Progress Reports
A child who receives [REDACTED] medication at the program does not have a signed parent medication consent form. Educator also does not record a medication administration log.
One child has a progress report that was last completed in April 2024. Some children did not have any progress reports in their file. Written progress reports must be completed every three months for infants and children with special needs, every six months for toddlers and preschoolers, and once a year for school aged children (5 years and older).

2025 EEC evacuation log was reviewed, and last drill was conducted in August. Evacuation drills must be conducted with the children and logged every month.
Five children's files were reviewed during the visit. One child has an outdated enrollment packet and no annual update form in his file. The other children had current enrollment packets or annual update forms signed in October 2025. Physical/immunization documentation was up to date.
Exit signs were not posted as required. Plan for meeting potential emergencies is complete in the EEC parent handbook. Reviewed a written emergency evacuation plan.
Sleep mats are used for napping older children. Bedding for the children was observed to touching on a shelf in the closet. These napping materials must be stored separately.

The educator did not have an evacuation log available for review. Evacuation drills must be conducted with the children and logged every month.

Attendance Logs
Hot water temperature in the bathroom sink was 132 degrees. Discussed turning off the hot water to that sink or placing a clamp on it. Hot water where the children wash their hands must not exceed 120 degrees.
Observed a dangling blind cord in the bedroom window. Discussed ensuring the cords are never dangling as this poses a choking hazard.

The cabinets and drawers under the bathroom sink were not locked and contained various hazards. Cabinet and drawers must be locked, or hazards removed.
This investigation inspection recorded no violations or advisories.
Outdoor Playspace - Hazards/Porches and Decks
Backyard playspace. No new equipment added. Observed ladders next to the playspace. Discussed putting a tarp over them or surrounding with a play gate to prevent children access if they cannot be removed.
Indoor Space - free of hazards
The cabinet under the bathroom sink was not locked and contained hazards such as Listerine and cleaning sprays. Cabinet must be locked or hazards removed.
First aid kit is missing roller bandage, small scissors, and a thermometer. Emergency cards with photos of each child are kept on a keychain by the kitchen door.

Extra clothing for the children is kept in a wooden cabinet in the living room. Clothing is not separated. Discussed using large Ziploc bags or bins for each child.
