Licensor inspected all of the licensed, indoor space during the renewal visit. The only hazard that was observed was a lamp that is on top of a shelving unit in the playroom, and the cord is hanging down. This cord needs to be secured because if a child pulled on it, the entire lamp could fall down from the high shelf.
The children were using a 24 inch plastic slide, which did not have a fallzone. The slide needs to either be removed, or a fallzone needs to be added.

The parents bring extra clothing for the children in bags everyday. Discussed that the extra clothes have to be stored at the program.

Educator stated she has conducted a fire drill every month. However, she could not locate her evacuation log during the visit.
Licensor randomly reviewed 3 of the children's records. Of these, 2 had out of date signatures in the enrollment packet, and the 3rd had an incomplete enrollment packet. Please review all of the children's records and ensure they are all complete and up to date.
The 1st aid kit was missing a CPR mouth guard, a thermometer, a roll of gauze, medical tape, and the children’s emergency contact information.
This monitoring visit inspection recorded no violations or advisories.

Licensor reviewed 4 of the children's records. Of these, 2 were complete and up to date, but the other 2 had out of date physicals.
Licensor reviewed 6 of the children's records. None of them were complete and up to date. Licensor observed the following: **1** - no signature on pg. 6 of enrollment packet under Basic Transport ** 2 ** - no signature under Basic Transport ** 3 ** - missing signature page (pg. 6) of the enrollment packet, annual update form does not have an accurate date on it (2021-2022 is written), no information on the child’s physical - there is only a vaccination list ** 4 ** - annual update form does not have an accurate date (2021-2022 is written), out of date IHCP [REDACTED] out of date physical ** 5 ** - out of date physical ** 6 ** - out of date physical ** Licensor also observed that there are 10 children enrolled in the program, but Educator only had records for 8 children.

Licensor reviewed 5 of the children's records and they all had out of date progress reports. Educator reported that she completed the progress reports over the summer but she gave them to the parents and did not keep a copy or receive them back from the parents. Discussed ensuring that a copy of the progress reports be kept in the child's record.
Licensor reviewed 5 of the children's records: 1 was complete and up to date, 1 just had an out of date physical, and the other 3 had out of date physicals as well as out of date signatures on the enrollment packet. Please review all of the children's records and ensure they are all complete and up to date.

There is one child in the program who has a [REDACTED]. This child’s IHCP is out of date.
Educator completed 18 hours of professional development over the last 3 years. This means that she needs to complete an additional 12 hours to meet the minimum requirement.
Licensor reviewed 5 of the children’s records. Of these, 4 had a complete and up to date progress report, but the 5th record did not have a progress report.
There was a bottle of bleach on the bathroom floor, behind the door. Combination smoke and carbon monoxide detectors hardwired throughout the home, tested and worked appropriately.
Licensor inspected all of the licensed, indoor space during the renewal visit, as well as the outdoor playspace. The following hazards were observed: ** PLAYROOM - small bin full of sunblocks and insect repellent on a shelf very low to the floor (about 18 inches high), easily accessible to the children ** BATHROOM - bottle of bleach on the floor ** OUTSIDE -fenced in back yard, no hazards.

Educator reviewed 6 of the children's records. Of these files, 3 were complete and up to date, 1 was missing a signature on Pg. 6 of the enrollment packet under Basic Transport, 1 had an overdue physical, and [REDACTED}. Please ensure that all of the children's records are complete and up to date and please also provide a plan as to how these files will remain up to date in the future.
CPR and First Aid Issued 3/10/18, CPR is expired. Licensor discussed that Educator has 7 days to retake a CPR class and update her certification.
Educator did not have a copy of her Parent Handbook and stated she would email it to Licensor, but Licensor has not received it.
The children's napping materials are stored in a manner in which they can touch each other. Discussed obtaining larger containers so that the blankets are not overflowing, in order to avoid cross contamination.
Educator stated it depends on the day if the children’s teeth are brushed or not. Discussed that regulations state the children's teeth need to be brushed on a daily basis.
Licensor walked through all of the licensed space and observed the following hazards: ** Basement playroom - large doll house is a tipping hazard ** Bathroom - cleaner spray bottle and empty plastic bags accessible to the children
