Educator uses the food program calendar to record daily attendance. August attendance was not filled in. Attendance must be accurate at all times with arrival and departure times of the children filled in as they occur.
Progress Reports
The educator has not been completing progress reports as required. Ages and Stages questionnaires reviewed were last completed in April 2023. Written progress reports must be completed every three months for infants and children with special needs, every six months for toddlers and preschoolers, and once a year for school aged children (5 years and older).
Staff Records
[REDACTED] Regular Assistant license expires 4/25/24. [REDACTED]works 3 days a week, at least 20 hours a week, with the educator. Educator does not have a file for [REDACTED] at the program. File should include training certificates, including first aid and CPR, a current medical form, etc. [REDACTED]is required to complete at least 10 hours of professional development training per year.

Attendance is tracked using the food program calendar. Attendance was not filled in when reviewed by the licensor. Daily arrival and departure times of the children must be accurate at any given point in the day and completed daily.
Food remnants and a big, serrated knife were observed on the kitchen table. Cabinet under the sink with cleaning supplies was not locked. Wasp killer spray was accessible on the mantel in the living room. Outlets observed with no covers in the living room.

Reviewed five children's files. Educator uses the EEC enrollment packet and annual update forms. Three of the children had out of date forms and one child's enrollment forms were not completed by the parent. Physicals and immunizations were up to date.

Observed a broken baseboard that had fallen down in the playroom. Two small trampolines are accessible in the fenced in play space. The use of trampolines is prohibited in child care.
