There was no evidence that two educators completed EEC Essentials 2.0 by June 30, 2025.
During a review of five children's records, the enrollment paperwork was expired for three children, and progress reports were only conducted once per year for five children.

During a review of the staff records checklist, it was found that two educators were missing required documentation, including EEC Essentials 2.0 training certificates and one educator was missing a current physical.
Several storage shelves in the classroom and the new shelves in the children's bathroom were tippable and not bolted to the wall creating a hazard that may cause an injury to children.
It was observed that a tall black shelf in the dramatic play area and several white shelves near the tables were tippable.
It was observed that the rest mats were not stored in a safe and sanitary manner. The mats were covered in sheets and stacked on top of each other.

The Individual Health Care Plan for a child with a seizure disorder expired on 12/3/2024.

One educator was alone with 11 children during rest time for more than 60 minutes, and the 2nd educator was not directly accessible if needed.
Children did not wash their hands before eating afternoon snack as required by 7.11(10)(c)(02) Educators and children must wash their hands at least before eating or handling food.
During a review of children’s records, it was found that two children were missing physicals and one child’s physical was expired.
During a review of staff files, it was found that none of the educators have current CPR certification.

Rest materials stored appropriately
It was observed that the rest mats with sheets are stored on top of each other, and the children's blankets are not stored in a safe and sanitary manner.
During a review of staff files, it was learned that the program director’s BRC is expired. The program director reported that her fingerprinting appointment is on 1/21/2022.

On the day of the visit there were bookcases that were not appropriately secured creating a tipping hazard. On the day of the visit there were several children's chairs stacked in front of an emergency exit.
From an inspection of the playground it was revealed that the playground was not well maintained and was cluttered with many large and small toys creating a safety hazard. The play structure had broken or rotting boards creating a safety hazard. The playground fence had several areas with exposed nails, broken stiles and wood rot creating a safety hazard.

During a review of staff records, it was found that two educators did not have an updated PQ Registry certificate and current First Aid and CPR and one educator did not have current health records or an EEC Essentials Training certificate.
During a review of four children's records, it was found that two children were missing a physical and immunizations records, and three children had expired enrollment paperwork.
The attendance records indicated that there were 10 children in attendance; however, there were 14 children present on the day of the visit.
The depth of the impact absorbing material did not meet the requirement of 9 inches of sand in the fall zones under and around the climbing structure near the garage.
There were three areas along the perimeter of the fenced in outdoor space with entrapment hazards. The distance between the bottom of the fence and the ground measured 5 to 7 inches.
