Licensor inspected all of the licensed, indoor space during the visit, as well as the outdoor playspace, the backyard. The following hazards were observed: ** KITCHEN/DININGROOM **- no hazards. ** DAYCARE ROOM 2 **- no hazards ** DAYCARE ROOM 1 **- no hazards ** LIVINGROOM **- 2 outlets were not covered ** BATHROOM **- laundry detergent needs to be up at least 4 ft high ** OUTSIDE **- no hazards
Educator randomly reviewed 4 of the children's records. One was missing the emergency card information, one had out of date signatures and no lead testing, one had out of date signatures and an out of date physical and lead testing, and the last one had no physical/immunizations.

All of the required items were observed in the 1st aid kit. However, the children's emergency contact forms were out of date.

Licensor inspected all of the licensed, indoor space during the monitoring visit, as well as the outdoor playspace, which is the back yard. The following hazards were observed: **KITCHEN/DININGROOM** - top drawer to the right of the dishwasher had skewers and other sharp objects inside, and the left drawer on the island had a sharp knife inside. **DAYCARE ROOM 2** - no hazards **DAYCARE ROOM 1** - no hazards **LIVINGROOM** - no hazards **BATHROOM** - no hazards **OUTSIDE** - trailer and large equipment off to the side need to be made inaccessible.
This follow-up visit inspection recorded no violations or advisories.

Both exits were easily accessible. However, there were no exit signs.
No hazards were observed on the deck. However, the paint on the deck was peeling.

Of these, 1 was complete and up to date, 1 was only missing lead testing, and the 3rd had no signature for general permission, basic transport on pg.6 of the enrollment packet. Please review all of the children's records and ensure they are all complete and up to date.

Licensor was able to inspect all of the licensed space during the virtual visit. The only hazard that was observed was that there some outlets that were not covered in the livingroom.

All of the required items were observed in the first aid kit. However, there was an expired [REDACTED] for a child with peanut allergy.
