It was observed that the rest mats were not at least two feet apart or children’s faces at least three feet apart in the toddler room during nap time.

The written attendance records were inaccurate in two classrooms. The preschool group did not have the written attendance records with the group in the outdoor space.
The outdoor space contained hazards: 1) The depth of the impact absorbing material under and around the climbers measured 5 inches, instead of the required 9 inches. 2) The gate leading to the playground was open and not secured to keep children in the play area.
The emergency medications were not stored properly. Medications were in locked bags, and the educators in the preschool room did not have the key to the locked bag.

The educators did not wash their hands before putting on food prep gloves and serving snack brought from home to children.
Infection Control/Hand Washing - R Apr 2025
During an inspection of the outdoor playground, it was found that the depth of the impact absorbing material is 3-4 inches in the fall zones of the climbers.
The windows guards in the upstairs preschool room were positioned to create a head entrapment. The space between the windowsill and the window measured 4 inches and a head entrapment is between 3 1/2 and 9 inches. The folding tables in the infant room and gross motor room are tippable, not sturdy, and not installed. Two cords in the infant room are dangling at children’s reach. Plastic sheets are covering the cubbies and shelving units in the infant room creating a suffocation hazard.
When the Licensor arrived for the visit, it was observed that (4) preschool children were left unsupervised while they were sleeping on mats. In the other room (separated by a doorway), an educator was supervising (10) children sleeping an mats. This room is considered two classrooms, and a certified educator must supervise each group.

During the visit, it was found that an inhaler was not available at the program for a child with asthma.

It was observed that window guards were missing from the 2nd floor windows in the PreK classroom. Regulation 7.07(10)(h) states, "Windows and glass doors must be constructed, adapted, or adjusted through the use of window guards or other means to prevent injury to children."

Medication
On the day of the visit at least two children’s prescribed emergency medications were not readily available.
On the day of the visit not all individual health care plans were accurately and or entirely completed signed and dated as required. Not all staff were adequately familiar with at least one child’s IHCP. Not all staff were adequately trained to administer required treatment for at least one child’s possible symptom’s of a chronic medical condition.
On the day of the visit at least one child’s prescribed emergency medications were not maintained in its original container.

During the visit, the program did not adhere to appropriate staff and child ratios and an educator was working alone with 14 preschool children during rest time.
It was observed that an educator did not follow proper diapering procedures and the educator did not wash their hands or the child's hands after diapering.
Soap and water and bleach and water spray bottles were not labeled with the contents.

During an inspection of the playground, it was found that the depth of the impact absorbing material on the surface of the playground was 5 inches, and insufficient for the two climbing structures with slides.
During a review of children's records, it was learned that one child's IHCP was expired.
During the visit, it was found that a child's Epi-pen was expired.
