The furniture in the Preschool and Toddler classrooms was unsecured and tippable.
A tile was observed cracked and lifting in the Pre-K bathroom causing a tripping hazard.

A review of three IHCPs showed the following: One student was missing an IHCP for asthma and two IHCP's were missing parent signature, what staff were trained and who trained them.
