It was learned that the attendance form was inaccurate, and one child was not listed on the form.
The Licensor measured the impact absorbing material (mulch) in the fall zones under and around the swings and climbing structure in two locations, and it measured 7 inches. While digging, the Licensor also determined that the mulch is compacted in the fall zones.

During a review of the staff files, it was found that all staff were missing the staff orientation checklist and the BRC suitability letter, and that one staff was missing some of the EEC Essentials Training certificates.
