Space (health risks) - R Apr 2025
The educator has a fenced in backyard. Front yard is currently being used; however, the educator lives adjacent to a busy road and therefore the children cannot play in the front yard unless it is fully fenced in with a permanent fence that is at least 4 feet in height. Discussed the educator installing a garden fence that can be staked into the ground or removing hazards in the backyard so the children can use that space. Licensor will need to conduct a follow up visit to inspect the outdoor play space choice made by the educator. A small trampoline was observed on the front yard. The use of trampolines is prohibited in childcare.

10 hours of professional development are missing. Reviewed 20 hours of professional development training for this licensing cycle including 10 hours at the time of the visit. The educator uploaded only 10 hours of training in LEAD- Caring for Children with Autism (4 hours) and Nutrition in Early Childhood (6 hours). Educator must email licensor proof of completion of 10 more hours.

Indoor Hazards
Hot water temperature was tested at 122 degrees. Temperature must not exceed 120 degrees. Discussed putting a regulating clamp on or shutting off the hot water to the bathroom sink.
Observed multiple pairs of shoes and a backpack in the pathway of the front door. These items are a tripping hazard. The back door exit was not shoveled.
A cabinet in the bathroom was not locked and contains potential hazards such as cords, etc. Twin sized bed in the bedroom was partially blocking the door so it could not fully open. Small table and chairs used by the children had food remnants on it and had not been cleaned since last used.
This investigation inspection recorded no violations or advisories.
Developmental checklists reviewed were completed in 2022 and 2020. Written progress reports must be completed every three months for infants and children with special needs, every six months for toddlers and preschoolers, and once a year for school aged children (5 years and older).

Attendance is tracked in a notebook with the arrival and departure times of the children. At the time of the visit the attendance for the day was not filled in. Attendance must be accurate at any given point in the day.
Observed various hazards on the bathroom counter including bobby pins and lotions. Dangling electronic cords, Resolve cleaning spray, Coca Cola cans and random clutter observed in the living room. The educator's large purse/tote was accessible to children.

Reviewed three children's files. Physical/immunization documentation is up to date. EEC enrollment packet and annual update form are used. One child needs a current annual update form signed by the parent.
Two emergency card pages are outdated and must be renewed by parents.
First aid kit appears complete with bandaids, a CPR mouth guard, etc. Educator has purchased a touchless thermometer. Emergency card page is outdated and parent must complete a new form.

Observed the child care child in the educator's daughter's bedroom with the daughter and with the door closed. The bedroom is on the license, but the door must remain open at all times so the educator can adequately supervise what is happening inside the room.
Empty metal umbrella stand on the deck is on the deck right outside the back door and could pose a tripping hazard.
Shed being built with no doors and hazards inside are accessible. Uncovered fire pit with charred pieces of wood accessible. Little Tykes climber with no fall zones. Fall zone materials such as playground mulch must be installed 9 inches deep and 6 feet around or the structure must be removed or made inaccessible to child care children. Educator must email the licensor a picture of the corrective action taken. Peeling paint observed on a picnic table in the outdoor play space. The educator must email the licensor a picture of the corrective action taken.
Cleaning sprays were observed in the bathroom to be accessible to children on the roll of paper towels. Discussed moving the sprays to a high shelf in the bathroom. Cabinet under the sink in the bathroom is locked. Observed cleaning supplies accessible on the kitchen counter. Discussed storing them in the locked kitchen cabinet under the sink.

Observed a child napping behind a closed bedroom door. Doors must be left ajar and educator should check on the child at least every 15 minutes.
Supervision
Shed with hazards was not locked in the backyard. Black shelf in the playroom was not secured to the wall and poses a tipping hazard. Cosmetic, lotions and ear bud cords accessible in the bathroom. Educator's purse was accessible on the kitchen table. Large flat screen television in the living room was not secured to the wall or furniture. An electric fan was observed running on the floor in the bedroom where a child was napping.

Educator stated she has completed online Care courses. Discussed 10 hours of training required per year. Training certificates or other written proof of completion must be available for review by licensor during the visit.
Educator stated that she gives parents a copy of the EEC parent handbook. A complete handbook was not available for review at the time of visit. Discussed keeping a filled in copy accessible.
Educator could not find the refrigerator thermometer. Refrigerator temperature must always be between 32 and 40 degrees.
The back exit in the kitchen was not shoveled.
