Electric outlets covered, no frayed electrical cords
One of the second-floor bedrooms has a flat screen television that is not attached to the wall and a small oscillating fan plugged in on a nightstand next to the bed. The other second floor bedroom has medication that could be accessible to children and must be removed.
Stairs to the second floor were gated at the bottom. An attached gate is needed for the top of the stairs.
Electrical outlets were not covered in the second-floor bedrooms.

EEC evacuation log for 2022 is complete up to December. Educator has not begun a log for 2023. Evacuation drills must be conducted and recorded each month.

Gate surrounding the pellet stove in the living room is not attached to the wall or floor. Tension gate needed for the bottom of the deck stairs outside.
Outdoor Playspace - Hazards/Porches and Decks
Fully fenced in play space. Observed a plastic climber and 2 slides over 18 inches in height. Educator has turf underneath them.Required fall zones must be installed or this playground equipment removed. Fall zones must be 9 inches in depth and 6 feet around the structures. Educator will need to send the licensor a photo of the corrective action taken.
Changing table with a waterproof pad. Each child has their own cloth as a layer between them and the pad. The cloths are stored stacked on top of each other. These cloths must be stored not touching.
Fire pit in the yard was uncovered with sticks inside. Grill on the back deck was uncovered, but propane tank was not connected.Gate was missing from the bottom of the stairs and random items of clutter were clustered at the base of the stairs. Tall metal shelf in the playroom was not secured to the wall and poses a tipping hazard. Electrical outlet in the dining room was observed uncovered. Educator placed on a cover during the visit.
Progress reports are not being completed as required. Evaluations must be completed every three months for infants and children with special needs, every six months for toddlers and preschoolers, and once a year for school aged children.
