educator participate in the PDC coaching program.
The Educator has used an assistant whose certificate expired in February - 2021.
The Educator’s first aid has expired.

The Licensor observed steak knives in one kitchen draw and hazards such as plastic bags, garage bags, and plastic gloves under the kitchen sink that were accessible. The Licensor observed shampoos accessible in the bathroom.
